Systemic lupus erythematosus and exercise.

Systemic lupus erythematosus (SLE) is a rheumatic disease characterized by a variety of symptoms, especially fatigue, pain and reduced quality of life. Physical exercise is a useful tool for improving cardiovascular fitness, reducing metabolic abnormalities and fatigue and improving quality of life. However, very few studies have focused on the relationship between SLE and physical exercise. This paper reviews the main SLE symptoms that can be alleviated by exercising, as well as the results of studies seeking to analyse the exercise capacity and physical training possibilities of SLE patients

Hypoparathyroidism in systemic lupus erythematosus.

Hypoparathyroidism is a rare disease. The main cause of hypoparathyroidism is postsurgical hypoparathyroidism. However, cases of hypoparathyroidism in patients suffering from SLE exist although it is uncommon. Only three previous cases have been reported. We present the case of a woman suffering both from systemic lupus erythematosus and hypoparathyroidism. This reported association of hypoparathyroidism with lupus expands the spectrum of endocrine disorders seen in this disease. We suggest that there may be a common underlying pathophysiological process linking these diseases.

Infection in systemic lupus erythematosus.

BACKGROUND: Infections are common in patients with systemic lupus erythematosus (SLE) throughout the course of their disease and remain a source of mortality. The aim of this study was to determine the prevalence of infections, to describe their nature, and analyze their risk factors in adults with SLE. PATIENTS AND METHODS: We performed a descriptive study, at the Farhat-Hached Hospital in Sousse, and retrospectively analyzed the charts of 75 patients with SLE seen between 1990 and 2004. The group of patients with documented infections was compared to a control group. A logistic regression a

Systemic lupus erythematosus and pregnancy.

Women with systemic lupus erythematosus (SLE) face significant risks when embarking on a pregnancy, but attending a multidisciplinary clinic staffed by an experienced team can improve pregnancy outcome for women and their babies. Pregnancy in SLE should be planned and a management strategy should be agreed in full consultation with the patient, prior to conception. Pregnancy increases the likelihood of a lupus flare. It is not possible to predict when, or if, an individual patient will flare, although flare is more likely if disease has been active within 6 months of conception. Worsening of p

Systemic lupus erythematosus and pregnancy.

PURPOSE OF REVIEW: Pregnancy in patients with systemic lupus erythematosus is associated with a high risk of maternal disease exacerbation and adverse fetal outcome. This review summarizes recent published findings on lupus pregnancy. RECENT FINDINGS: The literature is in agreement that for most women with inactive and stable systemic lupus erythematosus, pregnancy is safe for both mother and fetus. The main risk factors for adverse pregnancy course and outcome are active disease, nephritis with proteinuria, hypertension, and maternal serum antibodies to SS-A/Ro, SS-B/La, cardiolipin, beta2-g

Hypoparathyroidism in systemic lupus erythematosus.

We present the case of a man diagnosed with lupus and hypoparathyroidism. This combined diagnosis has very rarely been reported, which is surprising given the evidence that idiopathic hypoparathyroidism can be due to autoimmunity, the multiplicity of organ systems that lupus can affect and the evidence that other endocrinopathies such as hypothyroidism do occur at increased rates in patients with lupus. Hypoparathyroidism does not always cause overt clinical symptoms but the resultant hypocalcemia be a significant health risk. In addition to presenting the case, we review the relevant literatu

Mucormycosis in systemic lupus erythematosus.

OBJECTIVE: To describe a case of mucormycosis in systemic lupus erythematosus (SLE) and to review other patients reported in the English literature. METHOD: A Medline search for articles about mucormycosis in SLE published between 1970 and 2002 was performed by using the key words "lupus," "mucormycosis," "zygomycosis," "Mucorales," "Rhizopus," and "Mucor." Cases were pooled for analysis, and the mycology, diagnosis, treatment, and outcome of mucormycosis in SLE was reviewed. RESULTS: Eight cases of mucormycosis in SLE were identified (female:male = 7:1). The mean age at the time of infectio

Stroke in systemic lupus erythematosus.

