Using United States medicare program claims to identify reliable measures of low value services influenced by primary care physician decisions.

BACKGROUND: Low-value services (LVS) are those that "… provide little to no benefit to patients…."1 Among an existing set of 31 LVS are invasive procedures not typically performed by primary care physicians (PCPs), and other LVS that may be commonly provided by specialists. To facilitate research on the role of primary care physicians in low-value care, we sought to identify the reliability of two subsets of LVS measures that are more directly subject to PCPs' decisions. We refer to these as primary care adjacent ("PCA") LVS measures. METHODS: Retrospective observational cohort study to ident

Estimating the societal costs and benefits of de-implementation of low-value care: developing a modelling approach through case assessment.

BACKGROUND: Previous estimates of potential costs savings from de-implementation of low-value care have been calculated with simple modelling approaches, focused on direct medical costs and did not take substitution into account. Therefore, the aim of this study was to develop and evaluate a modelling approach to calculate a more realistic estimate of societal costs and benefits of de-implementation of low-value care. METHODS: The modelling approach was developed and evaluated in three steps: (1) review of reviews to identify aspects that may affect the costs and benefits of de-implementation,

Kidney Nutrition for Value-Based Care Models: The Role of Low Protein Diets and Keto-Analogue Supplementation to Delay Dialysis.

Government-led dialysis programs in most countries primarily focus on quality and safety measures for dialysis initiation and management. However, in the United States, CKD management may be shifting upstream of dialysis initiation toward value-based care models that prioritize delaying dialysis initiation and improving outcomes for patients with advanced CKD (stages 4 and 5 CKD). Nutritional interventions, particularly low and very low protein diets supplemented with keto-analogues of amino acids, are re-emerging as key strategies to reduce nitrogenous waste production, manage uremia, and slo

Rising trends on folic acid test requests in a middle-income large academic hospital: A low-value care target for improvement.

OBJECTIVES: To evaluate the trends in serum folate test ordering and their abnormal results. METHODS: The authors retrieved data from an anonymized laboratory database from a single, large, academic hospital from Brazil. The authors retrieved serum folate test results, the number of repetitions and other laboratory measurements obtained in the same time window, ordering setting and clinic. The authors then evaluated the trends in test ordering, and prevalence of low (< 3 ng/dL) results, and assessed targets for improvement in test ordering. RESULTS: From January 1st, 2018, to December 31st,

Sustainability of the de-implementation of low-value care in infants with bronchiolitis: 2-year follow-up of a cluster randomised controlled trial.

BACKGROUND: In 2017, the PREDICT (Paediatric Research in Emergency Departments International Collaborative) network conducted a cluster randomised controlled trial (cRCT) at 26 Australian and New Zealand hospitals to improve bronchiolitis care. Findings demonstrated that targeted interventions significantly improved adherence with five evidence-based low-value bronchiolitis practices (no chest radiography, salbutamol, glucocorticoids, antibiotics and epinephrine) in the first 24 hours of hospitalisation (adjusted risk difference, 14.1%; 95% CI: 6.5% to 21.7%; p<0.001). During the intervention

Implementing a Multifaceted Intervention among Internal Medicine Residents with Audit and Educative Data Feedback Significantly Reduces Low-Value Care in Hospitalized Patients.

BACKGROUND: The dissemination of recommendations on low-value care alone may not lead to physicians' behavioral changes. The aim of this study was to evaluate whether a multifaceted behavioral intervention among internal medicine residents could reduce low-value care in hospitalized patients. METHODS: A pre-post quality improvement intervention was conducted at the Internal Medicine Division of La Tour hospital (Geneva, Switzerland) from May 2020 to October 2021. The intervention period (3 months) consisted of a multifaceted informational intervention with audits and educative feedback about

Behavioural 'nudging' interventions to reduce low-value care for low back pain in the emergency department (NUDG-ED): protocol for a 2×2 factorial, before-after, cluster randomised trial.

