AIM: Leukapheresis is an invasive treatment modality used for hyperleukocytosis. Various drugs and fluids are used during the leukapheresis. Aging itself and associated factors such as increased comorbidity, decreased tolerance to drugs, increased drug toxicity give rise to the application of other treatment modalities in elderly patients. Treatment of acute leukemia in the elderly differs from young patients. Consequently, we assumed that outcome, effectiveness, and side effects of leukapheresis treatment used for acute leukemia patients with hyperleukocytosis may be different in elderly comp

Poor venous flow during leukapheresis may compromise cell collection; however, reliable predictors remain limited. We retrospectively analyzed 146 leukapheresis procedures involving patients with hematologic malignancies (n = 79) and healthy donors (n = 67) at two institutions, with continuous monitoring of peripheral blood flow using laser Doppler flowmetry (LDF). Poor venous flow, defined in this study as persistent low inlet pressure requiring a > 10% reduction in the draw rate to continue leukapheresis, occurred in 29.1% of patients and 26.9% of donors. In patients, multiple myeloma, lower

The American Society for Apheresis recommends leukapheresis as a category II recommendation for symptomatic hyperleukocytosis. Mortality in these patients varies between 8%-29%. However, no tool is currently available to assess the mortality risk following the procedure. This study aims to assess the factors associated with in-hospital mortality and develop a risk stratification tool consisting of clinically relevant variables to categorize patients based on their mortality risk following the procedure. The National Inpatient Sample Database (2016-2021) and ICD-10 coding were utilized to ident

Large volume leukapheresis (LVL) reduces the number of procedures required to obtain adequate peripheral blood progenitor cells (PBPCs) for autologous hematopoietic reconstitution. LVL involves the processing of > 15 L or 5 patient blood volumes using high flow rates. We report our experience with LVL evaluating its efficiency and adverse effects in 71 adult patients with hematologic or solid organ malignancies. All were mobilized with chemotherapy and granulocyte colony-stimulating factor (G-CSF). All collections used a double lumen apheresis catheter. Mean values per LVL were as follows: blo

Calcium supplementation to prevent or treat hypocalcemic symptoms during leukapheresis is not standardized. Institutions vary in practice. Within an institution, there is also nursing variability. Recognizing the need to harmonize our practices, the apheresis nurses at our institution launched a process improvement initiative to develop a standardized Citrate Reaction Grading and Treatment Chart for leukapheresis procedures. Based on nurse surveys, interdisciplinary collaboration, and clinical data, the chart enhances recognition and management of hypocalcemia-related symptoms, aiming to impro

A 64-year old woman with chronic lymphocytic leukemia (CLL) developed a prolymphocytic transformation with severe anemia, thrombocytopenia and deterioration of the general clinical condition. Neither treatment with chlorambucil and prednisolone, nor a combination chemotherapy (cyclophosphamide, vincristine, prednisolone) were successful. Splenectomy was not considered as the bone marrow presented signs of dyserythropoiesis. Encouraged by the results of therapeutic leukapheresis in different forms of leukemia, it was decided to start such therapy in our patient. During a seven-month period, thi

Forty-eight patients with fever greater than 38.3 degrees C for at least 24 hr despite broad spectrum antibiotics and an absolute granulocyte count less than 1000/microliter were randomly allocated to 4 days of granulocyte transfusions obtained by leukapheresis using filtration (n = 27) or gravity (n = 21) techniques, the latter permitting simultaneous nonmechanical collection of granulocytes and platelets utilizing hydroxyethyl starch as a sedimenting agent. Patient characteristics and dose of granulocytes obtained from both techniques were similar. Complete response to granulocyte transfusio

Immunotherapy, particularly chimeric antigen receptor T cells (CAR T)s, has changed the landscape of B-cell malignancy treatment and represents a promising approach to cancer therapy. The use of CAR T-based therapy in pediatric patients presents several critical issues such as the quality of the leukapheresis process and the treatment-related toxicity. Nevertheless, the experience with anti-CD19 CAR Ts in treating pediatric B-cell precursor acute lymphoblastic leukemia (BCP-ALL) demonstrated its feasibility and efficacy with a complete response rate greater than 80%. Although CAR T therapy is

This study investigated whether initial respiratory status in hyperleukocytic acute myeloid leukemia (AML), as defined by oxygen/ventilatory support, is (1) associated with early mortality and overall survival and (2) improved after leukapheresis. A retrospective chart review of 89 patients requiring leukapheresis was performed. White blood cell count (WBC) decreased from 153 (56-475) × 10(9)/L to 60 (17-259) × 10(9)/L after first leukapheresis (p < 0.01). Initial respiratory status was room air (n = 40), low (n = 31) or high flow oxygen therapy (n = 8) or mechanical ventilation (n = 10). As c

