INTRODUCTION: Tinnitus loudness is a subjective measure, and it does not directly reflect either tinnitus severity or the impact on daily life. Nevertheless, loud tinnitus may be the most frequent clinical complaint of tinnitus patients. Factors contributing to the loudness of the phantom sound have rarely been studied. We evaluated both matched and self-rated loudness in a large sample of patients with tinnitus and analyzed the influencing factors among demographic, hearing, and tinnitus characteristics.
METHODS: Two hundred ninety-nine patients with chronic tinnitus were enrolled. We evalua

Our 15 years of research have generated the first short- and long-term efficacy data for speech treatment (Lee Silverman Voice Treatment; LSVT/LOUD) in Parkinson's disease. We have learned that training the single motor control parameter amplitude (vocal loudness) and recalibration of self-perception of vocal loudness are fundamental elements underlying treatment success. This training requires intensive, high-effort exercise combined with a single, functionally relevant target (loudness) taught across simple to complex speech tasks. We have documented that training vocal loudness results in d

Loudness recruitment is a common symptom of hearing loss induced by cochlear lesions, which is defined as an abnormally fast growth of loudness perception of sound intensity. This is different from hyperacusis, which is defined as "abnormal intolerance to regular noises" or "extreme amplification of sounds that are comfortable to the average individual". Although both are characterized by abnormally high sound amplification, the mechanisms of occurrence are distinct. Damage to the outer hair cells alters the nonlinear characteristics of the basilar membrane, resulting in aberrant auditory nerv

Some environmental sounds have strong amplitude fluctuations that may affect their perceived loudness and annoyance. This study assessed the effect of beat rate (fb) and center frequency (fc) on the loudness of low-frequency beating tones. The loudness of two-tone complexes (TTCs) with fc = 40, 63, 80, and 1000 Hz was matched with that of unmodulated tones (UTs). Frequency differences between the TTC components, corresponding to fb = 1, 2, 5, and 12 Hz, were used. To compensate for the steep decline in hearing sensitivity below 100 Hz, prior to the loudness match, subjects adjusted the relativ

A discrimination task was used to assess changes in the loudness of speech that accompanied changes in the spectral tilt of a simulated hearing aid's frequency response. Band-limited (0.25-4 kHz) spondaic words were spectrally shaped at comparison tilt-factor values of -6, 0, and +6 dB per octave and delivered monaurally via insert earphone to each of 10 listeners with normal hearing (NH) and 15 listeners with mild-to-moderate sensorineural hearing impairment (HI). Results for the NH listeners indicated that loudness differences among the tilt factors were generally perceptible and that loudne

Some active middle-ear implants use round-window stimulation to extend their application to mixed hearing loss. However, current loudness models are unable to predict loudness under round-window stimulation, which prevents the development of fitting procedures. This paper presents a new loudness model for round-window stimulation. The model consists of a peripheral auditory front-end module, which translates the signal picked up by the microphone to round-window stimulation and to effective cochlear response, and a loudness perception back-end module for converting the cochlear response into l

In DFNA8/12, an autosomal dominantly inherited type of nonsyndromic hearing impairment, the TECTA gene mutation causes a defect in the structure of the tectorial membrane in the inner ear. Because DFNA8/12 affects the tectorial membrane, patients with DFNA8/12 may show specific audiometric characteristics. In this study, five selected members of a Dutch DFNA8/12 family with a TECTA sensorineural hearing impairment were evaluated with pure-tone audiometry, loudness scaling, speech perception in quiet and noise, difference limen for frequency, acoustic reflexes, otoacoustic emissions, and gap de

OBJECTIVES: To determine whether auditory brainstem response (ABR) wave I amplitude is associated with measures of auditory perception in young people with normal distortion product otoacoustic emissions (DPOAEs) and varying levels of noise exposure history.
DESIGN: Tinnitus, loudness tolerance, and speech perception ability were measured in 31 young military Veterans and 43 non-Veterans (19 to 35 years of age) with normal pure-tone thresholds and DPOAEs. Speech perception was evaluated in quiet using Northwestern University Auditory Test (NU-6) word lists and in background noise using the wo

Members of a Dutch DFNA13/COL11A2 family were evaluated with pure tone audiometry, stapedial reflexes, otoacoustic emissions, loudness scaling, difference limen for frequency, gap detection, and speech perception in quiet and noise. The tone audiometry showed a predominant loss for the low and middle frequencies, with only a few otoacoustic emissions at thresholds better than 25 dB hearing level. The stapedial reflexes appeared elevated, and loudness growth curves were shifted parallel to those for normal-hearing subjects, indicating a shift of the dynamic range toward higher presentation leve

Adjustment of a speech processor of the cochlear implant is the first procedure in rehabilitation of patients after cochlear implantation. Influence of setting understated and overstated threshold levels of current perception on processing of a speech signal was studied. The results of the study show that only precise estimation of current perception threshold and comfortable loudness allow transmitting maximal acoustic information for each patient.

