The sprained ankle. Diagnosis and management of lateral ligament injuries.

Ankle sprains are common injuries. They are often associated with residual disability with the ankle tending to give way on uneven ground (functional instability). Good management and rehabilitation require accurate diagnosis of the nature and extent of the injury, attention to the physical treatment and follow up with ankle strengthening and proprioceptive exercises to minimise the risk of functional instability.

[Lateral ligament ruptures of the upper ankle joint in children].

This report about 44 cases of fibular ligament lesions compares the results of two series of 22 cases, one investigated from 1978 to 1981 and the other from 1981 to 1983. Introduction of a special holding device in 1981 has resulted in more objective diagnostics. To prevent false indications for surgery the displacement under stress need to be compared with the healthy ankle. If there is 8 degrees difference an operation is indicated in children. Following the example of adult traumatology the introduction of a longitudinal epifibular incision and new techniques employed in ligament surgery ha

Differential diagnosis of ruptures of the lateral ligaments of the ankle joint.

Ruptures of the lateral ligaments of the ankle joint occur very frequently in young persons and this subject is therefore one which deserves more attention. Once a fracture or avulsion of a ligamentary insertion has been excluded the nature and extent of the ligamentary lesion should be precisely assessed. The only objective criterion of rupture of a ligament is demonstrable instability of the joint. Since the anterior talo-fibular ligament is always found to be torn immediately following supination-inversion injury to the ligaments the talar drawer sign will be demonstratable both clinically

The stress-tenogram in the diagnosis of ruptures of the lateral ligament of the ankle.

The stress-tenogram is a radiological technique for the investigation of injuries to the lateral ligament of the ankle, and combines the information previously provided by inversion and anterior stress radiographs, and the peroneal tenogram. It is designed to differentiate between stable and unstable ankles, and between isolated ruptures of the anterior talofibular ligament and combined tears of the anterior talofibular and calcaneofibular ligaments. A high degree of diagnostic accuracy has been confirmed at operative repair in a group of thirty-two patients.

Impact of Calcaneofibular Ligament Morphology on Recurrent Instability Following Arthroscopic Anterior Lateral Ankle Ligament Repair for Chronic Ankle Instability.

BACKGROUND: Arthroscopic anterior talofibular ligament (ATFL) repair for chronic lateral ankle instability (CLAI) has shown favorable outcomes; however, instability recurrence has been reported. CLAI is frequently associated with concomitant calcaneofibular ligament (CFL) injury; its effects on postoperative outcomes remain unclear. This study aimed to investigate the association between preoperative magnetic resonance imaging (MRI) findings of the CFL and postoperative recurrence. METHODS: This hypothesis-testing retrospective cohort study included 65 ankles in 60 patients (mean age, 34.0 ±

Hamstring muscle recovery following arthroscopic anatomic lateral ankle ligament reconstruction.

PURPOSE: Arthroscopic anatomic lateral ankle ligament reconstruction using gracilis autograft has become common practice in the treatment of chronic lateral ankle instability. While morbidity of hamstring harvesting has been extensively studied in anterior cruciate ligament reconstruction, no study has assessed the impact of this autograft on muscle recovery in ankle ligament reconstruction. The purpose of this study was to evaluate short-term hamstring recovery using isokinetic tests. METHODS: All patients operated on for chronic lateral ankle instability by arthroscopic anatomic ligament re

[Recognition of ankle sprain: not only lateral collateral ligament injury].

Ankle sprains are a common cause of morbidity in the general and athletic population. The majority of sprains involve the lateral aspect of the ankle, affecting the anterior talofibular ligament. However, the extent and categories of injury will depend upon the mechanism of injury. The ankle injury caused by ankle sprain is not only the injury of lateral collateral ligaments, but also chondral or osteochondral lesions of the talus, anterior ankle impingement, posterior ankle impingement and so on. In addition to the involvement of ankle joint, ankle sprain often leads to the injury of subtalar

The lateral fibulotalocalcaneal ligament complex: an ankle stabilizing isometric structure.

PURPOSE: Ankle lateral collateral ligament complex has been the focus of multiple studies. However, there are no specific descriptions of how these ligaments are connected to each other as part of the same complex. The aim of this study was to describe in detail the components of the lateral collateral ligament complex-ATFL and CFL-and determine its anatomical relationships. METHODS: An anatomical study was performed in 32 fresh-frozen below-the-knee ankle specimens. A plane-per-plane anatomical dissection was performed. Overdissecting the area just distal to the inferior ATFL fascicle was av

Return to Play Following Ankle Sprain and Lateral Ligament Reconstruction.

