Adverse reactions to vancomycin and cross-reactivity with other antibiotics.

PURPOSE OF REVIEW: Glycopeptide antibiotics such as vancomycin are frequently utilized to treat resistant Gram-positive infections such as methicillin-resistant Staphylococcus aureus. The current literature on glycopeptide and lipoglycopeptide structure, hypersensitivity and potential cross-reactivity was reviewed, highlighting implications for safe prescribing. RECENT FINDINGS: Structurally similar, glycopeptides could theoretically cross-react. Immediate reactions to vancomycin include non-IgE-mediated reactions (e.g. red man syndrome) and IgE-mediated hypersensitivity (e.g. anaphylaxis), s

Cerebrospinal fluid penetration of new antibiotics in the treatment of central nervous system infections.

BACKGROUND: Multidrug-resistant (MDR) bacterial infections present significant challenges to the management of central nervous system (CNS) infections, particularly in neurosurgical patients and those with cerebrospinal fluid (CSF) diversion devices. Achieving adequate CNS exposure is complicated by variable blood-brain barrier permeability and physiological changes, and CSF concentrations are not reliably predicted by plasma concentrations. Several new antibiotics have been introduced in response to rising MDR rates. To date, none are licensed for CNS infections, and data on CSF penetration a

Single-dose Antibiotics for Treatment of Skin and Soft Tissue Infections.

Single-dose antibiotics offer opportunities to optimize care for select patients with skin and soft tissue infections. There are 2 long-acting lipoglycopeptides available in the United States, dalbavancin and oritavancin, which have broad gram-positive activity and significantly long-half lives, allowing for single-dose therapy. Both have been shown to be noninferior to standard of care antibiotics when administered as a single dose. These drugs can prevent patients from requiring hospitalization for intravenous antibiotics or expedite discharge for those already admitted. They also have a rol

Heartbreaking Decisions: The Dogma and Uncertainties of Antimicrobial Therapy in Infective Endocarditis.

Infective endocarditis (IE) is a rare but increasingly prevalent disease with high morbidity and mortality, requiring antimicrobials and at times surgical intervention. Through the decades of healthcare professionals' experience with managing IE, certain dogmas and uncertainties have arisen around its pharmacotherapy. The introduction of new antimicrobials and novel combinations are exciting developments but also further complicate IE treatment choices. In this review, we provide and evaluate the relevant evidence focused around contemporary debates in IE treatment pharmacotherapy, including b

Clinical Pharmacokinetics and Pharmacodynamics of Telavancin Compared with the Other Glycopeptides.

Telavancin was discovered by modifying the chemical structure of vancomycin and belongs to the group of lipoglycopeptides. It employs its antimicrobial potential through two distinct mechanisms of action: inhibition of bacterial cell wall synthesis and induction of bacterial membrane depolarization and permeabilization. In this article we review the clinically relevant pharmacokinetic and pharmacodynamic data of telavancin. For comparison, the pharmacokinetic and pharmacodynamic data of the other glycopeptides are presented. Although, in contrast to the newer lipoglycopeptides, telavancin demo

Fluorescence assay to predict activity of the glycopeptide antibiotics.

Here, we describe a fluorescent assay developed to study competitive binding of the glycopeptide antibiotics to live bacteria cells. This assay demonstrated that the mechanism of action of the lipoglycopeptide antibiotics strongly depends on the hydrophobicity of the substitutes, with the best antibacterial activity of the glycopeptide antibiotics equally sharing properties of binding to D-Ala-D-Ala residues of the nascent peptidoglycan and to the membrane.

Effects of Microplate Type and Broth Additives on Microdilution MIC Susceptibility Assays.

The determination of antibiotic potency against bacterial strains by assessment of their minimum inhibitory concentration normally uses a standardized broth microdilution assay procedure developed more than 50 years ago. However, certain antibiotics require modified assay conditions in order to observe optimal activity. For example, daptomycin requires medium supplemented with Ca2+, and the lipoglycopeptides dalbavancin and oritavancin require Tween 80 to be added to the growth medium to prevent the depletion of free drug via adsorption to the plastic microplate. In this report, we examine sys

Dalbavancin for treatment of implant-related methicillin-resistant Staphylococcus aureus osteomyelitis in an experimental rat model.

Dalbavancin is a new semisynthetic lipoglycopeptide with improved antimicrobial activity against various gram-positive pathogens. It demonstrates an extensive plasma half-life which permits outpatient parenteral antimicrobial therapy with weekly intervals and might therefore be an excellent treatment alternative for patients requiring prolonged antimicrobial therapy. The present study investigated dalbavancin monotherapy in an experimental implant-related methicillin-resistant Staphylococcus aureus (MRSA) osteomyelitis model. A clinical MRSA isolate and a Kirschner-wire were inserted into the

Genomic and phenotypic adaptations of methicillin resistant Staphylococcus aureus during vancomycin therapy.

Methicillin-resistant Staphylococcus aureus (MRSA) poses a significant global health challenge, particularly associated with serious infections such as bacteremia. Lipoglycopeptide antibiotics, including vancomycin, dalbavancin, and daptomycin, are critical in MRSA treatment. In this study, we analyzed two MRSA isolates (XF1 and XF2) from a bacteremia patient treated with vancomycin. Antimicrobial susceptibility testing revealed that XF1 was sensitive to vancomycin, dalbavancin, and daptomycin, whereas XF2 exhibited 8- to 16-fold higher minimum inhibitory concentrations for these antibiotics,

Synthesis of Substrates and Biochemical Probes for Study of the Peptidoglycan Biosynthetic Pathway.

Several widely used antibiotics such as beta-lactams, glycopeptides, and lipoglycopeptides exhibit their activity by interfering with peptidoglycan biosynthesis. Ever-increasing resistance to these and other agents has placed a renewed emphasis on the need to understand the reactions in this bio-synthetic pathway at the molecular level. While efficient access to many of the biosynthetic enzymes has been gained, the isolation of their natural substrates has proven difficult. Chemical synthesis has provided valuable tools to circumvent this problem and has allowed convenient access to several ke
