Percutaneous left atrial appendage closure: procedural techniques and outcomes.

Percutaneous left atrial appendage closure technology for stroke prevention in patients with atrial fibrillation has significantly advanced in the past 2 decades. Several devices are under clinical investigation, and a few have already received Conformité Européene (CE)-mark approval and are available in many countries. The WATCHMAN device (Boston Scientific, Natick, Massachusetts) has the most supportive data and is under evaluation by the U.S. Food and Drug Administration for warfarin-eligible patients. The Amplatzer Cardiac Plug (St. Jude Medical, Plymouth, Minnesota) has a large real-world

Managing Migrated Left Atrial Appendage Closure Devices in the Left Ventricle.

A 59-year-old woman with a pacemaker for paroxysmal atrial fibrillation developed left hemiparesis secondary to occlusion of the right middle cerebral artery. Following hybrid radiofrequency ablation and left atrial appendage occlusion, acute hypotension occurred due to device migration into the left ventricle. Optimal approach selection remains a significant clinical challenge.

Efficacy and safety of concomitant left atrial appendage closure and pulmonary vein isolation compared to left atrial appendage closure only - A meta-analysis.

BACKGROUND: Atrial fibrillation (AF) is associated with an increased risk of ischemic stroke, primarily originating from the left atrial appendage (LAA). Percutaneous LAA closure (LAAC) reduces appendage-borne thromboembolism, while pulmonary vein isolation (PVI) restores sinus rhythm. Observational data on the combined one-stop procedure are heterogeneous, and data on its risk-benefit profile remain scarce. Thus, a meta-analysis was performed to compare the efficacy and safety of combined LAAC+PVI versus LAAC-only. METHODS: A structured systematic search of PubMed, MEDLINE, Scopus and Web of

The impact of preoperative left atrial appendage thrombus on surgical outcomes in atrial fibrillation patients undergoing epicardial left atrial appendage closure.

BACKGROUND: The presence of a left atrial appendage (LAA) thrombus is traditionally considered a contraindication to epicardial left atrial appendage closure (LAAC) because of the risk of periprocedural embolization. This presents a significant challenge for atrial fibrillation (AF) patients with a high bleeding risk who are intolerant to long-term anticoagulant treatment. The safety and outcomes of epicardial LAAC in patients with a preexisting LAA thrombus remain poorly defined. METHODS: This retrospective cohort study included 108 AF patients who underwent epicardial LAAC at a single cente

Presence of A Left Atrial Appendage Thrombus After Successful Surgical Closure of the Left Atrial Appendage: A Case Report.

A 64-year-old African American woman presented for defibrillator threshold testing (DFTs) after a recent hospitalization for ventricular fibrillation terminated by her ICD. She had a known history of non-ischemic cardiomyopathy, atrial fibrillation, rheumatic mitral valve disease s/p mitral valve replacement and a redo after subsequent endocarditis. In preparation for the DFTs, the patient underwent a transesophageal echocardiogram (TEE) to rule our left atrial (LA) or LA appendage thrombus. Patient was found to have a surgically closed appendage. Within the body of the closed appendage, a mob

Interventional closure of the left atrial appendage for stroke prevention.

BACKGROUND: Transcatheter left atrial appendage (LAA) closure is an alternative therapy for stroke prevention in atrial fibrillation (AF) patients. However; real-world efficacy, safety and complications have yet to be investigated. We sought to determine the procedural outcomes and potential complications of LAA closure in routine clinical practice at a high-volume center. METHODS AND RESULTS: The study group comprised 179 patients (105 males; 72.7±9.0 years) with AF undergoing LAA closure at a single center in Germany. The rate of successful implantation was 98.9% (2 patients did not undergo

Migration of the left atrial appendage closure device to the aorta.

Percutaneous closure of the left atrial appendage may be indicated in patients with contraindications to anticoagulation therapy, for example, after recurrent gastrointestinal bleeding. It is an effective and safe procedure but is not without complications. We present a patient who presented with severe aortic insufficiency due to migration of the left atrial appendage closure device, which required urgent cardiac surgery for its removal.

[LEFT ATRIAL APPENDAGE CLOSURE IN A PATIENT WITH SYSTEMIC SCLEROSIS].

We report the clinical history of a 69 year-old female who suffered from systemic sclerosis and in whom we performed a percutaneous left atrial appendage closure due to recurrent gastrointestinal bleedings under anticoagulant therapy for chronic atrial fibrillation. We review the impact of scleroderma on the cardiac and digestive systems and discuss the issue of anticoagulation and its alternatives in uncommon clinical situations. We also describe the indications, technical aspects and potential complications of percutaneous left atrial appendage closure.

