Differential Expression and Correlation of High-mobility Group Box 1 and Vascular Endothelial Growth Factor Messenger Ribonucleic Acid across Leprosy Clinical Groups and Healthy Controls: A Cross-sectional Study.

BACKGROUND: Erythema nodosum leprosum (ENL) is an acute inflammatory complication of multibacillary leprosy. High-mobility group box 1 (HMGB1) mediates inflammatory signaling, whereas vascular endothelial growth factor (VEGF) regulates angiogenesis and vascular permeability. We compared whole-blood HMGB1 and VEGF messenger ribonucleic acid (mRNA) expression across leprosy groups and assessed their correlations. METHODS: This cross-sectional study included 146 participants: 35 with ENL, 36 with multibacillary leprosy without ENL, 38 with paucibacillary leprosy, and 37 healthy controls. HMGB1 a

Facial tuberculoid leprosy: case report.

Leprosy is a chronic, infectious, systemic disease caused by Mycobacterium leprae and is classified as paucibacillary and multibacillary types. It is contagious and has an insidious onset. Clinical presentation is characterised by hypopigmented skin lesions with reduced sensation. Presence of acid-fast bacilli in tissue specimens is regarded as a gold standard for diagnosis. Treatment is based on multi-drug regimens. We report a case of borderline tuberculoid leprosy in a 31-year-old woman.

Electrochemical magneto-immunoassay based on Mycobacterium leprae mimotope for serological diagnosis of leprosy.

Leprosy is an infectious disease classified as Neglected Tropical Disease (NTD) by the World Health Organization (WHO). Its diagnosis is challenging, relying on clinical symptoms and invasive procedures. Delays can cause severe physical disability, including hand, foot, and eye impairments. Herein, we describe the development of a magneto-immunoassay based on an electrochemical disposable microfluidic device for the serological diagnosis of leprosy. The assay makes use of magnetic particles (MPs) modified with Mycobacterium leprae mimotope as a probe to capture and separate target-specific ant

[Current status of leprosy].

Leprosy, known since antiquity, is a world infectious disease due to Mycobacterium leprae. The transmission is probably via nasal droplets. The clinical range, from tuberculoid to lepromatous leprosy is a result of variation in the cell-mediated immune response, with a chronic inflammation in skin and peripheral nerves. Diagnosis of leprosy is clinical with anesthetic skin lesion and skin smears detect acid fast bacilli. Besides the classification of patients due to the Ridley scale which is clinically useful, WHO proposed is a simple field classification based on the number of skin patches (p

CHANGING TRENDS OF LEPROSY IN ARMED FORCES.

A total of 651 cases of leprosy were hospitalised from Jan 1987 to Dec 1992. Each patient underwent haemogram, total and differential white cell count, urinalysis, liver function tests, skin slit smear for AFB and skin biopsy. Nerve conduction studies, electomyographic studies and nerve/nerve sheath biopsies were undertaken as and when indicated. These patients were managed with multidrug therapy. Paucibacillary (PB) leprosy accounted for 476 (73.1%) cases which comprised of indeterminate leprosy 90 (13.8%), tuberculoid leprosy 14 (2.2%), borderline tuberculoid leprosy 310 (47.6%) and neuritic

Assessment of serum level of interleukin-1b and interleukin-12 in leprosy: impact of previous Bacillus Calmitte Guerin vaccination.

Macrophages play an important role in attempt to eliminate mycobacteria, via production of cytokines, including interleukin-1, and interleukin-12. Bacillus Calmitte Guerin (BCG) vaccination, known to induce interleukin-1β in tuberculosis, was originally aimed at tuberculosis control, but it showed efficacy against leprosy. Our aim was to estimate serum levels of interleukin-1β and interleukin-12, in leprosy, and to assess the impact of previous BCG vaccination on their levels. Serum interleukin-1β and interleukin-12 p70 were estimated in 43 leprotic patients and 43 controls by enzyme-linked im

Advances in molecular techniques for detecting Mycobacterium leprae: A narrative review of Progress, challenges, and future directions.

Leprosy remains a neglected infectious disease where delayed diagnosis leads to disability and ongoing transmission, particularly in paucibacillary and pure neural forms. This narrative review summarizes molecular diagnostic approaches for Mycobacterium leprae, including polymerase chain reaction (PCR), isothermal nucleic acid amplification tests, clustered regularly interspaced short palindromic repeats (CRISPR)-Cas-based diagnostic systems, next-generation sequencing (NGS), microarrays, and biosensors. PCR remains the most established method, now expanded to multiplex, quantitative, and drop

Isolation and characterization of cultivable mycobacteria from leprosy skin.

Attempts were made to isolate cultivable mycobacteria from 129 biopsies/slit-skin scrapings from the skin of leprosy patients (73 multibacillary-BB/BL/LL and 56 paucibacillary-TT/BT/I) as well as 50 healthy controls. Among the 19 isolates obtained, 17 were from specimens from leprosy cases whereas two were from healthy controls. 14 of the 17 isolates were from multibacillary cases and three were from paucibacillary patients. The mycobacteria isolated were: M.scrofulaceum (4 = all LL cases); M.avium (3 = 2 from LL cases and 1 from healthy control); M.avium-intracellulare complex (1 LL); M.gordo

Pathogenesis of dry eye in leprosy and tear functions.

