Follicular lymphoma (FL) is presented as a germinal centre B cell lymphoma that is characterized by an indolent clinical course, but remains - paradoxically - largely incurable to date. The last years have seen significant progress in our understanding of FL lymphomagenesis, which is a multi-step process beginning in the bone marrow with the hallmark t(14;18)(q32;q21) translocation. Follicular lymphoma (FL) is the most common subtype of indolent lymphomas. Several lines of evidence suggest that the prognosis of patients with FL has improved since the introduction of rituximab, although cure cannot be achieved. Although the treatment paradigm for FL has changed over the past decade with the introduction of rituximab and other agents, there is still no standard therapy to fit all patients. Follicular lymphoma (FL) grade 3B (FL3B) is defined as FL with more than 15% centroblasts per high resolution field present as solid sheets. Coexistence with diffuse large B-cell lymphoma (DLBCL) is frequent. Composite lymphoma is very rare and a combination of Hodgkin lymphoma and non-Hodgkin lymphoma and even histiocytic tumors can occur. Because of the unfamiliarity, not only can this cause diagnostic problems, but can also affect treatment plan. We report a case of composite lymphoma in a 40-year-old male. Initial biopsy showed a composite lymphoma of follicular lymphoma grade 1 and classic Hodgkin lymphoma. After chemotherapy, another lymph node was taken because of disease progression, which revealed follicular lymphoma, grade 3a without Hodgkin lymphoma component.

This case report describes an elderly man who presented with mild lower urinary tract symptoms without haematuria. During his workup, an incidental finding of a urachal mass was diagnosed by CT. He underwent robotic partial cystectomy with en bloc urachal excision achieving clear frozen section margins. Follicular lymphoma has been widely thought to be an incurable disease. However, if cure is defined as a remission that continues indefinitely and the patient eventually dies of another disease, some patients with follicular lymphoma are cured. Follicular lymphoma grade 3 acts much like diffuse large B-cell lymphoma, and cure is frequent. Even in follicular lymphoma grade 1 and 2, some patients have durable responses to initial therapy, and a significant proportion of patients can have durable responses after hematopoietic stem cell transplantation.
