Effects of a family home-based intervention on global health and 24h movements: an exploratory family cluster-based approach to childhood obesity.

OBJECTIVES: This study aimed at (i) exploring the effects of a 12-month home-based family intervention on perceived global health and 24 h movement patterns among children with overweight/obesity (OW/OB) and their family members; (ii) identifying intra-family behavioral clusters and their influence on the intervention's effectiveness. METHODS: 142 families (n = 223 legal guardians) with at least one child with OW/OB were included. At baseline and 12 months, perceived health, sleep, physical activity (PA) and sedentary behaviors (SB) were assessed. RESULTS: Children with OW/OB showed poorer p

Involuntary Treatment of Minors with Severe and Enduring Anorexia Nervosa.

Anorexia nervosa is among the most lethal of all psychiatric illnesses and is increasingly prevalent in children and adolescents. There are limited treatment options specifically for youth with severe and enduring illness who decline treatment. Although treatment guidelines increasingly favor outpatient family-based treatments, there is a continued role for inpatient psychiatric treatment and involuntary commitment for high-risk patients. Providers may be reluctant to pursue involuntary treatment given its controversial nature, and differences in state's commitment laws complicate the developm

Enrolling HIV-positive adolescents in mental health research: A case study reflecting on legal and ethical complexities.

BACKGROUND: Adolescents living with HIV are an emerging group in the global HIV/AIDS epidemic. Mental health in this population affects HIV care, treatment, consequential morbidity and secondary transmission. There is a paucity of research regarding these youth in South Africa (SA), partly because section 71 of the National Health Act of 2003 (NHA) requires parental or guardian's consent. OBJECTIVE: To explore legal and ethical issues related to conducting adolescent mental health research in SA. METHODS: After obtaining a High Court order permitting research on minors aged <18 years with

Prevalence of enuresis and daytime urinary incontinence in children and adolescents with sickle cell disease.

PURPOSE: There is a known association between sickle cell disease and enuresis. However, the cause of this association is unclear. We tested the hypothesis that children with sickle cell disease would have more symptoms of overactive bladder than a control group. MATERIALS AND METHODS: Questionnaires were distributed to 155 legal guardians of children and adolescents 5 to 17 years old with sickle cell disease and to 100 legal guardians of a control group of children. RESULTS: Individuals with and without sickle cell disease were distributed uniformly regarding gender and age. A total of 50 p

Non-consensual sterilization of the adult with learning disabilities.

Community nurses may be asked for advice on sterilization operations for adults with learning disabilities by worried parents/carers. This article sets out the legal position advocated by the English courts. Sterilization for adults with learning disabilities is generally non-consensual. The courts cannot consent on behalf of the adult but can rule on the lawfulness of the operation. Cases need not be brought before the court when the operation is to be carried out to treat a specific menstrual malady and where sterilization is an incidental result. However, the Law Commission (1995) has set g

Treatment of divorcing fathers: social and psychotherapeutic considerations.

The author summarizes the psychotherapeutic issues most frequently encountered in his crisis treatment of 16 divorcing fathers for whom divorce was fundamentally a child-centered crisis. Fear and guilt over separation from children and wife, outrage and disappointment with the legal system, anxiety about the social and sexual readjustment to single life, and the need to develop strategies for continued child contact were most prevalent. The author's suggestions for the management of these issues include the therapist's familiarization with divorce as it affects and is influenced by intrapsychi

Decision-making authority and substance abuse treatment for adolescents: a survey of state laws.

PURPOSE: State laws concerning decision-making authority for voluntary inpatient substance abuse (SA) treatment of minors may be a potential barrier to appropriate treatment. We sought to identify and classify relevant laws related to the provision of voluntary inpatient SA treatment to adolescents 12 to 17 years (minors) as an exploratory assessment to improve understanding of how these laws might affect treatment decisions. METHODS: In summer 2006, we conducted a survey of statutes, regulations, and legal cases in the 50 states and the District of Columbia regarding the authority of parents

Examining the needs of youth in secure custody: why we need to "recreate the wheel" in Canadian juvenile justice.

