BACKGROUND: Exposure to actual or simulated weightlessness is known to induce orthostatic intolerance in humans. Many different methods have been suggested to counteract orthostatic hypotension. The repetitive or prolonged application of lower body negative pressure (LBNP) has shown beneficial effects to counter orthostatic intolerance, but devoting so much time to countermeasures is not compatible with space mission objectives or costs. The purpose of the present study was to assess the effects of brief LBNP sessions against orthostatic intolerance during a 21-d head-down tilt (HDT) bed rest.

BACKGROUND: Optic nerve sheath diameter (ONSD) measured by ultrasound provides a non-invasive surrogate of intracranial pressure. Although lower body negative pressure (LBNP) is widely used to simulate central hypovolemia, the dose-response relationship between graded LBNP and ONSD, and possible sex differences therein, are incompletely characterized.
METHODS: Forty-four healthy young adults (22 female, 22 male) underwent graded LBNP ( - 10 to - 40 mmHg) in the supine position; valid ONSD data were available in 37 participants (18 female, 19 male) who constituted the analytical sample. ONSD

Objective. To investigate the effect of lower body negative pressure (LBNP) in upright seated position on cerebral blood flow velocity, heart rate (HR), and blood oxygen saturation (SaO2) in human. Method. Fifteen young men were selected as subjects. Blood flow velocity in middle cerebral artery (VMCA), HR and SaO2 were measured at 0.5, 1, 2, 3, 4, and 5 min during LBNP and 1, 3, 5 min after -4.00 kPa and -6.67 kPa LBNP in upright seated position respectively. Result. Under -4.00 kPa LBNP, VMCA slowed down at 4, 5 min during LBNP (P<0.05), and HR speeded up at 3, 4, 5 min during LBNP (P<0.05).

PURPOSE: Pulse transit time (PTT) is crucial in developing non-invasive cuffless blood pressure (BP) measurement devices. Sympathetic activation, due to its effect on PTT, can lead to erroneous estimation of BP. Sympathetic activation might affect the PTT differentially depending on the site where PTT is measured in the upper limb. This study aimed to decipher regional variation in PTT in response to sympathetic activation in three segments of the upper limb arteries. Exposure to graded lower body negative pressure (LBNP) at hypotensive (-30 mmHg and -40 mmHg) and non-hypotensive (-10 mmHg and

Postural orthostatic tachycardia syndrome (POTS) is characterized by an excessive heart rate (HR) response upon standing and symptoms indicative of inadequate cerebral perfusion. We tested the hypothesis that during lower body negative pressure (LBNP), individuals with POTS would have larger decreases in cardiac and cerebrovascular function measured using magnetic resonance (MR) imaging. Eleven patients with POTS and 10 healthy controls were studied at rest and during 20 min of -25 mmHg LBNP. Biventricular volumes, stroke volume (SV), cardiac output (Qc), and HR were determined by cardiac MR.

Introduction. The effect of concurrent head-up tilt and lower body negative pressure (LBNP) have been examined on middle cerebral artery velocity (MCAv) at rest; however, it is unknown the superimposed effect these factors have on blunting the elevation in cerebral blood velocity associated with moderate-intensity exercise.Methods. 23 healthy adults (11 females / 12 males, 20-33 years) completed three visits. The first consisted of a maximal ramp supine cycling test to identify the wattage associated with individualized maximal MCAv. Subsequent visits included randomized no LBNP (control) or L

A single bout of exercise as well as exposure to a hypercapnic environment increases cerebral blood flow (CBF) and is an adaptation linked to a post-intervention executive function (EF) benefit. In the present investigation we sought to determine whether a transient reduction in CBF impairs EF. Accordingly, we employed 10-min -30 mmHg and -50 mmHg lower-body negative pressure (LBNP) interventions as well as a non-LBNP control condition. LBNP was employed because it sequesters blood in the lower legs and safely and reliably decreases CBF. Transcranial Doppler ultrasound was used to measure mid

