Lung ultrasound of the dependent lung detects real-time changes in lung volume in the preterm lamb.

BACKGROUND: Effective lung protective ventilation requires reliable, real-time estimation of lung volume at the bedside. Neonatal clinicians lack a readily available imaging tool for this purpose. OBJECTIVE: To determine the ability of lung ultrasound (LUS) of the dependent region to detect real-time changes in lung volume, identify opening and closing pressures of the lung, and detect pulmonary hysteresis. METHODS: LUS was performed on preterm lambs (n=20) during in vivo mapping of the pressure-volume relationship of the respiratory system using the super-syringe method. Electrical impedanc

Breath-by-breath respiratory timing and volume control during periodic breathing.

We examined the control of respiratory pattern during non-rapid-eye-movement sleep-related periodic breathing (PB) in adults, with and without hypoxia. We analyzed 186 cycles of PB from 18 epochs occurring in eight subjects; the mean (+/- SD) cycle duration was 30.8 +/- 8.4 s. Significant oscillations occurred in inspired tidal volume (VT), inspiratory duration (TI), mean inspired flow, inspired minute ventilation, and expiratory duration (TE) (P less than 0.005). For each epoch of PB, moving cross-correlation (MCC) functions were employed to describe the time-dependent intervariable relations

Donor and recipient predicted lung volume and lung size after heart-lung transplantation.

Lung volumes after heart-lung transplantation (HLT) were recorded and compared with measurements at the time of assessment for surgery and the predicted values for recipients. The influence of donor lung size and recipients' underlying lung disease was evaluated. All patients underwent HLT between April 1984 and April 1991, and only those 82 who survived for at least 6 mo were studied. Mean total lung capacity (TLC) at preoperative assessment was 112% (SD = 28%) of the value predicted for recipients. One month after HLT, mean TLC was 83% (SD = 15%) of the predicted value but increased to 100%

Impairment of lung volume perception and breathing control in hypermobile Ehlers-Danlos syndrome.

Breathing difficulties and exertional dyspnea are frequently reported in hypermobile Ehlers-Danlos syndrome (hEDS); however, they are not clearly explained. An impaired proprioception or the addition of a cognitive task could influence ventilatory control. How can the perception of lung volume be measured? Is lung volume perception impaired in hEDS patients? Is the breathing control impaired during a cognitive task in hEDS patients? A device was developed to assess the accuracy of lung volume perception in patients with hEDS and matched control subjects. In the second step, ventilation was rec

In vivo imaging of canine lung deformation: effects of posture, pneumonectomy, and inhaled erythropoietin.

Mechanical stresses on the lung impose the major stimuli for developmental and compensatory lung growth and remodeling. We used computed tomography (CT) to noninvasively characterize the factors influencing lobar mechanical deformation in relation to posture, pneumonectomy (PNX), and exogenous proangiogenic factor supplementation. Post-PNX adult canines received weekly inhalations of nebulized nanoparticles loaded with recombinant human erythropoietin (EPO) or control (empty nanoparticles) for 16 wk. Supine and prone CT were performed at two transpulmonary pressures pre- and post-PNX following

A structural model of perfusion and oxygenation in low-flow states.

BACKGROUND: Recent investigations underscore the critical importance of ventilation strategies on resuscitation outcomes. In low perfusion states, such as cardiac arrest and traumatic shock, the rise in intrathoracic pressure that accompanies positive-pressure ventilation can significantly impede venous return and lead to a decrease in cardiac output. The optimal ventilation strategy in these "low-flow" states remains unclear. OBJECTIVE: To create a mathematical model of perfusion and oxygenation to predict the effects of PPV with both normotension and hypotension. METHODS: The lung pressure

A comparison of two definitions of abnormality on pulmonary outcome in epidemiologic studies.

Definition of abnormal outcome on lung function testing has not been well standardized. Observed test results have been compared with predicted values based on age, height, and sex in 2 ways: as percent of the predicted (PP) and in relation to limits established by the 95% confidence interval around the predicted (CI). The present study examines these 2 methods of comparison with respect to effect on pulmonary outcome in 377 railroad workers and 193 pipefitters exposed to asbestos. Using accepted PP and CI criteria to define abnormal test results, we classified subjects as restricted or obstru

Computer-aided quantitative MSCT measurements may be useful for congenital lung malformations surgical approach selection.

PURPOSE: To examine the association between the MSCT quantitative measurements of congenital lung malformations (CLM) and the selection of surgical approaches (lobectomy vs. lung-sparing surgery). METHODS: This retrospective study evaluated CLM surgical cases at our institution from 2016 to 2018. MSCT quantitative measurements were generated by a semi-automated approach: the volume of the lesion (Vlesion), the volume of the lesion-involved lobe (Vlobe), the volume of the lesion-involved lung (Vlung) and the volume of the total lung (Vtotal lung). The proportions of Vlesion to Vlobe (Plesion/l

Effect of immersion on lung capacities and volumes: implications for the densitometric estimation of relative body fat.

