A case of hairy cell leukemia variant.

Hairy cell leukemia variant (HCLv) is a rare B-cell chronic lymphoproliferative disorder with features of the classic HCL but presenting some particularities, a poor response to conventional therapy of classic HCL and a more aggressive course of disease with shorter survival than classic HCL. We present a case of a 52-year-old man hospitalized in July 2012 in the Clinic of Hematology of Craiova, Romania, having splenomegaly, leukocytosis with lymphocytosis, anemia and thrombocytopenia, without monocytopenia, which exposed, in the peripheral blood and bone marrow cells, intermediate morphology

[Recurrent somatic mutation in hairy cell leukemia].

Hairy cell leukemia is a mature B-cell non-Hogkin lymphoma characterized by unique clinical, morphological and immunhistochemical features. Patients with hairy cell leukemia usually present with splenomegaly, progressive pancytopenia and a relative indolent clinical course. The diagnosis does not always indicate immediate treatment, as treatment depends on the clinical stage of the leukemia. Asymptomatic disease without progression requires a watchful waiting policy, while other categories usually need treatment. The treatment of choice is purine nucleoside analogues (pentostatin, cladribine)

My treatment approach to hairy cell leukemia.

Hairy cell leukemia (HCL) is a rare chronic lymphoproliferative disorder characterized by circulating B cells with cytoplasmic projections, pancytopenia, splenomegaly, and a typical flow cytometry pattern. Recently, the BRAF V600E mutation was uniformly identified in one HCL series, which may provide insights into the pathogenic mechanisms. The disease course is usually indolent but inexorably progressive. Patients require treatment when they have significant cytopenia or occasionally recurrent infections from immunocompromise. In the mid-1980s, interferon replaced splenectomy as the initial t

Underexpression of RhoH in Hairy Cell Leukemia.

The cause of hairy cell leukemia (HCL) is unknown. Current treatments seem effective only for a limited period of time. In addition, a significant proportion of patients remain refractive to all treatment options. These considerations indicate the need to develop alternative therapeutic strategies for HCL. Here, we report that HCL is characterized by underexpression of RhoH. In vitro reconstitution of RhoH expression inhibits the aberrant adhesion and transendothelial migration that drives disease pathogenesis. In an in vivo model of HCL, RhoH reconstitution limits malignant progression and pr

Paraneoplastic vasculitis associated with hairy cell leukemia.

Hairy cell leukemia (HCL) is a rare B-cell lymphoproliferative disease, accounting for about 2-3% of all leukemias in adults. The skin lesions were described in about 10-12% of patients. Vasculitis in lymphoproliferative disease is relatively uncommon and may predate the diagnosis of lymphoproliferative disease. A 54-year old female with one month history of general symptoms and sudden onset of maculopapular exanthema on the skin, suffered from anemia, leukopenia and thrombocytopenia. Examination of the skin biopsy revealed lymphocytic vasculitis. Immunophenotyping of the skin biopsy revealed

TIA-1 expression in hairy cell leukemia.

We measured T-cell intracellular antigen-1 (TIA-1) expression in neoplastic cells from patients with hairy cell leukemia. Five of nine cases were positive for cytoplasmic TIA-1, with a small, dot-like, granular expression pattern. However, neoplastic cells were granzyme B- and perforin- negative in all cases. Other positive markers were CD20 in 9/9 cases, CD19 in 9/9 cases, DBA44 in 8/9 cases, LeuM5(CD11C) in 8/9 cases, IL-2R(CD25) in 7/9 cases, CD103 in 7/9 cases, FMC7 in 6/9 cases, and tartrate- resistant acid phosphatase in 5/7 cases. We also analyzed TIA-1 expression in 94 B cell lymphomas

Remission of hairy cell leukemia without treatment.

A patient is described with hairy cell leukemia, in whom a remission occurred spontaneously or in association with Legionnaires or subsequent HIV infection. Possible pathophysiological mechanisms are reviewed. Currently it is seventeen years since the diagnosis of hairy cell leukemia and fourteen years since the infusion of HIV infected blood. Hairy cell leukemia has yet to return and HIV infection has not progressed to AIDS.

[Biology and therapy of hairy cell leukemia].

In the paper recent advances in etiology and pathogenesis of hairy cell leukaemia (HCL) are reviewed. Role of human T-cell leukaemia virus II (HTLV-II) and Epstein-Barr virus as well as benzene and other environmental factors in the etiology of this leukaemia is presented. The new achievements in diagnosis including electron microscopic, cytogenetic and immunologic studies are discussed. A rare variants of HCL are described. The special attention is paid to the modern approach in treatment including alfa-interferon, 2'-deoxycoformycin and 2-chlorodeoxyadenosine.

[Alpha interferon treatment in hairy cell leukemia].

Since 1984 alpha-interferon (alpha IFN) has been the treatment of choice in cases of hairy-cell leukaemia. However, eight years' experience shows that, although a large majority of the patients are improved, they are not cured. Thus, a new type of cytostatic drug, perhaps capable of cure as well, may replace alpha-IFN for the treatment of hairy-cell leukaemia in the future.

