One important step in lumbar discotomy is the safe penetration and exact partial removal of the Lig. flavum. This is a crucial prerequisite for the selective and gentle removal of prolapsed and sequestrated intervertebral disc tissue. Extensive lancing of the Lig. flavum should be avoided, to minimize intraoperative and postoperative complications, such as injuries to the nerves and the Dura mater, instabilities, bleeding and scarring. As there has up to now been insufficient information on the anatomy of the Lig. flavum especially regarding microdiscotomy, an analysis of the Ligg. flava L2/L3

To clarify the effect of mechanical stress on hypertrophy of the ligamentum flavum (LF), we compared the thicknesses, histological findings, and total collagen content of 161 lumbar LF in two groups defined by radiographic signs. LF with one or more signs of spondylolisthesis, disk degeneration, vacuum phenomenon, traction spur, or facet joint osteoarthrosis were considered degenerative; all others were considered nondegenerative. Thickness was measured by computed tomographic scan, and total collagen content by Woessner's method. We found that the ligament was thicker in the degenerative grou

BACKGROUND: The ligamentum flavum (LF) is a crucial spinal component that, under pathological conditions, can compress the spinal cord and adjacent segments, significantly impacting patient quality of life and function. Despite extensive research and advancements in the field, no bibliometric analysis has been conducted. This study applies bibliometrics to examine the LF research landscape.
METHODS: We searched the Web of Science Core Collection (WoSCC) database for studies on LF from January 1, 1975, to March 1, 2024. Bibliometric analyses were performed using VOSviewer, the R package "Bibli

BACKGROUND Ligamentum flavum (LF) hypertrophy is a key contributor to spinal pathologies such as lumbar and cervical disc herniations and spinal stenosis. Smoking, as a modifiable lifestyle factor, is implicated in systemic inflammation and oxidative stress, potentially exacerbating LF hypertrophy. This study aimed to compare the expression of alpha-1 antitrypsin (AAT), interleukin-1 beta (IL-1 beta), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-alpha) in the lumbar ligamentum flavum from 27 smokers and 31 non-smokers. MATERIAL AND METHODS LF tissue samples were collected during

INTRODUCTION: Ligamentum flavum haematoma (LFH) is an extremely rare entity, found mostly in the lumbar and thoracic ligamentum flavum and seldom in the cervical ligamentum flavum. Cervical LFH can cause paralysis in patients. We describe a case of LFH in the cervical spine that accepted surgical treatment.
CASE PRESENTATION: A 70-year-old man with incomplete spinal cord injury presented with sudden paralysis of his left limbs for 10 days and hemi-hypaesthesia below the level of the right clavicle. Magnetic resonance imaging (MRI) showed a space-occupying lesion in the left ligamentum flavum

Hypertrophy of the ligamentum flavum (LF) is a major cause of lumbar spinal stenosis (LSS), and the pathology involves disruption of elastic fibers, fibrosis with increased cellularity and collagens, and/or calcification. Previous studies have implicated the increased expression of the proteoglycan family in hypertrophied LF. Furthermore, the gene expression profile in a rabbit experimental model of LF hypertrophy revealed that biglycan (BGN) is upregulated in hypertrophied LF by mechanical stress. However, the expression and function of BGN in human LF has not been well elucidated. To investi

BACKGROUND: Epidural blocks are widely used for the management of acute and chronic pain. The technique of loss of resistance is frequently adopted to determine the epidural space. A discontinuity of the ligamentum flavum may increase the risk of failure to identify the epidural space. The purpose of this study was to investigate the anatomic variations of the cervical and high thoracic ligamentum flavum in embalmed cadavers.
METHODS: Vertebral column specimens of 15 human cadavers were obtained. After vertebral arches were detached from pedicles, the dural sac and epidural connective tissue

Thoracic ossification of the ligamentum flavum (T-OLF) is a relatively rare spinal disorder that generally requires surgical intervention, due to its progressive nature and the poor response to conservative therapy. The prevalence of OLF has been reported at 3.8%-26%, which is similar to that of cervical ossification of the posterior longitudinal ligament (OPLL). The progression of OPLL after cervical laminoplasty for the treatment of OPLL is often shown in long-term follow-up. However, there have been no reports on the progression of OLF following surgery. We report a case of thoracic myelopa

