Algorithm for the analysis of pre-extraction computed tomographic images to evaluate implanted lead-lead interactions and lead-vascular attachments.

BACKGROUND: The number of lead extractions is growing because of the greater population and increasing age of individuals with a cardiac implantable electronic device. Lead extraction procedures can be complex undertakings with risk of significant mortality, and vascular tears in the superior vena cava are of greatest concern. OBJECTIVE: The purpose of this study was to study whether a novel algorithm that analyzes pre-extraction computed tomographic (CT) images can determine the likelihood and location of lead-lead interactions and lead-vessel attachment within patients' venous vasculatures.

Comparative value of phosphate sources on the immobilization of lead, and leaching of lead and phosphorus in lead contaminated soils.

The mobility and bioavailability of lead (Pb) in soils can be mitigated by its immobilization using both soluble and insoluble phosphate (P) compounds. The effectiveness of insoluble P sources on Pb immobilization depends on their rate of dissolution which can be enhanced by phosphate solubilizing bacteria (PSB). In this study, the effect of soluble (potassium dihydrogen phosphate) and insoluble (rock phosphate in the presence and absence of PSB) P compounds on the immobilization of Pb, and leaching of Pb and P was examined using both naturally contaminated (SR soil: NH₄NO₃ extractable Pb: 28.

Evaluation of Lead Properties Affecting Complete Radiological Success in Transvenous Lead Extraction.

BACKGROUND: The insertion of cardiac implantable electronic devices has become increasingly common, resulting in a growing need for lead extraction, most often due to infection and malfunction. The majority of the research indicated that the lead indwell time was the most important factor in determining the procedural success of extraction. In this study, we examined the factors affecting complete radiological success. METHODS: A total of 192 lead extractions were performed on 102 patients at Başakşehir Çam and Sakura City Hospital. These procedures were compared based on lead indwell time an

Coronary sinus lead delay index for optimization of coronary sinus lead placement.

AIM: Optimization of coronary sinus (CS) lead position to the latest activated left ventricular (LV) area is important to increase cardiac resynchronization therapy (CRT) response. We aimed to detect the relationship between coronary sinus lead delay index (CSDI) and echocardiographic, electrocardiographic response to CRT treatment. METHODS: We prospectively included 137 consecutive patients with heart failure (HF) diagnosis, QRS ≥ 120 ms, left bundle branch block (LBBB), New York Heart Association score (NYHA) II-IV, LV ejection fraction (LVEF) <35% and scheduled for CRT (84 male, 53 female;

Riata lead failure with normal electrical lead parameters and normal fluoroscopic appearance.

Recently, insulation breach of the Riata lead raised a big concern. Management of these externalized leads has been addressed by professional organizations. However, what to do in patients with Riata leads without manifested failure is an ongoing clinical dilemma. Here, we present two clinical scenarios where the implantable cardioverter defibrillator system failed to deliver shock therapy in spite of having a new ICD generator and "appropriately functioning leads" as revealed by lead interrogation.

Lead-tin mirror formation from mixtures of red lead and tin sulphide.

Mosaic gold, tin (IV) sulphide, is a yellow pigment which was known in antiquity but whose use was superseded by other more easily obtainable yellow pigments by the Renaissance. The identification of mosaic gold residues in a burnished golden mirror decoration on a XIIIth Century Spanish polychrome statue is important in that the first reference to the use of mosaic gold in the European literature dates from the XIVth Century, although the use of this material in China had been recorded some time before. In this paper, Raman spectroscopy, XRD and SEM are used in the analysis of the conditions

12-Lead ECG Reconstruction Based on Data From the First Limb Lead.

PURPOSE: Electrocardiogram (ECG) data obtained from 12 leads are the most common and informative source for analyzing the cardiovascular system's (CVS) condition in medical practice. However, the large number of electrodes, specific placements on the body, and the need for specialized equipment make the ECG acquisition procedure complex and cumbersome. This raises the challenge of reducing the number of ECG leads by reconstructing missing leads based on available data. METHODS: Most existing methods for reconstructing missing ECG leads rely on utilizing signals simultaneously from multiple kn

Blood lead, bone lead and child attention-deficit-hyperactivity-disorder-like behavior.

