A study on the efficacy of late lingual nerve repair.

The level of sensory recovery was studied in 13 consecutive patients who had undergone lingual nerve repair after a delay of 7-32 (mean 16) months since the initial injury. In all patients the damaged segment of nerve was excised and the cut ends directly apposed by 6-10 (mean 7) epineurial sutures. The final outcome was assessed 12-24 months after repair. Preoperatively none of the patients could detect light touch stimuli in the denervated area, whereas 10 patients could detect some stimuli after repair. Pin-prick was detected in 6 preoperatively and in some areas by all 13 patients after re

A method of assessment in cases of lingual nerve injury.

A method for assessing lingual sensation is described, comprising sensory testing, using touch and moving two-point discrimination and patient subjective reporting. The clinical application is seen to be the evaluation of lingual nerve injury consequent upon lower third molar surgery. Using this method it is considered possible to identify many of those patients unlikely to make full spontaneous recovery at the stage of 3 months following injury, with a view to achieving an earlier timing of surgical repair than that which prevails at present.

Lingual nerve injuries: recognized complications or preventable errors?

Lingual nerve injuries (LNIs) represent a significant clinical challenge that can compromise speech, taste perception, and overall patient well-being. These may occur during third molar extractions, inferior alveolar nerve blocks, implant placement, root canal treatment and other dental, oral, and maxillofacial procedures. A major controversy centers on whether LNIs should be regarded as recognized complications-unavoidable events despite competent care-or potential negligence arising from substandard practice. Such classification hinges on factors including adherence to standard surgical prot

Lingual Nerve Measurements in Cadaveric Dissections: Clinical Applications.

PURPOSE: Lingual nerve (LN) injury is quite prevalent despite its long-known anatomic course. The purpose of this study was to accurately predict the LN pathway by identifying and measuring close anatomic landmarks; these measurements should help lessen the incidence of LN injury. MATERIALS AND METHODS: LN dissection was carried out on 15 halved cadaver skulls (total, 28 specimens). RESULTS: On average, the LN position was approximately 7 mm below the alveolar crest at the distal end of the mandibular second molar, 5.5 mm anterior to the lingula, and 14.6 mm distal to the alveolar crest at t

Lingual nerve vulnerability: risk analysis and case report.

A known neurologic complication of third molar surgery is lingual nerve injury. Certain risk factors can cause lingual nerve injury during the extraction of mandibular third molars. This article will review the existing literature regarding lingual nerve injuries and related risk factors, and it attempts to study lingual nerve vulnerability during extraction of lower third molars, particularly in cases of distoangular impaction. The case report describes exposure of the lingual nerve during the extraction of the lower third molar. Also presented are 4 specific risk factors of lingual nerve inj

Idiopathic trigeminal neuralgia complicated by lingual nerve dysesthesia.

The treatment of facial pain disorders has become a multifaceted discipline that involves numerous scientific fields. Diagnostic and treatment modalities may be beneficial to the patient but at times may also complicate the problem and compromise the outcome. We present an interesting case of left trigeminal neuralgia complicated by unassociated lingual nerve dysesthesia.

Lingual nerve damage due to inferior alveolar nerve blocks: a possible explanation.

BACKGROUND: An explanation for the predominance of injuries to lingual nerves over those to inferior alveolar nerves as a result of inferior alveolar nerve blocks may be due to the nerves' fascicular pattern. A unifascicular nerve may be injured more easily than a multifascicular nerve. METHODS: The authors unilaterally dissected lingual and inferior alveolar nerves from 12 cadavers. They cut the specimens 2 millimeters above the lingula for both the lingual nerve and inferior alveolar nerve and opposite the site of the middle of the third molar for the lingual nerve, and they counted the num

Anatomy of the lingual nerve: Application to oral surgery.

The purpose of this research is to obtain morphological information about the traveling route, branching pattern, and distribution within the tongue of the lingual nerve, all of which are important for oral surgical procedures. Using 20 sides from 10 Japanese cadaveric heads, we followed the lingual nerve from its merging point with the chorda tympani to its peripheral terminal in the tongue. We focused on the collateral branches in the area before reaching the tongue and the communication between the lingual and hypoglossal nerves reaching the tongue. The collateral branches of the lingual ne

Clinical anatomy of the lingual nerve and identification with ultrasonography.

Our objective was to investigate the pathway of the lingual nerve and find out whether it can be identified using ultrasonography (US) intraorally. It is a dominant sensory nerve that branches from the posterior division of the mandibular aspect of the trigeminal nerve, and is one of the two most injured nerves during oral surgery. Its anatomy in the region of the third molar has been associated with lingual nerves of variable morphology. If surgeons can identify its precise location using US, morbidity should decrease. We searched published anatomical and specialty texts, journals, and websit

Properties of the lingual and LVP branches of the glossopharyngeal nerve.

We investigated the effects of various stimuli on the afferent and efferent branches of the glossopharyngeal nerve in the rat soft palate. One of the sensory components, the lingual branch, responded to tactile stimulation, while the LVP branch responded to stretching of the levator veli palatini muscle. We also obtained physiological and morphological evidence of the existence of muscle spindles in the levator veli palatini muscle and showed that tactile stimulation of the contralateral soft palate and stretching of the contralateral LVP modulated discharges from the motor component of the ip

[Sensory injury to the lingual nerve after nerve block at the lingula mandibulae].

Prolonged and possibly permanent change in sensation due to lingual nerve damage can occur after mandibular block anesthesia. The condition is rare and little can be done to prevent its occurrence. A case report is presented and functional as well as legal implications are discussed. Also practical recommendations are given to help the dentist when faced with the situation.

Anatomical Relationship Between the Lingual Nerve and Submandibular Duct.

The purpose of this study was to investigate the anatomical relationship between the lingual nerve and submandibular duct. This study included 1403 patients with submandibular or sublingual gland diseases who underwent intraoral removal of submandibular gland sialoliths, submandibular glands, or sublingual glands. Of all patients, 33 patients underwent bilateral surgeries. All surgeries were performed a single surgeon, and the anatomical relationship between the lingual nerve and submandibular duct was always identified intraoperatively and recorded in the operation recorded. The anatomical re

Variation in Lingual Nerve Course: A Human Cadaveric Study.

The lingual nerve is a terminal branch of the mandibular nerve. It is varied in its course and in its relationship to the mandibular alveolar crest, submandibular duct and also the related muscles in the floor of the mouth. This study aims to understand the course of the lingual nerve from the molar area until its insertion into the tongue muscle. This cadaveric research involved the study of 14 hemi-mandibles and consisted of two parts: (i) obtaining morphometrical measurements of the lingual nerve to three landmarks on the alveolar ridge, and (b) understanding non-metrical or morphological a

Interventions for iatrogenic inferior alveolar and lingual nerve injury.

BACKGROUND: Iatrogenic injury of the inferior alveolar or lingual nerve or both is a known complication of oral and maxillofacial surgery procedures. Injury to these two branches of the mandibular division of the trigeminal nerve may result in altered sensation associated with the ipsilateral lower lip or tongue or both and may include anaesthesia, paraesthesia, dysaesthesia, hyperalgesia, allodynia, hypoaesthesia and hyperaesthesia. Injury to the lingual nerve may also affect taste perception on the affected side of the tongue. The vast majority (approximately 90%) of these injuries are tempo