INTRODUCTION: Opioids and imaging are considered low-value care for most people with low back pain. Yet around one in three people presenting to the emergency department (ED) will receive imaging, and two in three will receive an opioid. NUDG-ED aims to determine the effectiveness of two different behavioural 'nudge' interventions on low-value care for ED patients with low back pain. METHODS AND ANALYSIS: NUDG-ED is a 2×2 factorial, open-label, before-after, cluster randomised controlled trial. The trial includes 8 ED sites in Sydney, Australia. Participants will be ED clinicians who manage b

Factors influencing physiotherapists implementation of high value care in the management of nonspecific low back pain in Indian healthcare settings: A qualitative study.

BACKGROUND: Non-specific low back pain is a common musculoskeletal condition that often requires the involvement of physiotherapists for effective management. However, there is limited understanding of the factors influencing physiotherapists' implementation of high-value care in the management of non-specific low back pain (NSLBP), particularly in Indian healthcare settings. OBJECTIVE: The aim of this study was to explore the barriers and facilitators of implementation of high-value care in managing NSLBP from the perspective of physiotherapists practicing in Indian healthcare settings. Valuing Invisible Labor: A Feminist Political Economy Analysis of the Cost and Burden of Critical Care Nursing in Low- and Lower-Middle-Income Countries.

Critical care nursing is central to intensive care delivery, yet its costs and burdens remain largely invisible in low- and lower-middle-income countries. As critical care services expand in response to epidemiological transitions and global health emergencies, economic evaluations increasingly guide policy and resource allocation. However, prevailing health economic frameworks privilege infrastructure, technology, and physician-led interventions while treating nursing labor as a fixed or residual input. Using a feminist political economy and social reproduction lens, this discursive paper arg

Diagnosis Value of Patient Evaluation Components Applicable in Primary Care Settings for the Diagnosis of Low Back Pain: A Scoping Review of Systematic Reviews.

Low back pain ranks as the leading cause of years lived with disability worldwide. Although best practice guidelines share a consistent diagnostic approach for the evaluation of patients with low back pain, confusion remains as to what extent patient history and physical examination findings can inform management strategies. The aim of this study was to summarize evidence investigating the diagnostic value of patient evaluation components applicable in primary care settings for the diagnosis of low back pain. To this end, peer-reviewed systematic reviews were searched in MEDLINE, CINAHL, PsycI

Value of blood pressure for detecting serious illness in children attending acute care settings in low- and middle-income countries: a multicentre observational study.

BACKGROUND: In children, the value of routine blood pressure measurement in first assessment at the emergency department has been questioned. This has, however, not yet been evaluated in low- and middle-income countries (LMICs) where the incidence of serious illness is high. This study assessed the predictive value of low systolic blood pressure to detect serious illness in children attending acute care settings in LMICs. METHODS: This observational study is based on data from children attending three acute care settings in The Gambia and Suriname. Hypotension was defined by age-adjusted refe

Assessment of Care Cascades Following Low-Value Prostate-Specific Antigen Testing Among Veterans Dually Enrolled in the US Veterans Health Administration and Medicare Systems.

IMPORTANCE: Older US veterans commonly receive health care outside of the US Veterans Health Administration (VHA) through Medicare, which may increase receipt of low-value care and subsequent care cascades. OBJECTIVE: To characterize the frequency, cost, and source of low-value prostate-specific antigen (PSA) testing and subsequent care cascades among veterans dually enrolled in the VHA and Medicare and to determine whether receiving a PSA test through the VHA vs Medicare is associated with more downstream services. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used VHA

Rectal screening displays high negative predictive value for bloodstream infection with (ESBL-producing) Gram-negative bacteria in neonates with suspected sepsis in a low-resource setting neonatal care unit.

OBJECTIVES: We analysed the concordance of rectal swab isolates and blood culture for Gram-negative bacteria (GNB) isolates in neonates with a suspicion of neonatal sepsis admitted to a neonatal care unit in Haiti. METHODS: We matched pairs of blood and rectal samples taken on the date of suspected sepsis onset in the same neonate. We calculated the proportion of rectal isolates in concordance with the blood isolates by species and genus. We calculated the negative predictive value (NPV) for GNB and extended-spectrum β-lactamase (ESBL)-producing GNB for all rectal and blood isolate pairs in n