Seventy-five leprosy patients and an equal number of age- and sex-matched controls were examined for tear functions, using Schirmer's test and tear break up time (BUT). There was no statistically significant difference in the Schirmer's test, but the tear BUT showed a statistically significant lower value of < 10 seconds in multibacillary patients compared to paucibacillary patients. Leprosy patients with lagophthalmos and decreased corneal sensation showed a lower value of tear BUT which was also statistically significant. This study shows that even though the quantity of tears is not affecte

BACKGROUND: Leprosy has been a major public-health problem in many developing countries for centuries. According to the National Leprosy Elimination Programme report of March 2012, there were a total of about 0.13 million cases of leprosy in India, 9.7% of which were children. Numerous studies have investigated child leprosy amongst reported cases however, studies pertaining to proportion and characteristics of undetected childhood cases in the community are very few. AIM: To examine the clinical, bacteriological, and histopathological characteristics of newly detected child leprosy cases in

Single lesion multibacillary leprosy, a treatment enigma: a case report.

INTRODUCTION: Leprosy exhibits a wide spectrum of presentation, varying from the tuberculoid to the lepromatous pole, with immunologically unstable borderline forms in-between, depending upon the immune status of the individual. The clinical system of classification for the purpose of treatment includes the number of skin lesions and nerves involved as the basis for classifying the patients into multibacillary and paucibacillary. CASE PRESENTATION: A 20-year-old man belonging to a moderately endemic leprosy area in the Terai region of Nepal reported a large single, hypopigmented, well defined

Image Morphometric Analysis of B Cells and Plasma Cells in Erythema Nodosum Leprosum With Clinicopathological Correlation.

Erythema nodosum leprosum (ENL) occurs as an immunological complication of multibacillary leprosy (MBL). The pathogenesis of ENL is long considered to be a T-cell-mediated process. The role of B cells and plasma cells in ENL is not well described in the literature. Therefore, we investigated the B-cell and plasma cell infiltrates in the skin biopsies of biopsy-proven cases of ENL by immunohistochemistry and image morphometry and compared the result with paucibacillary leprosy and MBL. Moreover, we sought a correlation of the B-cell and plasma cell infiltrates with different clinical, hematolog

[Late diagnosis nodular lepromatous leprosy from the Peruvian Amazon, with development of a type 2 reaction].

Leprosy or Hansen's disease is one of the classic neglected diseases that still persists in Peru. The infection is caused by Mycobacterium leprae. The disease varies in a wide range of manifestations from tuberculoid (paucibacillary) to lepromatous (multibacillary) leprosy. We present the case of a 55-year-old man, farmer and wood extractor, from the Peruvian Amazon with old infiltrating cutaneous lesions on the face, neck, thorax, abdomen and extremities. The smear and biopsy examinations confirm the presence of acid-alcohol-resistant bacilli compatible with leprosy. It is concluded, with a l

Radiolabelled M. leprae resident in human macrophage cultures as an in vitro indicator of effective immunity in human leprosy.

Twelve strains of human derived, freshly extracted M. leprae maintained within human macrophages showed a 2.1-13.2-fold increase in the incorporation of 3H-thymidine compared to parallel cultures containing heat-killed bacilli of the same strain. The addition of antigen stimulated lymphokines from five paucibacillary, tuberculoid leprosy patients resulted in the inhibition of the uptake of the radiolabel by 49-87%. Minimal, or no, inhibition was noted in the presence of similar culture supernatants from five bacilliferous lepromatous leprosy individuals. The results indicate that in contrast t

Borderline tuberculoid leprosy misdiagnosed as erysipelas: A case report and literature review.

Leprosy is a disabling infectious disease caused by Mycobacterium leprae. This article reports the case of a 51-year-old woman who presented with a 20-day history of erythema, swelling, numbness, and pain in her right upper limb, which showed no improvement following initial treatment for erysipelas. Subsequently, based on a positive repeat skin smear for Mycobacterium leprae, the diagnosis was revised to leprosy, with definitive confirmation provided by histopathological examination and a positive acid-fast stain. This case highlights the critical diagnostic pitfall in paucibacillary leprosy,

A rare case of type 1 leprosy reactions following tetanus infection in a borderline tuberculoid leprosy patient and a literature review.

BACKGROUND: Type 1 leprosy reaction, also known as "reversal reaction", is related to cellular immune responses to Mycobacterium leprae antigens. The risk factors that trigger type 1 leprosy reactions are poorly understood. Leprosy with concurrent tetanus is rare, and there are no publicly available reports of a leprosy patient infected with tetanus that induced type 1 leprosy reactions. CASE PRESENTATION: A 56-year-old Chinese Han female presented to our hospital with symptoms of erythematous plaques and pain over her left upper limb for 2 days and foreign object sensation in her throat for

Type 1 reactions in leprosy--heterogeneity in T-cell functions related to the background leprosy type.

Nineteen each of paucibacillary borderline tuberculoid (BT) and multibacillary borderline borderline (BB)/borderline lepromatous (BL) leprosy patients undergoing type 1 reactions were compared with nonreactional stable patients of the appropriate leprosy type. In the BT reactional group, both phytohemagglutinin-induced and, more importantly, antigen-induced lymphoproliferation was reduced in 80%-90% of the patients. On the other hand, leukocyte migration inhibition was reduced in 40% and remained unchanged in the others. Suppressor-cell activity as evaluated by a costimulant assay was also red