INTRODUCTION: Beetroot juice (BRJ) improves peripheral endothelial function and vascular compliance, likely due to increased nitric oxide bioavailability. It is unknown if BRJ alters cerebrovascular function and cardiovagal baroreflex control in healthy individuals.
PURPOSE: We tested the hypotheses that BRJ consumption improves cerebral autoregulation (CA) and cardiovagal baroreflex sensitivity (cBRS) during lower-body negative pressure (LBNP).
METHODS: Thirteen healthy adults (age: 26 ± 4 years; 5 women) performed oscillatory (O-LBNP) and static LBNP (S-LBNP) before (PRE) and 3 h after con

CASE: We present the case of a 49-year-old woman who underwent a 237-day hunger strike on the streets of Pittsburgh, Pennsylvania. Despite medical supervision and appropriate micronutrient supplementation, including higher-than-recommended dosing of oral thiamine, the patient developed Wernicke's Encephalopathy and subsequent Korsakoff Syndrome. She is now permanently impaired.
INTRODUCTION: Hunger strikers are subject to numerous sequelae of micronutrient deficiency. Among these are the paired conditions of Wernicke's Encephalopathy and Korsakoff Syndrome, conditions secondary to prolonged t

BACKGROUND: Orthostatic intolerance is common in hypermobile Ehlers-Danlos syndrome (hEDS) with one third of patients fulfilling postural orthostatic tachycardia syndrome criteria. Our aim was to assess cerebral blood flow in patients with hEDS and postural orthostatic tachycardia syndrome during orthostasis, which may be responsible for orthostatic intolerance.
METHODS: In 18 individuals with hEDS and postural orthostatic tachycardia syndrome and 20 healthy controls, we conducted real-time phase contrast magnetic resonance imaging of the middle cerebral artery with and without an orthostatic

The majority of cerebral autoregulation research has focused on the middle cerebral artery. However, many symptoms of presyncope indicate posterior cerebral hypoperfusion. To address this issue, we measured cerebrovascular reactivity, cerebral blood flow velocity and dynamic cerebral autoregulation in the middle cerebral artery and vertebral arteries during orthostatic stress to presyncope in 9 healthy subjects. There was no significant difference in either the decline in cerebral blood flow velocity or indices of dynamic cerebral autoregulation between the middle cerebral and vertebral arteri

We studied the influence of preload alteration on the pattern of transmitral flow as obtained by pulsed Doppler echocardiography. In protocol I, 14 patients with old myocardial infarction (OMI) were observed by simultaneous measurements of transmitral flow by PDE and left ventricular end-diastolic dimension (LVDd) using M-mode echocardiography during the control period and after sublingual nitroglycerin. In protocol II, eight patients with OMI were similarly observed during stepwise lower body negative pressure (LBNP: 0, -10 and -20 mmHg). The rapid filling phase was categorized in two subphas

We hypothesize that excessive central blood flow reduction leads to failure in the mechanism that appropriately distributes cerebral blood flow (CBF), preceding the pathological drop in blood pressure. This study aims to evaluate changes in CBF and cerebrovascular resistance in response to reduced cardiac output (CO) during severe central hypovolemia and elucidate the breakdown of cerebral circulation maintenance mechanisms before presyncope. Nine healthy men underwent maximal lower body negative pressure (LBNP), with ultrasound measurements of the internal carotid artery (ICA) and vertebral a

IMPORTANCE: Astronauts returning from long-duration spaceflight experience ocular remodeling related to cephalad fluid shifts induced by microgravity. It is hypothesized that the absence of diurnal reductions in intracranial pressure in microgravity creates a low but persistent pressure gradient at the posterior aspect of the eye, which results in ocular remodeling and space-associated neuro-ocular syndrome (SANS) over many months.
OBJECTIVE: To determine whether partial reintroduction of footward fluid shifts during simulated microgravity via lower body negative pressure (LBNP) during sleep

Cardiovascular deconditioning after a 4-h head-down bed rest (HDBR) might be a consequence of the time of day relative to pre-HDBR testing, or simply 4 h of confinement and inactivity rather than the posture change. Ten men and 11 women were studied during lower body negative pressure (LBNP) before and after 4-h HDBR and 4-h seated posture (SEAT) as a control for time of day and physical inactivity effects to test the hypotheses that cardiovascular deconditioning was a consequence of the HDBR posture, and that women would have a greater deconditioning response. Following HDBR, men and women ha