[Hairy cell leukemia: pathomorphology, cytology and cytochemistry].

In 10 cases of hairy cell leukaemia histological, cytological, cytochemical and in some of the cases immunocytological investigations were carried out. In addition to clinical, cytological and cytochemical findings arising suspicion of hairy cell leukaemia, in three cases the multifocal reaction of acid-phosphatase appeared to be tartarate sensitive (at the concentration of L+-tartaric acid--0,75%), indicating in accordance with findings of Burns et al. (9), that tartarate resistency of the acid-phosphatase reaction ("isoenzyme 5" Katayama és mtsai, 24) has no absolute diagnostic significance.

Almost a Near Miss: Hairy Cell Leukemia.

Hairy cell leukemia (HCL) is an infrequently encountered chronic B-lymphocyte hematological malignancy, which is found to be more prevalent in males. HCL can present with a myriad of nonspecific symptoms involving the reticuloendothelial system. Usually, patients are diagnosed after an incidental finding of pancytopenia. In the majority of cases, HCL follows an indolent course, and many patients remain asymptomatic. Treatment with nucleoside analogs is the first line of treatment and is indicated for patients with severe anemia, thrombocytopenia, neutropenia, or severe systemic symptoms. Here,

Patients with relapsed/refractory hairy-cell leukemia.

BACKGROUND: Hairy cell leukemia (HCL) is a rare chronic B-cell neoplasm with good long-term prognosis. First and second-line therapies include purine nucleoside analogues (PNAs) and rituximab, but until recently, limited alternative options were available for patients with two or more relapses. AIM: The aim of this study is to describe our real-life experience with HCL patients in third and fourth-line therapies. METHODS AND RESULTS: Data from 49 HCL patients with two or more relapses, including 16 patients with three or more relapses, were collected from the French retrospective HCL cohort

The Biology of Classic Hairy Cell Leukemia.

Classic hairy cell leukemia (HCL) is a rare mature B-cell malignancy associated with pancytopenia and infectious complications due to progressive infiltration of the bone marrow and spleen. Despite tremendous therapeutic advances achieved with the implementation of purine analogues such as cladribine into clinical practice, the culprit biologic alterations driving this fascinating hematologic disease have long stayed concealed. Nearly 10 years ago, BRAF V600E was finally identified as a key activating mutation detectable in almost all HCL patients and throughout the entire course of the diseas

CD27-positive hairy cell leukemia-Japanese variant.

We report a very rare case of a 45-year-old Japanese male patient with hairy cell leukemia-Japanese variant (HCL-JV) expressing CD27. The patient showed a high number of abnormal peripheral lymphocytes, thrombocytopenia, and severe splenomegaly but no lymphadenopathy. Histology of the resected spleen showed small-sized lymphoma cells diffusely infiltrating the red pulp without follicle formation. By immunohistochemistry, lymphoma cells were negative for CD3, CD5, CD8, CD10, CD34, cyclin-D1, and annexin A1 but positive for CD20 and BCL2. BRAF V600E mutation was not observed. Bone marrow aspirat

Hairy cell leukemia: a 'hair-raising' update.

Hairy cell leukemia (HCL) is an uncommon low grade B cell leukemia that is marked by pancytopenia, splenomegaly, and characteristic cytoplasmic hairy projections. The current standard of care is treatment with purine analogs, like cladribine or pentostatin, which provide a high complete remission rate with a median duration of response of 5 years. Many patients who show initial remission will relapse, and others with refractory disease may show no response. The discovery of the BRAF mutation has created a therapeutic target exploited by oral inhibitors like vemurafenib and dabrafenib. Targeted

Hairy-cell leukemia with inaugural joint manifestations.

Hairy-cell leukemia is a chronic B-cell malignancy seen in adults. The presenting manifestations consist of splenomegaly, pancytopenia, and characteristic monocyte depletion. The presence in peripheral blood or bone marrow of hairy cells exhibiting the CD19(+) CD20(+) CD25(+) CD11c(+) phenotype establishes the diagnosis. Rarely, patients present with inaugural joint manifestations related either to the hematological malignancy or to immune dysfunction. The resulting polymorphic polyarticular symptoms may cause diagnostic wanderings. Monocytopenia is a valuable diagnostic clue. The identificati

Hairy cell leukemia variant: fact or fiction.

Hairy cell leukemia variant (HCL-V) is a poorly described, rare B-cell lymphoproliferative disorder typically positive for CD103 and CD11c, while lacking CD25. Splenic marginal zone lymphomas (SMZL) also have this unusual phenotype in 15% to 25% of cases, have other overlapping clinical or morphologic features, and are more common than HCL-V. The purpose of our study was to better characterize HCL-V and determine whether most cases could be distinguished from SMZL. Cases with an HCL-V phenotype were identified from our flow cytometry service, and 10 were selected for further study based on bon