Immersion of 18 male subjects in water caused a 20.4% (787 ml) increase (P less than 0.05) in the mean inspiratory capacity (IC) whereas there were no changes (P greater than 0.05) in tidal volume (VT) and the frequency of respiration. All the means for the other pulmonary variables decreased (P less than 0.05) by varying amounts: total lung capacity (TLC) = 8.4% (599 ml), vital capacity (VC) = 5.5% (308 ml), functional residual capacity (FRC) = 42.6% (1386 ml), expiratory reserve volume (ERV) = 61.9% (1095 ml) and residual volume (RV) = 19.7% (292 ml). Variation of only the RV in the body den

Respiratory regulation and lung volume during aquatic and land-based exercise in healthy Young adults.

Elevated hydrostatic pressure during water immersion reduces lung volume and compliance at rest. These alterations may persist during exercise, influencing both the respiratory regulation and lung volume. This study compared respiratory regulation and lung volume between land-based (LC) and aquatic (AC) cycling matched for oxygen uptake (VO2). Ten healthy young adults underwent cycling at low and moderate intensities in both environments. Expiratory gas variables (VO2) and respiratory variables (minute ventilation and respiratory rate: VE and RR, respectively) were continuously measured using

Impact of Endobronchial Valve Treatment on Lung Function Decline.

INTRODUCTION: Endobronchial valve (EBV) treatment has been shown to be beneficial for patients with severe emphysema. The forced expiratory volume in 1 s (FEV1) was found to be significantly higher compared to baseline for up to 3 years after treatment although the magnitude of improvement gradually decreases over time. So far, it has not been investigated whether this treatment decelerates the decline in lung function. Therefore, our aim was to investigate the lung function decline before and after EBV treatment. METHODS: We included patients who were treated with EBVs in our hospital, of wh

Comprehensive integrated spirometry using raised volume passive and forced expirations and multiple-breath nitrogen washout in infants.

With the rapid somatic growth and development in infants, simultaneous accurate measurements of lung volume and airway function are essential. Raised volume rapid thoracoabdominal compression (RTC) is widely used to generate forced expiration from an airway opening pressure of 30 cmH(2)O (V(30)). The (dynamic) functional residual capacity (FRC(dyn)) remains the lung volume most routinely measured. The aim of this study was to develop comprehensive integrated spirometry that included all subdivisions of lung volume at V(30) or total lung capacity (TLC(30)). Measurements were performed on 17 hea

[The role of the small airways in childhood asthma].

INTRODUCTION: Asthma, the most common chronic disease in paediatrics, is characterised by an inflammatory process involving the proximal and distal airways, with remodelling of the airways and changes in respiratory function. STATE OF THE ART: Recent studies in children have provided evidence of damage to the small airways identical to that occurring in adults. In fact inflammatory cells as well as structural alterations of the airways have been found in both bronchial biopsies and broncho-alveolar lavage fluid in infants and young children. At the functional level this damage is manifest by

Lung function tests in neonates and infants with chronic lung disease of infancy: functional residual capacity.

This is the second paper in a review series that will summarize available data and discuss the potential role of lung function testing in infants and young children with acute neonatal respiratory disorders and chronic lung disease of infancy. The current paper addresses the expansive subject of measurements of lung volume using plethysmography and gas dilution/washout techniques. Following orientation of the reader to the subject area, we focus our comments on areas of inquiry proposed in the introductory paper to this series. The quality of the published literature is reviewed critically, an

Role of lung volumes in sleep apnoea-related oxygen desaturation.

We studied the influence of lung volumes on apnoea-induced desaturation in ten subjects with sleep apnoea syndrome. Lung volumes were measured by helium dilution in the sitting and supine position and closing volume with the single-breath nitrogen washout test. To characterize the severity of apnoea-induced desaturations, we determined a desaturation curve for each patient. This curve was obtained by plotting the fall in arterial oxygen saturation (SaO2) reached at the end of each apnoea against the apnoea duration. From this curve we selected two indices: 1) the SaO2 fall following 30 s of ob

Measurement of pressure-volume curves in patients on mechanical ventilation. Methods and significance.

In critically ill patients measurements of pressure volume curves has been suggested as a method for assessing the severity of lung injury and for monitoring the evolution of the lung disease; it can also guide the ventilatory adjustments to optimize the mechanical ventilation. The static pressure-volume curves are impaired in acute respiratory distress syndrome (ARDS). The evaluation of the lower and upper inflection point on the pressure-volumes curves at the bedside of patients with acute respiratory failure means to apply a "protective ventilatory strategy". The combined application of pos

Glottic dimensions in healthy men and women.

Glottic aperture is important in modulating respiratory system resistance. Male patients with obstructive sleep apnea (OSA) have a smaller glottic cross-sectional area compared to controls. Since OSA has a strong male predominance, we reasoned that glottic dimensions may differ between healthy men and women. Therefore, we utilized the acoustic reflection to measure glottic cross-sectional area in 44 non-smoking, non-obese, healthy subjects, 25 men and 19 women. Glottic area was measured during a continuous slow expiration from total lung capacity (TLC) to residual volume (RV). We compared glot

Differences in CT density between dependent and nondependent portions of the lung: influence of lung volume.

OBJECTIVE: Lung tissue, blood, and air determine the physical density of the lung and hence the attenuation measured on CT scans. These components are not homogeneously distributed throughout the lungs, and their relative proportion changes continuously during respiration. The objective of this study was to measure densities in various areas of the normal lung with CT and to examine the influences of gravity and of the degree of lung inflation on these densities. SUBJECTS AND METHODS: The subjects in the study were six healthy male volunteers. CT scans were obtained at the level of the aortic