With improved control of the immunological phenomena of systemic lupus erythematosus, the epidemiology of this disease is changing. While both cardiac and neurological manifestations of systemic lupus erythematosus have been consistently described, there is increasing awareness of the need for general cardiovascular prevention in systemic lupus erythematosus. An excess of stroke is seen in systemic lupus erythematosus especially, in those cases with anticardiolipin antibodies. There is increasing evidence for the contributory role of other cardiovascular risk-factors, including hypertension, h

Pregnancy in systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) is an autoimmune disease that predominantly affects women of reproductive age. Pregnancy and its outcome is a major concern to most SLE patients. Queries regarding the risk of disease flares during pregnancy, chance of fetal loss, and the safety of various drugs are often raised. With the improvement in the understanding of the pathogenesis of SLE and the judicious use of immunosuppressive drugs, better disease control can now be achieved and SLE patients should not be deprived of the opportunity for bearing children. Prepregnancy counselling and close collab

Systemic lupus erythematosus and pregnancy.

Systemic lupus erythematosus (SLE) may improve, exacerbate or be unchanged during pregnancy. The activity of SLE at the time of conception has a major influence on pregnancy outcome: SLE patients with quiescent disease do well, while those with active, uncontrollable disease have a high abortion rate. There are specific autoantibodies that confer a high risk of abortion (e.g., IgG antiphospholipid antibodies). There are others, such as anti-Ro/SSA and anti-La/SSB, which confer a risk for neonatal lupus that includes lupus dermatitis, congenital complete heart block, thrombocytopenia and choles

Autoantibodies in systemic lupus erythematosus.

Cationic germ line gene-encoded anti-DNA antibodies appear to cause inflammatory lesions via deposition and in situ formation of immune complexes, but also perhaps through apoptosis of glomerular mesangial cells. Somatic mutation is not critical for the expression of the electrical net charge of anti-DNA. In one transgenic mouse model, the nephrogenicity of anti-DNA was dependent on the expression of interleukin alpha by epidermal cells. Anti-P autoantibodies are present in the serum of healthy children but are masked by IgG antibodies. Some anti-P antibodies appear to be a subset of antilymph

Autoantibodies in systemic lupus erythematosus.

Lupus anti-DNA may have higher homology with germline than those from normal subjects. However, in NZB/NZW mice, bacterial DNA is more antigenic than mammal DNA, which could indicate an antigen-driven origin. High-affinity antibodies to double-stranded DNA cross-react with small nuclear ribonucleoprotein and ribosomal P proteins. These cross-reactive anti-DNA may penetrate live cells. Antibodies to ribosomal P proteins are associated with neuropsychiatric, renal, and hepatic lupus involvement. IgG antibodies to (H2A-H2B)-DNA complexes antedate procainamide-induced lupus. Autoantibodies to some

[Erythermalgia and systemic lupus erythematosus].

We report three cases of erythermalgia associated with systemic lupus erythematosus corresponding to different clinical situations in such an association. The first patient developed erythermalgia during the course of systemic lupus erythematosus. In the second, erythermalgia preceded other symptoms of systemic lupus erythematosus by four years. In the third, erythromelalgia was not related to a flare-up of systemic lupus erythematosus, but to thrombocythemia, a complication of immunosuppressive therapy. These cases permit a discussion on terminology and classification of erythromelalgia and e

Asplenism and systemic lupus erythematosus.

Although functional hyposplenism, secondary to Fc-receptor blockage by circulating immune complexes saturation, has been described in systemic lupus erythematosus (SLE), only few cases of complete asplenism have been reported. We observed a 60-year-old woman with congenital asplenism who presented with active SLE. The course and the clinical characteristics of such patients are reviewed and the relationship between the asplenic state and initiation and severity of SLE are discussed. These patients are at high risk for fatal pneumococcemia and pneumococcal vaccine is recommended even if long te

Sacroiliitis in systemic lupus erythematosus.

A 19-year-old white woman with severe systemic lupus erythematosus developed symptomatic sacroiliitis early in her disease. Over the next 14 years she showed radiological progression of sacroiliitis. She had neither HLA-DR3 nor B27 antigens. The rare coexistence of SLE and ankylosing spondylitis may not be determined solely by genetic factors; sacroiliitis may be just an infrequent manifestation of SLE.

Anemia in systemic lupus erythematosus.

Anemia is a common finding in children with systemic lupus erythematosus (SLE). Anemia in SLE may result from several mechanisms and more than one may be operative at any time. Anemia of chronic disease is the most common, but a low hemoglobin can be caused by auto-antibodies to red cells as part of the auto-immunity, or it may be the result of impaired erythropoietin production by kidneys involved in the SLE, gastrointestinal blood loss from anti-inflammatory therapy, increased red cell destruction from hypersplenism or a drug-induced immune phenomenon. We briefly review the important process
