Trabecular microstructure of the human lunate in Kienbock's disease.

The trabecular microstructure of normal lunates and lunates with Kienböck's disease was investigated using micro-computed tomography (micro-CT). Five lunates with advanced Kienböck's disease were obtained during lunate excision and scaphocapitate fusion, and five control lunates were from embalmed cadavers. Microstructural morphometric parameters were measured using micro-CT images. Trabeculations of lunates with Kienböck's disease were 2.67 times denser and 1.84 times thicker than those of normal lunates. Furthermore, bone surface areas were 1.43 times greater and bone volume 2.67 times great

Trans-4 Portal as a New Portal for Accessing the Lunate in Wrist Arthroscopy: a Cadaveric Study.

BACKGROUND: Evolving wrist arthroscopy requires creating new portals, and creating portals reciprocally leads to increased indications for arthroscopic wrist procedures. To facilitate access to the lunate bone and fossa for new arthroscopic procedures, a new portal was used. This is a cadaveric study of this portal. OBJECTIVES: In this cadaveric study, we evaluated a portal in wrist arthroscopy for procedures involving the lunate bone and lunate fossa. MATERIALS AND METHODS: Seventeen wrists from 10 fresh cadavers were included in this study. After diagnostic arthroscopy, a portal (Trans-4)

Lunate chondromalacia: evaluation of routine MRI sequences.

OBJECTIVE: Chondromalacia is a commonly encountered abnormality at arthroscopy and may be responsible for significant clinical symptoms and disability. In the wrist, the most common location for chondromalacia is the lunate bone. Consequently, we sought to study the accuracy of clinical MRI in the assessment of lunate articular cartilage. MATERIALS AND METHODS: MR images of 34 patients who underwent arthroscopy and had an MRI examination within 1 month of surgery were evaluated by two reviewers for the presence and location of lunate cartilage defects and subchondral edema. RESULTS: Lunate c

[Comparison of screw implantation parameters between two approaches in capitolunate arthrodesis: an imaging analysis based on CT of the normal wrist].

OBJECTIVE: To compare the parameters of screw implantation in capitolunate arthrodesis between the 2nd and 3rd metacarpal bones and via the distal dorsal capitate bone approach based on CT images of the normal wrist, and provide reference for the selection of surgical approaches and planning of screw insertion trajectory. METHODS: CT data of 50 patients who met the criteria between February 2022 and April 2022 were selected as the study objects. There were 30 males and 20 females, and the age ranged from 18 to 69 years (mean, 37 years). The normal wrist alignment was conformed in all CT image

Histopathology Findings of the Lunate in Stage III Kienböck's Disease.

Background: The etiology and natural history of Kienböck's disease remain unclear. Traditionally it has been defined as avascular necrosis of the lunate. The authors tried to demonstrate different tissue distribution, the area ratio of necrotic tissue and vessel counts inside the whole Kienböck lunate to reveal a dynamic process of the lunate collapse. Methods: Five lunates from patients with stage III Kienböck's disease and one cadaveric lunate not involved by Kienböck's disease were sampled. They were sectioned, H&E stained, and evaluated. The thickness of trabecular bone and the area of nec

Cyst-like lesions of the lunate resembling Kienböck's disease: a case report.

A 33-year-old man presented with a cyst-like lesion of the lunate resembling Kienböck's disease. Radiographs showed collapse of the proximal portion of the lunate and a lucent lesion in the triquetrum. T1-weighted magnetic resonance images showed a low signal in the collapsed part of the lunate but not in the remaining area. During surgery the lesion of the triquetrum contained serous fluid and the lunate was partially collapsed. Histologically, the triquetrum consisted of fibrous connective tissue and the lunate consisted of a mixture of bone, cartilage, and fibrous tissue without necrosis. T

Further investigations of the intraosseous pressure characteristics in necrotic lunates (Kienböck's disease).