OBJECTIVE: Spinal cord injury is a common occurrence during spinal surgery. In this study, we proposed a zoning laminectomy, which could reduce the incidence of nerve injury. We also discussed the safety and clinical efficacy of the zoning laminectomy for thoracic ossification of the ligamentum flavum (TOLF).
METHODS: Forty-five patients with TOLF who underwent zoning laminectomy from October 2016 to February 2020 were included in the retrospective analysis. The Japan Orthopedic Association (JOA) score was used to evaluate clinical outcomes. Meanwhile, the occurrence of complications was reco

Thoracic ossification of the ligamentum flavum (TOLF) is a heterotopic ossification of spinal ligaments. TOLF is the major cause of thoracic spinal canal stenosis and myelopathy, and its underlying mechanisms are not clear. Bone formation is a complex developmental process involving the differentiation of mesenchymal stem cells to osteoblasts, and regulated by BMP2, RUNX2, Osterix (OSX), etc. In this study, we continue to further characterize properties of TOLF. Our immunohistochemistry experiments showed that expressions of osteoblastic factors such as BMP2 and RUNX2 increased in TOLF. Accord

INTRODUCTION: Gender differences may play a role in the pathogenesis of lumbar spinal stenosis. However, few reports that discuss the effects of gender differences in ligamentum flavum (LF) hypertrophy have been published, and no study has investigated the relationship between LF thickness and the quantitative value of intervertebral disc (IVD) degeneration. This study aimed to investigate the impact of gender on the pathomechanisms underlying LF hypertrophy, focusing on the relationship among LF thickness, IVD degeneration, and age.
METHODS: The subjects include 100 patients with low back pa

Ossification of the ligamentum flavum (OLF) is a rare cause of thoracic myelopathy. Mostly described in Japan and East Asia, a number of cases have recently been reported in western subjects. We present the case of a patient, an asymptomatic carrier of lumbar canal stenosis, presenting with a slowly progressive posterior cord compression and a T12 sensitive level. The diagnosis of thoracic OLF was retrospectively made thanks to medical imaging. We reviewed the literature and focused on the epidemiology, physiopathology, presentation, treatment and prognosis of the thoracic OLJ with or without

Ossification of Ligamentum Flavum (OLF) is associated with serious neurologic symptoms including thoracic myelopathy and spinal stenosis. The pathogenesis of thoracic OLF is mainly due to the localized mechanical stress on the ligament induced enchondral ossification. However, despite numerous epidemiological and basic science studies, the mechanism of this process remains unclear. Studies have suggested that inflammatory cytokines, such as IL-6, TNF-α, seem to play a crucial role in OLF. In this review, we summarise the mechanistic information on the roles of inflammation cytokines in OLF and

STUDY DESIGN: A retrospective review.
OBJECTIVE: The aim of this study is to highlight one of the under recognized and under reported aspects of ossification of the ligamentum flavum (OLF), namely, dural ossification in OLF and to discuss the incidence, radiologic signs, causes as well as the surgical and prognostic implications of dural ossification in OLF.
SUMMARY OF BACKGROUND DATA: OLF is being increasingly recognized as a cause of myelopathy. One of the surgical pitfalls in the management of this condition is ossification of the dura mater in OLF. Preoperative identification of ossified

BACKGROUND: Ossification of the ligamentum flavum overlying the lower thoracic spine frequently produces myelopathy. This study analyzed the postoperative outcomes after decompressive laminectomy for thoracic OLF.
METHODS: We retrospectively studied 13 patients (10 male, 3 female; mean age, 58 years; range, 39-69). The mean follow-up duration was 66 months (range, 21-107). All patients had undergone decompressive laminectomy and excision of the OLF. The clinical course was evaluated according to the Frankel grading system and JOA scores. The number of vertebral segments demonstrating OLF, the