The current study reviews the personal characteristics of 32 consecutive admissions to a secure custody centre in one southwest Ontario jurisdiction under the Young Offenders Act. Results indicated that there was considerable variability amongst the group regarding court history and the seriousness of the charge on which committal was made. Background history data suggested that the problems of youths committed to secure custody reflect considerable difficulties within families and school. The discussion questions whether the youths in this group are better served through the dispositions emph

Experiences of parents who receive a false-positive CK-MM screening for their newborn.

Duchenne muscular dystrophy (DMD) is a rare inherited, X-linked neuromuscular disorder that leads to a progressive decline in physical mobility, muscle atrophy, and premature death. Newborn screening (NBS) offers the potential for earlier diagnosis and earlier intervention, although NBS using the creatine kinase isoenzyme (CK-MM) assay may detect conditions other than DMD and lead to initial false-positive results. Early Check, a voluntary supplemental newborn screening study in North Carolina, screened 16,566 newborns for DMD over nearly 3 years. Parents and legal guardians of infants who rec

[Pregnant and mentally incompetent].

Two women, aged 31 and 36 years, under treatment for schizophrenia and psychotic episodes, respectively, became pregnant and decided to discontinue their medication. Thereupon the psychiatric disorder recurred and they refused--i.e. they were unable--to consent to a proposed treatment or investigation regarding their pregnancies. The necessary consent had to be obtained from a legal guardian. This was the husband in the first, urgent case. In the second case approval from the judge was obtained for the patient to be admitted against her will to protect her life and that of the foetus. In the N

Delays in diagnosis of nephrotic syndrome in children: A survey study.

BACKGROUND: Clinical experience suggests that childhood nephrotic syndrome is frequently diagnosed incorrectly, leading to delays in providing effective treatment. We hypothesized that the health care setting is an important determinant of diagnostic success, with implications for the patient and family health care experience. Our objectives were: (1) to characterize the relationship between diagnostic success and health care setting for the diagnosis of nephrotic syndrome, (2) to determine types and frequencies of incorrect diagnoses, and (3) to understand the burden placed on patients and fa

Evaluation of criteria for the diagnosis of asthma using an epidemiological questionnaire.

OBJECTIVE: To evaluate criteria for the diagnosis of asthma in an epidemiological survey. METHODS: Adolescents (13-14 years of age) and legal guardians of schoolchildren (6-7 years of age) in the city of Santo André, Brazil, completed the International Study of Asthma and Allergies in Childhood (ISAAC) standard written questionnaire. Affirmative responses regarding wheezing within the last 12 months, asthma ever, bronchitis ever (question added at the end of the questionnaire), as well as the overall ISAAC score above the predefined cutoff points, were considered indicative of asthma. [Treatment against the patient's will exemplified by electroconvulsive therapy : Clinical, legal and ethical aspects].

BACKGROUND: Severe affective and psychotic disorders may be accompanied by legal incapacity. If in this case the patient refuses treatment and in parallel there is a risk of serious damage to health, treatment can be carried out against the patient's non-autonomous will under defined prerequisites. Due to its good and partly superior effectiveness in the treatment of severe and pharmacotherapy-resistant affective and psychotic disorders, electroconvulsive therapy (ECT) is an important treatment option in such constellations. AIM: Description of the general principles and prerequisites of ther

Paediatric biobanking for health: The ethical, legal, and societal landscape.

Biobanks play a central role in pediatric translational research, which deals primarily with genetic data from sample-based research. However, participation of children in biobanking has received only limited attention in the literature, even though research in general and in clinical trials in particular have a long history in involving minors. So, we resolved to explore specific challenging ethical, legal, and societal issues (ELSI) in the current pediatric biobanking landscape to propose a way forward for biobanking with children as partners in research. Methodologically, we first establish

The Unbefriended Patient, Their Professional Guardians, and Clinical Liaison Psychiatry: The Challenging Ethics of Changing Goals of Care.

AbstractUnbefriended patients are those with decisional impairments who lack family or friends to serve as healthcare surrogates. When such patients cannot make decisions, the court typically appoints a professional guardian to make choices aligned with the patient's values and preferences. However, this case report illustrates ethical challenges that can arise when professional guardians disregard the patient's authentic wishes. In this case study, the 38-year-old unbefriended African American male patient expressed fears about traumatic resuscitation efforts and ultimately desired de-escalat