To confirm the hypothesis that venous congestion plays a role in idiopathic lunate necrosis (Kienböck's disease), intraosseous pressure in 12 normal and 12 necrotic lunates was measured. Intraosseous pressure in the capitate of 12 healthy subjects served as reference measurement. The intraosseous pressure was recorded in neutral position and 60 degrees extension of the wrist under normotensive conditions and during venous stasis. In all groups, the mean intraosseous pressure rose significantly with extension of the wrist, with the largest increase being 92.3 mmHg for necrotic lunates, followed

Bilateral lunate intraosseous ganglia.

An intraosseous ganglion is a relatively uncommon, benign cystic lesion that occurs in young and middle-aged adults. Bilateral and symmetrical lesions of the wrist are rare. Intraosseous ganglia of the carpal bones are uncommon causes of chronic wrist pain. Isolated cases of intraosseous ganglion have been reported most commonly in the lunate and scaphoid. The lunate was most frequently affected, followed by the capitate, scaphoid, and triquetrum bones. Radiolucent lesions in the carpal bones are not uncommon and are often seen incidentally in asymptomatic patients. The differential diagnosis

Coronal Fracture of the Lunate in Advanced Kienböck Disease: Reestablishing Midcarpal Congruency to Enable Osteochondral Reconstruction: A Case Report.

CASE: We describe a patient with Bain grade-2b Kienböck disease with a coronal fracture of the distal lunate articular surface with proximal structure collapse. To reestablish midcarpal congruity, we used a bone-anchored suture to repair the coronal split in the lunate in preparation for lunate preservation with osteochondral medial femoral trochlea (MFT) reconstruction. CONCLUSION: In Bain grade-2b Kienböck disease, lunate reconstruction of both the proximal and distal joint surfaces can be performed. Successful reconstruction of the distal articular surface can be achieved using a bone-anch

Patient-specific lunate prosthesis with perilunate ligament reconstruction: surgical technique.

The lunate bone plays a central role in force transmission, proximal carpal row integrity, and kinematics of the wrist. In cases of irreparable pathologies to the lunate bone, prosthetic replacement is appealing to avoid salvage procedures. Previous attempts at lunate replacement yielded inconsistent results due to non-anatomical implant design and lack of ligament reconstruction, which posed a risk of dislocation and carpal collapse. Nowadays, the CAD design process and 3D printing of bio-compatible materials such as titanium make it possible to manufacture patient-specific lunate implants. W

Biomechanical Evaluation of Carpal Kinematics during Simulated Wrist Motion.

Background Flexion and extension of the wrist is achieved primarily at the radiocarpal and midcarpal joints. Carpal kinematics have been investigated, although there remains no consensus regarding the relative contribution of each bone to wrist motion. Purpose To determine the kinematics of the scaphoid, lunate, and capitate during unconstrained simulated wrist flexion/extension and to examine the effect of motion direction on the contribution of each bone. Materials and Methods Seven cadaveric upper extremities were tested in a passive wrist simulator with 10N tone loads applied to the wrist

Morphology and histology of the collapsed lunate in advanced kienböck disease.

Morphological and histological changes of 24 collapsed lunates in advanced Kienböck disease were investigated. Articular cartilage of a collapsed lunate were consistently attenuated or disappeared. The proximal articular cartilage facing the radius is generally more affected than the distal articular cartilage facing the capitate. There was one major fracture in addition to multiple minor fractures in the collapsed lunate. Fracture patterns were classified into three types according to the directions of major fracture, namely horizontal type, dual oblique type, and vertical type. There were vi

Clinical case report: understanding Kienböck's disease.

Kienböck's disease, or lunate osteonecrosis, is a rare condition characterized by avascular necrosis of the lunate bone in the wrist. The pathophysiology involves disruption of blood supply to the lunate, leading to bone ischemia and subsequent collapse. This case report presents the clinical course and diagnostic challenges of a patient diagnosed with Kienböck's disease. This case highlights the challenges on the importance of early diagnosis and appropriate treatment to mitigate progression and improve patient outcomes.