Left atrial appendage closure during mitral repair in patients without atrial fibrillation.

OBJECTIVE: Routine left atrial appendage closure during mitral repair in patients without atrial fibrillation (AF) is controversial. We aimed to compare the incidence of stroke after mitral repair in patients without recent AF according to left atrial appendage closure. METHODS: An institutional registry identified 764 consecutive patients without recent AF, endocarditis, prior appendage closure, or stroke undergoing isolated robotic mitral repair between 2005 and 2020. Left atrial appendages were closed via left atriotomy using a double-layer continuous suture in 5.3% (15 out of 284) patient

Percutaneous Closure of the Left Atrial Appendage in Atrial Fibrillation, Second Thoughts?

Life expectancy of the population is steadily increasing world wide. Consequently, the incidence and ultimately the prevalence of atrial fibrillation (AF) and it's sequelae will be rising proportionately. It is estimated that 3-5% of persons above 65 years of age have chronic AF, 30% of which will suffer at least one stroke. On the other hand, chronic AF is responsible for about 20% of all cerebrovascular accidents. Predictors of stroke in AF have been defined by the CHADS2 score, and in these patients, oral anticoagulation has been the cornerstone of thromboembolic disease prevention. Because

Multimodality imaging for pre-, intra-, and postprocedural guidance of left atrial appendage closure: a European Left Atrial Appendage Closure Club (ELAACC) expert consensus statement.

Left atrial appendage closure (LAAC) is an established alternative for stroke prevention in patients with non-valvular atrial fibrillation who are not suitable candidates for long-term oral anticoagulation. Imaging is central to procedural success and safety, yet variability in acquisition, measurement, and reporting contributes to inconsistent workflows and outcomes among centres. This expert consensus document from the European Left Atrial Appendage Closure Club provides a unified framework for pre-, intra-, and postprocedural imaging in LAAC with the aim of reducing variability, standardisi

Left atrial appendage closure after cryoballoon ablation in patients with atrial fibrillation.

BACKGROUND: Cryoballoon ablation (CBA) is effective for patients with drug-refractory symptomatic atrial fibrillation (AF). For patients with a high risk of stroke (CHA2DS2-VASc score ≥2), life-long oral anticoagulation therapy should be continued even after successful catheter ablation. We investigated the safety and efficacy of concomitant use of a second-generation CBA catheter for pulmonary vein isolation (PVI) and a left atrial appendage closure (LAAC) device in patients with AF. METHODS: We enrolled 27 patients (64.7 ± 6.3 years, 74% male, 63% paroxysmal AF, 37% persistent AF, 4.8 ± 1.4

Left atrial appendage closure in patients with intracranial haemorrhage and atrial fibrillation.

INTRODUCTION: The use of oral anticoagulants in patients with a history of atrial fibrillation (AF) and intracranial haemorrhage (ICH) is controversial on account of the risk of haemorrhagic stroke recurrence. This study presents our experience regarding the safety and efficacy of percutaneous left atrial appendage closure (LAAC), an alternative to anticoagulation in these patients. METHODS: We conducted a retrospective, single-centre, observational study. LAAC was performed in patients with a history of ICH and non-valvular AF. Risk of ischaemic and haemorrhagic events was estimated using th

Catheter ablation in combination with left atrial appendage closure for atrial fibrillation.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, affecting millions of individuals worldwide. The rapid, irregular, and disordered electrical activity in the atria gives rise to palpitations, fatigue, dyspnea, chest pain and dizziness with or without syncope. Patients with AF have a five-fold higher risk of stroke. Oral anticoagulation (OAC) with warfarin is commonly used for stroke prevention in patients with AF and has been shown to reduce the risk of stroke by 64%. Warfarin therapy has several major disadvantages, however, including bleeding, non-tolerance, interact

Percutaneous Left Atrial Appendage Closure: is there a Role in Valvular Atrial Fibrillation.

Atrial fibrillation, a chronic and highly morbid cardiovascular condition which affects over 33 million people worldwide, can be broadly categorized as valvular vs non-valvular in etiology. However, definitions of valvular atrial fibrillation have varied widely in the literature, and there is no clear consensus definition to date. Historically, patients with atrial fibrillation in the setting of rheumatic mitral valve disease have constituted a particularly high risk group for cardioembolic stroke, and for this reason many contemporary trials of pharmaceutical and device therapies for atrial f