BACKGROUND AND OBJECTIVE: Mounting evidence showed that lead exposure increased the risk of child attention-deficit-hyperactivity disorder (ADHD). Epidemiologic studies have typically used the blood-lead as a biomarker of lead exposure; blood-lead levels mostly reflect recent lead exposure. However, few studies have examined the relationship between bone-lead, a biomarker of cumulative exposure, and ADHD. Therefore, we aimed to compare the associations of bone-lead vs blood-lead levels with child ADHD symptoms and comorbidities. METHODS: A total of 164 children aged 3-15 years were enrolled d

Persistence of a pacemaker lead-like "ghost" 6 months after lead extraction.

Residual fibrous structures, referred as "ghosts", are occasionally encountered following lead extraction. Though typically detected in patients with cardiac device infections, their prognostic and therapeutic implications remain speculative. We describe a 62-year-old gentleman with history of sick sinus syndrome who presented atrial fibrillation with rapid ventricular response. The patient had history of transvenous lead extraction following pacemaker pocket infection 6 months earlier. In echocardiography, an echo-dense tubular lead-like structure was seen that followed the route of a pacer l

Management of Lead Encephalopathy with DMSA After Exposure to Lead-Contaminated Moonshine.

BACKGROUND: Lead encephalopathy is a severe manifestation of lead poisoning that can present with altered mental status and seizures and has been associated with illicit moonshine consumption. Lead encephalopathy has traditionally been treated using dimercaprol (British anti-Lewisite, BAL) and calcium disodium ethylenediaminetetraacetic acid (CaNa2EDTA). CASE REPORT: We describe a patient with lead encephalopathy related to lead-contaminated moonshine consumption, who was treated using dimercaptosuccinic acid (DMSA) due to a national shortage of CaNa2EDTA. A 66-year-old woman presented to a h

Succimer treatment during ongoing lead exposure reduces tissue lead in suckling rats.

There is a concern that oral treatment with succimer (meso-2, 3-dimercaptosuccinic acid, DMSA) can promote gastrointestinal lead absorption if not performed in a lead-safe environment. The scope of our investigation was to evaluate the efficacy of oral DMSA treatment during oral lead exposure on tissue lead in suckling rats. Six-day-old Wistar rats of both genders were divided into two groups-untreated (Pb) and treated (Pb + DMSA)-with 10 animals per group. Lead (as acetate) was given orally at a dose of 2 mg kg(-1) body weight day(-1) for eight consecutive days (total dose 16 mg kg(-1), i.e.

Home lead-work as a potential source of lead exposure for children.

Health examinations for lead poisoning were made on 62 family members from 15 families of homes carrying on lead work, such as quench-hardening in a molten lead bath and type-printing, as work at home. The most interesting findings concern the occurrence of cases with an unduly high lead absorption among children, but not among adult family members other than home lead-workers. The home environments of the children with an unduly high lead absorption represented contamination with housedust high in lead contents. The ingestion of the contaminated housedust by hand-to-mouth is probably responsi

Subjective symptoms after long term lead exposure in secondary lead smelting workers.

The prevalence of lead induced subjective symptoms was evaluated by a standardised questionnaire in a group of 96 workers employed between nine and 45 years in a secondary lead smelting works. A control group of 96 non-lead exposed subjects, matched for age and sex, were chosen from the Glostrup population study. Blood lead concentrations were in excess of 60 micrograms/100 ml in about 30% of the lead workers. Zinc protoporphyrin levels were found to be higher than 500 mumol/Hb in nearly 18% of the lead workers. The prevalence of fatigue, headache, sleep disturbance, and digestive symptoms (co

Reductions in blood lead overestimate reductions in brain lead following repeated succimer regimens in a rodent model of childhood lead exposure.

Although many studies have demonstrated the efficacy of succimer chelation in reducing blood and brain lead levels, the relative efficacy of the drug in the two tissues is less well understood. This issue is important because blood lead levels after chelation are used clinically to estimate reductions in the brain, the most critical organ in considering lead-induced neurotoxicity. The present study was designed to further investigate this issue, using multiple chelation regimens. Long-Evans rats were exposed to one of three lead exposure regimens from birth until postnatal day 40, followed by