Intraosseous Ganglion Spanning the Scaphoid and Lunate: A Case Report.

Intraosseous ganglions (IOGs) are actually quite common but one spanning two adjacent carpal bones is uncommon. We report a case with an IOG spanning the scaphoid and lunate, which was treated surgically. A 16-year-old right-handed female noticed left wrist pain that started spontaneously five years previously. Physical findings indicated carpal instability in the left wrist. Posteroanterior radiographs of the left wrist showed small cysts in the lunate and scaphoid, while the lateral radiograph revealed volar flexion of the lunate. Bone curettage was performed using sharp curettes, and due to

Kienböck's disease in 2021.

The cause of Kienböck's disease is still unclear. It was initially considered as osteomalacia, before being recognized as avascular necrosis of the lunate. Its functional prognosis is doubtful, given that the progression often leads to wrist degeneration. Conservative treatment does not provide good results in adults; thus surgery is often needed. Certain anatomical factors such as the ulnar variance, configuration of the lunate or orientation of the radial glenoid have turned out not to contribute to necrosis but may contribute to lunate fracture. The lunate's vascularization can be precariou

Kienbock's disease: diagnosis and treatment.

Kienbock's disease, or osteonecrosis of the lunate, can lead to chronic, debilitating wrist pain. Etiologic factors include vascular and skeletal variations combined with trauma or repetitive loading. In stage I Kienbock's disease, plain radiographs appear normal, and bone scintigraphy or magnetic resonance imaging is required for diagnosis. Initial treatment is nonoperative. In stage II, sclerosis of the lunate, compression fracture, and/or early collapse of the radial border of the lunate may appear. In stage IIIA, there is more severe lunate collapse. Because the remainder of the carpus is

Histopathology of Kienböck's disease. Correlation with magnetic resonance and other imaging techniques.

Histopathological studies of extracted whole lunate bones obtained from 10 patients with Stage 3 Kienböck's disease at surgery for tendon-ball replacement were correlated with magnetic resonance imaging (MRI), computed tomography (CT) and tomography images made prior to surgery. A reforming zone, or a reactive interface between the reactive new bone and granulation tissue formation, and new vascularization were observed surrounding the bone necrosis area showing empty lacunae, fatty necrosis, and disappearance of osteoid. Findings of CT, tomography and microradiography of slices of extracted l

Rehabilitation Protocol for Lunate Fracture in a Clinical Case Report.

A carpal injury called a lunate fracture can cause severe carpal instability if treatment is not received. After the scaphoid, triquetrum, and trapezium, the lunate is the fourth most frequently fractured carpal bone. Due to lunate fracture, the functional prognosis is uncertain, and conservative treatment frequently results in surgery. Lunate fracture may be caused by anatomical features such as ulnar and radial variation, although necrosis is not commonly one of them. Vascularization in the lunate is unstable and dependent on the capsular arterioles. High shear stresses are frequently applie

Bipartite lunate incidentally noted on radiographs obtained for skeletal age estimation: A report of two cases.

Bipartite lunate is a rare congenital anomaly of carpal bones. We present a report of two cases where children referred for determination of skeletal age were incidentally found to have bipartite lunate. In both cases, radiographic appearance of lunate could have been mistaken for secondary centres of ossification in other carpal bones. It is imperative for all radiologists to be aware of this rare congenital anomaly in order to avoid pitfalls in clinical diagnosis.

Cartilage and subchondral bone distributions of the distal radius: a 3-dimensional analysis using cadavers.

OBJECTIVE: To quantify the spatial distributions of cartilage and subchondral bone thickness of the distal radius. DESIGN: Using 17 cadaveric wrists, three types of 3-dimensional models were created: a cartilage-bone model, obtained by laser scanning; a bone model, rescanned after dissolving the cartilage; and a subchondral bone model, obtained using computed tomography. By superimposing the bone model onto the cartilage-bone and the subchondral bone models, the cartilage and subchondral bone thickness were determined. Measurements along with the spatial distribution were made at fixed anatom