Ankle sprains are the most common musculoskeletal injury occurring during athletics. Proper initial treatment with supportive pain control, limited immobilization, early return to weight bearing and range of motion, and directed physical therapy are essential for preventing recurrent injury. Reconstruction of the lateral ligaments is indicated for patients with continued instability and dysfunction despite physical therapy. Return to athletic activity should be reserved for athletes who have regained strength, proprioception, and range of motion of the injured ankle. Athletes with a history of

Predictors of peroneal pathology in Broström-Gould ankle ligament reconstruction for lateral ankle instability.

BACKGROUND: Chronic ankle instability has a well-known association with intra- and extraarticular ankle pathologies, including peroneal tendonitis and subluxation. Patients with peroneal pathology are at risk for failure of conservative treatment for their ankle instability, thus identifying these patients is important and helps to guide management. There has been no literature looking at, in patients with chronic ankle instability, which associated ankle pathologies and patient characteristics are predictive of peroneal pathology. METHODS: A retrospective chart review was performed on all pa

Optimal suture anchor direction in arthroscopic lateral ankle ligament repair.

PURPOSE: In this study, the distance between the insertion point of the suture anchors and posterior surface of the fibula during arthroscopic lateral ankle ligament repair was investigated on computed tomography (CT) images. The hypothesis of this study was that there is an optimal insertional direction of the suture anchor to avoid anchor-related complications. METHODS: One hundred eleven ankles of 98 patients who had undergone three-dimensional CT scans for foot or ankle disorders without deformity of the fibula were assessed (59 males, 52 females; median age 25.5 years; age range 12-78 ye

Lateral ankle ligament anesthesia significantly alters single limb postural control.

Lateral ankle anesthesia has been used as a model to explore effects of ligament deafferentation related to ankle sprain on single limb postural control with conflicting results. Time-to-boundary (TTB) is a postural control measurement technique found to be sensitive in detecting subtle deficits in postural control in those with chronic ankle instability. The objective of this study was to determine the effects of lateral ankle ligament anesthesia on TTB measures of single limb postural control in healthy adults. Twenty-two healthy adults with no history of lower extremity injury within the pa

The anterior talofibular ligament-posterior talofibular ligament angle decreased after ankle lateral stabilization surgery.

PURPOSE: The angle between the anterior talofibular ligament (ATFL) and the posterior talofibular ligament (PTFL) is increased in patients with chronic ATFL injury. This study aimed to compare the AFTL-PTFL angle before versus after ankle lateral stabilization surgery, and to evaluate whether the ATFL-PTFL angle correlates with the ligament injury severity. METHODS: This retrospective study included 48 patients with mechanical ankle instability treated between 2016 and 2018. After arthroscopic evaluation, all patients underwent ankle lateral stabilization surgery comprising ligament repair (n

The effect of an ossicle of the lateral malleolus on ligament reconstruction of chronic lateral ankle instability.

BACKGROUND: Ossicles at the tip of the lateral malleolus are frequently found in patients with chronic lateral ankle instability (CLAI). However, the relationship between the presence or the size of an ossicle and the outcome of ligament reconstruction is poorly understood. Therefore, this study aimed to evaluate the effect of an ossicle at the tip of the lateral malleolus on ligament reconstruction in CLAI. MATERIALS AND METHODS: Seventy-four ankles with chronic lateral instability that received lateral ligament reconstruction using a modified Broström technique between January 2001 and Marc

Individual fascicles of the ankle lateral ligaments and the lateral fibulotalocalcaneal ligament complex can be identified on 3D volumetric MRI.

PURPOSE: Lateral ligament ankle sprains are common and the anatomy on imaging studies is vital for accurate diagnosis. The lateral fibulotalocalcaneal ligament (LFTCL) complex consists of the inferior fascicle of the anterior talofibular ligament (ATFL) which is connected by arciform fibres with the calcaneofibular ligament (CFL). The superior fascicle of ATFL is an independent structure that should be assessed individually. MRI evaluation of these distinct fascicles and the arciform fibres has not been described. The aim of this study is to identify the anatomical relationship of these compon