Background/Objectives: Therapeutic leukapheresis is widely used to manage hyperleukocytosis in hematologic malignancies, yet the factors determining its efficacy remain incompletely defined. We aimed to identify clinical, biochemical, and procedural determinants of leukapheresis efficacy in a single-center cohort. Methods: We retrospectively analyzed 125 leukapheresis procedures performed in adult patients with hematologic malignancies between March 2012 and June 2026. At our institution, emergency leukapheresis is performed before the initiation of cytoreductive therapy, so all first procedur

Chimeric antigen receptor (CAR) T-cell therapy is an individualized immunotherapy that genetically reprograms a patient's T cells to target and eliminate cancer cells. Tisagenlecleucel is a US Food and Drug Administration-approved CD19-directed CAR T-cell therapy for patients with relapsed/refractory (r/r) B-cell acute lymphoblastic leukemia and r/r diffuse large B-cell lymphoma. Manufacturing CAR T cells is an intricate process that begins with leukapheresis to obtain T cells from the patient's peripheral blood. An optimal leukapheresis product is essential to the success of CAR T-cell therap

While extensive data demonstrated that plerixafor improves stem cell harvest in difficult-to-mobilize patients, economic concerns limit a broader application. We retrospectively assessed the effect of an early plerixafor rescue regimen for mobilization in patients with multiple myeloma. Patients were intended for high-dose chemotherapy followed by autologous peripheral blood stem cell transplantation (ABSCT) and therefore received cyclophosphamide-based mobilization chemotherapy and consecutive stimulation with granulocyte colony-stimulating factor (G-CSF). Fifteen patients with poor stem cell

In peripheral blood progenitor cell (PBPC) collections from patients with solid tumour or haematological malignancy, monocytes were separated into two subpopulations. The majority of monocytes expressed CD14 at a high density without CD16 antigen (the CD14+CD16- monocytes). The remaining monocytes co-expressed CD14 and CD16 (the CD14+CD16+ monocytes). These CD14+CD16+ monocytes amounted to 20.6 +/- 15.8%, while those in peripheral blood (PB) obtained from healthy volunteers were 7.3 +/- 3.1% (P < 0.05). When subdividing the CD14+CD16+ monocytes into CD14brightCD16dim and CD14dimCD16bright cell

Hyperleukocytosis is a hematologic crisis caused by excessive proliferation of leukemic cells and has a relatively high early mortality due to a series of severe complications needing prompt and effective interventions. Leukapheresis performed using apheresis equipment to separate leukocytes from peripheral blood at the same time returns autologous plasma, platelets and erythrocytes to the patient and is applied clinically for treatment of hyperleukocytosis with symptoms of leukostasis. The aim of this study is to analyze the safety and effectiveness of therapeutic leukocytapheresis to allevia

Severe pertussis complicated by refractory hyperleukocytosis poses a critical therapeutic challenge in young infants, particularly when initial exchange transfusion fails to mitigate escalating leukocytosis. We report a 1.5-month-old, 4.3-kg infant with severe pertussis and marked hyperleukocytosis (white blood cell [WBC] count 57.52 × 109/L) unresponsive to conventional exchange transfusion. Paradoxically, WBC count increased to 48.07 × 109/L with lymphocyte count rising to 12.69 × 109/L post-exchange transfusion, necessitating emergency leukapheresis. After leukapheresis, the WBC count decre

INTRODUCTION: Hyperleukocytosis is associated with inferior clinical outcomes and early mortality. Leukapheresis provides a rapid reduction in the cell mass, which can cause complications known as leukostasis. This study evaluated the impact of leukapheresis versus standard medical therapy on acute short-term complications, specifically tumor lysis syndrome (TLS) and acute kidney injury (AKI).
METHODS: This single-center retrospective cohort study included 50 pediatric patients with hyperleukocytosis, admitted to the pediatric intensive care unit (PICU) between 2008 and 2023. Patients were gr

Leukapheresis is a potentially life-saving therapy for children with symptomatic hyperleukocytosis. However, the standard centrifugation-based approach exposes pediatric patients to significant complications due to its large extracorporeal volume, high flow rates, and considerable platelet loss. Here, we tested whether performing cell separation with a high-throughput microfluidic technology could alleviate these limitations. In vitro, our microfluidic devices removed ~85% of large leukocytes and ~90% of spiked leukemic blasts from undiluted human whole blood, while minimizing platelet losses.

BACKGROUND: The CAR-HEMATOTOX score has been associated with hematologic toxicity after chimeric antigen receptor T-cell therapy, but it is usually assessed shortly before lymphodepletion, when hematopoietic reserve may already be affected by bridging therapy. We investigated whether the CAR-HEMATOTOX score assessed at leukapheresis, before bridging therapy initiation (Pre-CAR-HEMATOTOX), could predict delayed platelet recovery after idecabtagene vicleucel (ide-cel) in relapsed/refractory multiple myeloma.
PATIENTS AND METHODS: We conducted a single-center retrospective cohort study of consec
