Epiglottic laryngoplasty for repair of blunt laryngopharyngeal trauma.

Two unique cases of blunt laryngopharyngeal trauma are presented in which identical injuries were sustained to the supraglottic larynx and hypopharynx. Examination of the patients included the use of magnetic resonance imaging to help define the extent of injury. A successful outcome was achieved in each instance by employing the reconstructive technique of epiglottic laryngoplasty to repair the mucosal defects.

Surgical histoanatomy for adduction arytenopexy using injection laryngoplasty.

BACKGROUND: In order to improve a large posterior glottal gap and/or aspiration, injections of augmentation substances should not only be administered at the mid-membranous vocal fold in the thyroarytenoid muscle, but also at the cartilaginous portion of the vocal fold to make adduction arytenopexy possible. METHOD: Ten adult human larynges were investigated using the whole-organ serial section technique. RESULTS: Vertical thickness of the posterior aspect of the thyroarytenoid muscle was relatively thin (3.4 ± 0.4 mm), especially in females (3.2 ± 0.3 mm). Consequently, care should be taken

Acquired idiopathic laryngomalacia treated by laser supraglottic laryngoplasty.

Laryngomalacia is the most common cause of stridor in neonates and infants, where the soft cartilages and tissues surrounding the upper larynx collapse inward during respiration. On the other hand, acquired idiopathic laryngomalacia in adults is quite rare, but should be borne in mind for differential diagnosis of upper airway distress. Allergic factors may cause airway distress, but have not been highlighted previously as the background of laryngomalacia. In this report, we describe two patients with acquired idiopathic laryngomalacia with reference to allergic rhinitis and high serum levels

Efficacy of additional injection laryngoplasty after framework surgery.

OBJECTIVES: The purpose of this study was to clarify the efficacy of additional injection laryngoplasty (AIL) after framework surgery (FS), while also trying to identify which patients or procedures were more likely to require AIL to obtain optimal results and to clarify why. METHODS: Fifty-two patients with unilateral vocal fold paralysis underwent FS (thyroplasty [TP], 23; arytenoid adduction [AA], 18; and AA with TP, 11). The numbers of patients who required AIL were calculated for each type of FS. The voice function after AIL was investigated. Thereafter, the width and the bowing ratios o

Epiglottic laryngoplasty after extended hemilaryngectomy for glottic cancer.

This paper reports the experience in the application of modified epiglottic laryngoplasty in 20 extended hemilaryngectomies for glottic carcinoma. The Kambic-Sedlacek-Tucker (K-S-T) technique using the epiglottis in glottic reconstruction can lead to early decannulation with an excellent airway, but a wide glottis formed after operation will increase the chance of aspiration and poor voice. Sphincteric function preservation of the glottis is a key point to success in partial laryngectomy. In order to prevent aspiration and to improve the quality of voice, the reconstructive technique as modifi

Centennial conference on laryngeal cancer papers. Primary laryngoplasty.

A new procedure is described for the surgical reconstruction of the larynx following total laryngectomy. Postoperatively, the voice was as good as that resulting from Asai's laryngoplasty. Aspiration of fluid into the trachea was minimal. The procedure may be briefly summarized as follows: 1. The lateral portion of both thyroid cartilages are left in situ. 2. The posterior laryngeal wall is formed with the superior horns of the same cartilages. 3. The inner surface of the reconstructed larynx is lined by mucosal flaps from the hypopharynx. 4. The reconstructed larynx is firmly sutured to the b

Time to Injection Laryngoplasty: A Quality Improvement Analysis.

OBJECTIVE: Vocal fold (VF) augmentation injection laryngoplasty with hyaluronic acid is an effective and rapid office-based treatment for glottic insufficiency. Early injection laryngoplasty, defined as augmentation within the first three months, can decrease the risk of hospital readmission, aspiration pneumonia, and potentially reduce the need for future permanent surgical procedures such as Type 1 thyroplasty. This study aims to identify the time to injection for first-time laryngoplasty, describe the demographics of patients requiring VF medialization, and investigate causes for delays in

Feminization of Transgender Women With Thyroid Chondroplasty and Laryngoplasty.

In recent years, gender affirmation surgery has broadened significantly from its previous focus on genital conformation only to include other areas of the body as well. As society reconsiders its interpretations of masculinity, femininity, and gender definitions, transgender individuals have realized that they will only be able to truly transition if they are recognized by the public in their chosen societal role. The authors review the literature and describe their own techniques for feminizing thyroid chondroplasty and laryngoplasty.

Medialization laryngoplasty with gore-tex: an animal study.

Vocal cord paralysis is a disease that can cause voice disability and aspiration problems. Expanded polytetrafluoroethylene (ePTFE; Gore-Tex) has been widely used in cardiovascular and plastic surgery; however, its biocompatibility and safety have not been established precisely in the larynx. In this study, the biocompatibility and safety of ePTFE used in vocal cord medialization in the rabbit larynx were assessed. Type 1 thyroplasty with Gore-Tex was performed on one side of the larynx in eight rabbits. A sham procedure was applied on the contralateral side of their larynx. The rabbits were s
