Partial liquid ventilation: Effect of initial dose and redosing strategy in acute lung injury.

OBJECTIVE: Partial liquid ventilation (PLV) with perfluorochemicals has been shown to be effective in treating acute respiratory failure in animal studies and human trials. To determine the influences of perfluorochemicals on initial dose and redosing strategy, we studied their effects on gas exchange, pulmonary mechanics, and lung architecture. DESIGN: After lung injury was induced by repeated warm saline lavages, the animals were instilled endotracheally with different doses of perflubron during 5-10 mins in PLV-treated groups. The animals were randomized to five groups: PLV-12S (12 mL/kg pe

Core body temperature control by total liquid ventilation using a virtual lung temperature sensor.

In total liquid ventilation (TLV), the lungs are filled with a breathable liquid perfluorocarbon (PFC) while a liquid ventilator ensures proper gas exchange by renewal of a tidal volume of oxygenated and temperature-controlled PFC. Given the rapid changes in core body temperature generated by TLV using the lung has a heat exchanger, it is crucial to have accurate and reliable core body temperature monitoring and control. This study presents the design of a virtual lung temperature sensor to control core temperature. In the first step, the virtual sensor, using expired PFC to estimate lung temp

Dynamic and quasi-static lung mechanics system for gas-assisted and liquid-assisted ventilation.

Our aim was to develop a computerized system for real-time monitoring of lung mechanics measurements during both gas and liquid ventilation. System accuracy was demonstrated by calculating regression and percent error of the following parameters compared to standard device: airway pressure difference (Delta P(aw)), respiratory frequency (f(R) ), tidal volume (V(T)), minute ventilation (V'(E)), inspiratory and expiratory maximum flows (V'(ins, max), V'(exp, max)), dynamic lung compliance (C(L,dyn) ), resistance of the respiratory system calculated by method of Mead-Whittenberger (R(rs,MW)) and by

[The role of liquid ventilation in the treatment of acute respiratory failure in children--a systematic review].

BACKGROUND: Liquid Ventilation consists of partially or completely filling the lungs with a fluid (perfluorcarbon). This brings about elimination of the air-liquid interface and reduction of the surface tension in lungs with surfactant deficiency. This article focuses on the role of liquid ventilation in the treatment of acute respiratory failure in children. METHODS: A systematic review of literature was conducted using MEDLINE, LILACS and COCHRANE LIBRARY databases and references of articles. Match terms were: Liquid Ventilation, Respiratory Distress Syndrome, Meconium Aspiration Syndrome,

Morphometrics of normal and hypoplastic lungs in preterm lambs with gas and partial liquid ventilation.

To test the hypothesis that perfluorochemical (PFC) instillation may reduce the pulmonary trauma commonly associated with conventional gas ventilation, we studied 12 lambs with normal lungs and 10 with pulmonary hypoplasia secondary to congenital diaphragmatic hernia (CDH). We used mechanical ventilation for up to 3.5 h, with and without tracheal instillation of LiquiVent PFC liquid. At the end of experimentation lungs were fixed for morphometrical analysis of their components and pulmonary trauma was evaluated by measurement of the perivascular compression index (PCI = % perivascular emphysem

A prototype of volume-controlled tidal liquid ventilator using independent piston pumps.

Liquid ventilation using perfluorochemicals (PFC) offers clear theoretical advantages over gas ventilation, such as decreased lung damage, recruitment of collapsed lung regions, and lavage of inflammatory debris. We present a total liquid ventilator designed to ventilate patients with completely filled lungs with a tidal volume of PFC liquid. The two independent piston pumps are volume controlled and pressure limited. Measurable pumping errors are corrected by a programmed supervisor module, which modifies the inserted or withdrawn volume. Pump independence also allows easy functional residual

Partial liquid ventilation with low dose of perflubron and a low stretch ventilation strategy improves oxygenation in a rabbit model of surfactant depletion.

To demonstrate the efficacy of partial liquid ventilation (PLV) with a low dose of perflubron (PFB), severe lung injury model followed surfactant-depleted rabbits underwent PLV with either moderate tidal volume (V(T)) and high positive end-expiratory pressure (PEEP) or high V(T) and low PEEP. PLV with low-dose PFB was effective in alleviating hypoxia in a severe lung injury model if adequate PEEP was applied. We concluded that the addition of a low dose of PFB might be beneficial to a low stretch ventilation strategy, which allows a less aggressive approach to achieve adequate oxygenation with

Neonatal total liquid ventilation: is low-frequency forced oscillation technique suitable for respiratory mechanics assessment?

This study aimed to implement low-frequency forced oscillation technique (LFFOT) in neonatal total liquid ventilation (TLV) and to provide the first insight into respiratory impedance under this new modality of ventilation. Thirteen newborn lambs, weighing 2.5 + or - 0.4 kg (mean + or - SD), were premedicated, intubated, anesthetized, and then placed under TLV using a specially design liquid ventilator and a perfluorocarbon. The respiratory mechanics measurements protocol was started immediately after TLV initiation. Three blocks of measurements were first performed: one during initial respira
