The modern day masquerader: a rare case of acute HIV infection with neurologic complications and a review of the literature.

Our case demonstrates a rare presentation of acute HIV infection (AHI) with myoclonus, rhabdomyolysis, and aseptic meningitis. It is imperative for primary care physicians to consider AHI. In this patient, laboratory findings demonstrated infection three to four months before presentation. The diagnosis of AHI is critical for early intervention and for decreasing transmission. We review the CNS manifestations of AHI, the laboratory stages of AHI, and discuss treatment options.

Classical and modern classifications of luxations and fractures of thoracolumbar spine. Biomechanical and clinical aspects. Review of literature.

The authors present the advantages and disadvantages of the Denis and AO classifications based on a review of classical and recent literature concerning the evolution of classification of thoracolumbar spine conditions. The newest classification of fractures proposed by Vaccaro (TLICS - Thoracolumbar Injury Classification Severity Score) is discussed in detail. Special attention is given to instability in these fractures. The principles of modern operative treatment are briefly discussed. Short transpedicular segmental stabilization and immobilization is a commonly accepted approach in unstabl

Brain dose-sparing radiotherapy techniques for localized intracranial germinoma: Case report and literature review of modern irradiation.

We examined the effects of intensity-modulated radiation therapy with dose-sparing and avoidance technique on a pediatric patient with localized intracranial germinoma. We also reviewed the literature regarding modern irradiation techniques in relation to late neurocognitive sequelae. A patient with a localized intracranial germinoma in the third ventricle anterior to the pineal gland received a dose-sparing intensity-modulated radiation therapy. The planning was compared to the radiation oncologist's guide of organs at risk and dose constraints for dosimetric analyses. The patient received ra

Sometimes Jello helps: perceptions of headache etiology, triggers and treatment in literature.

Throughout history writers have attempted to describe the symptoms and evoke the misery of "a dismal headache." Writers from Plato to Stephen King have used the phenomenology of headache to illustrate their work. Lewis Carroll, for example, vividly describes the central scotoma, tunnel vision, phono-phobia, vertigo, distortions in body image, dementia and visual hallucinations that often accompany migraine. Although many authors have discussed the topic seriously, others have addressed the issue in a dismissive and even contemptuous manner, relegating this very real disorder to the status of a

The death of Nicholas Bolkonski. Neurology in Tolstoy's War and Peace.

Painstaking realism is an essential feature of the fiction of Count Leo Tolstoy. One example of Tolstoy's attention to detail is the description of the death of Prince Nicholas Bolkonski in War and Peace. The information provided in War and Peace allows the identification of the prince's terminal illness as a brain-stem stroke and is probably the first description of the one-and-a-half syndrome. Prince Bolkonski is also portrayed as suffering from a dementing process. Tolstoy used the character of Prince Bolkonski to exemplify the rationalistic, Western-influenced aristocracy that dominated Ru

Patient Weight-bearing after Pelvic Fracture Surgery-A Systematic Review of the Literature: What is the Modern Evidence Base?

BACKGROUND: Little attention in the literature appears to have been paid to the issue of postoperative weight-bearing protocols for different injury patterns after pelvic fracture surgery. The primary aim of this study is to review the currently available literature to define the level of available evidence used to inform surgical decisions on weight-bearing after pelvic fracture surgery. Secondary aims are to assess the published methods of fracture classification, surgical management, and assessment or reporting of patient outcomes. METHODS: A systematic review of the English language liter

Modern management of non-chemotherapy drug-induced agranulocytosis: a monocentric cohort study of 90 cases and review of the literature.

BACKGROUND: The present study reports a monocentric experience of 90 drug-induced agranulocytosis cases and discusses their management, in particular the role of hematopoietic growth factors. METHODS: Data from 90 patients with drug-induced agranulocytosis who met the criteria of the IAAAS group and of Bénichou and Solal-Celigny [Nouv Rev Fr Hematol 1993; 33: 257.] were retrospectively reviewed. All cases were extracted from a cohort study of the Hopitaux Universitaires de Strasbourg, France. Data were specifically analyzed with regard to the use of hematopoietic growth factors (in 42 patients

The Use of Metal Sublaminar Wires in Modern Growth-Guidance Scoliosis Surgery: A Report of 4 Cases and Literature Review.

BACKGROUND: To avoid early fusion and allow residual growth of the spine in early onset scoliosis (EOS) treatment, growth-guided scoliosis surgery can be performed. Four patients with EOS are presented in which a growth-guidance instrumentation is used with sliding titanium (Ti) sublaminar cables. Residual growth of the spine can be preserved using metal sublaminar wiring; however, several drawbacks of this technique and type of material are illustrated. METHODS: Four patients with progressive neuromuscular scoliosis were treated with a posterior stabilization. A fusionless growth-guidance in

Rational basis of modern therapy of bacterial meningitis. Review of the literature and our clinical experience of 122 pediatric cases.

In recent years the treatment of bacterial meningitis has been modified on the basis of a better understanding of its physiopathological mechanisms. It has been shown, for example, that the inflammatory reaction is the primary cause of brain damage in bacterial meningitis. Inflammation and consequent brain damage are greatest in the first hours of antibiotic treatment when rapid and massive bacteriolysis takes place. In effect, the bacterial components activate metabolic pathways and cellular elements leading to the release of inflammation mediators: cytokines (TNF, IL-I) neutrophil degranulat

Shame, Enchantment, and the "There-ness" of Disability in The Secret Garden.

Frances Hodgson Burnett's The Secret Garden has been a repugnant object for contemporary scholars of disability studies because of the equivalence it draws between able embodiment and morality. While scholars have remarked on the novel's linguistic register of rectitude, few have focused on what it might mean to analyze The Secret Garden through the prism of shame. I attempt to develop such a reading here, exploring shame's textual and historical multivalence. I will ask: what is at stake-affectively and critically-in the practice of re-reading a novel that one experienced first in childhood?

[Mechanisms of power in disease: the case of the novel "The Plague" by Albert Camus].

This paper explores the elements of power that can be found in an epidemic like the plague. To undertake this task we first studied, the form of containment of the plague from a historical perspective and then, compare them with those described by Camus in his novel The Plague. We also studied the experience of sin among humans in an effort to determine divine power. This last point explores the fear of being touched during an epidemic and how this is overcome by the innate feeling of love among men. Finally in the novel, this is illustrated by the love of Orpheus for Eurydice.

Contagious Sympathies in The Last Man.

In The Last Man, Mary Shelley relies on theories of Romantic medicine that give her an unconventional way to conceptualize and narrate time and space. For the Romantics, organs, bodies, and communities were all intimately connected through "sympathy." The Last Man carefully employs the connection between biological sympathy and contagious disease to consider how suffering inhabits space. Lionel's individual suffering intertwines with the global suffering from the fictional plague of the novel, which threatens the boundaries of both Europe and the white, European body much as its non-fictional