Impulse sounds such as clinking dishes or slamming objects are often perceived as particularly intense or uncomfortable, yet their perceptual characterization remains insufficiently understood. The present study systematically examined subjective and physiological responses to ecologically valid impulse sounds in young normal-hearing adults. Twenty-seven participants rated nine impulse sounds presented at peak levels between 80 and 120 dB SPL in three acoustic conditions: anechoic, reverberant (room-convolved), and anechoic embedded in the International Speech Test Signal (ISTS) at 65 dB SPL.

We aimed to evaluate the impact of short-term tinnitus treatment on cognitive function and identify the effects of various treatment combinations on cognitive and tinnitus outcomes. A non-randomized prospective study was conducted with 32 tinnitus patients at a tertiary university hospital between May 2022 and May 2024. Patients received treatments, including neuromodulation, diuretics, gabapentin, selective serotonin reuptake inhibitors (SSRI), anxiolytics, muscle relaxants, hearing aids, and counseling. Cognitive function and tinnitus distress were assessed using the Mini-Mental State Examin

BACKGROUND: Individuals with Parkinson's Disease (IwPD) often fail to adjust their voice in different situations, without awareness of this limitation. Clinicians use self-report questionnaires that are typically designed for individuals with General Voice Disorders (GVD) in the vocal assessment of IwPD. However, these instruments may not consider that IwPD have a reduced self-perception of their vocal deficits. This study aimed to compare self-reported vocal symptoms and voice loudness between IwPD and GVD.
METHODS: 28 IwPD and 26 with GVD completed the Voice Symptom Scale (VoiSS) questionna

OBJECTIVES: Normalizing perceived loudness is an important rationale for gain adjustments in hearing aids. It has been demonstrated that gains required for restoring normal loudness perception for monaural narrowband signals can lead to higher-than-normal loudness in listeners with hearing loss, particularly for binaural broadband presentation. The present study presents a binaural bandwidth-adaptive dynamic compressor (BBDC) that can apply different gains for narrow- and broadband signals. It was hypothesized that normal perceived loudness for a broad variety of signals could be restored for

The objective of this study was to determine the reliability and validity of the Hungarian version of the original Tinnitus Handicap Inventory (THI). In the study design the original THI (THI-US) was translated into Hungarian by three independent investigators. The final Hungarian version (THI-HUN) was constructed on the basis of these Hungarian interpretations and then administered to 72 patients suffering from chronic tinnitus. Construct validity was assessed using short version of Beck's Depression Inventory (BDI), Perceived Stress Scale (PSS4 version), Visual Analogue Scale (VAS) (loudness

OBJECTIVE: Patients with chronic, subjective tinnitus are often administered a battery of audiometric tests to characterize their tinnitus percept. Even a comprehensive battery, if applied just once, cannot capture fluctuations in tinnitus strength or quality over time. Moreover, subjects experience a learning curve when reporting the detailed characteristics of their tinnitus percept, such that a single assessment will reflect a lack of familiarity with test requirements. We addressed these challenges by programming an automated software platform for at-home tinnitus characterization over a 2

PURPOSE: Recent evidence has suggested that amplitude modulated tones might have some advantages over broadband noise.
METHOD: Fifty-six subjects listened to S-Tones at a carrier frequency matched at the tinnitus pitch (amplitude modulation rate of 40 Hz) and to broadband noise. Subjects rated their tinnitus loudness before, during, and after a 120-s duration masker.
RESULTS: The results suggested that S-Tones were generally more effective at reducing tinnitus loudness than noise. In about one third (21/56) of the subjects, there was no significant effect from any masker. In other subjects,

In the present study, we use resting state fMRI to investigate whether nucleus accumbens (NAc) and extended frontostriatal networks are involved in the pathology of auditory phantom perception, i.e., tinnitus, through a study of functional connectivity. We hypothesize that resting state functional connectivity involving NAc will be increased relative to what is observed in healthy subjects and that this connectivity will correlate with clinical measures of tinnitus such as percept loudness, duration of symptoms, etc. We show that a large sample of patients with chronic tinnitus (n = 90) featur

HYPOTHESIS: Electrical tinnitus suppression by cochlear implants requires stimulation of a subset of neural elements in the cochlea.
BACKGROUND: Tinnitus is the phantom perception of sound in the ears and is a known correlate of hearing loss. Cochlear implants restore hearing and are known to lessen or extinguish tinnitus. The amount of electrical charge required and the number and location of electrodes required to extinguish tinnitus with a cochlear implant are factors that remain poorly understood.
METHODS: In a subject with single-sided deafness, with tinnitus in the deaf ear, we enabled
