PURPOSE OF REVIEW: Given the rarity of anaplastic large cell lymphoma (ALCL), studies evaluating new therapies have typically grouped ALCL together with other peripheral T cell lymphomas (PTCL). Thus, the treatment paradigm for ALCL largely mirrors that of PTCL in general. In this review, we discuss the current standard of care as well as emerging therapies, including antibody-based drugs, in systemic ALCL as well as primary cutaneous and breast implant-associated ALCL.
RECENT FINDINGS: High CD30 expression in ALCL has allowed the use of brentuximab vedotin, an anti-CD30 antibody-drug conjuga

Primary cutaneous anaplastic large cell lymphoma (pcALCL) forms 9% of the cutaneous T-cell lymphomas. It usually presents as solitary reddish brown ulcerating nodule or indurated plaque. Sometimes, it mimics other dermatological diseases such as eczema, pyoderma gangrenosum, pyogenic granuloma, morphea, and squamous cell carcinoma. Our case presented with large pyogenic granuloma like lesion with regional lymphadenopathy. Since pcALCL is rare, one can misdiagnose such cases and therefore high index of suspicion is necessary.

The clinical, prognostic, phenotypic, and genotypic findings of 30 patients with large cell anaplastic lymphoma (Ki-1-positive large cell lymphoma) were analyzed. There were 13 male and 17 female patients (male-female ratio, 0.8) whose ages ranged from 3 to 81 years of age (mean, 28 years of age; 67% of the patients younger than 30 years of age). The 5-year survival rate was 52%; this was better than that of other types of high-grade peripheral T-cell lymphoma. Histologic examination showed distinctive morphologic features such as tumor cell pleomorphism, sinus infiltration, fibrosis, partial

There is controversy in the literature as to whether anaplastic large-cell lymphoma of B-cell phenotype is related to the t(2;5)-positive T- or 'null' cell lymphoma of the same morphology. We report a study of 24 lymphomas with morphological features of anaplastic large-cell lymphoma which expressed one or more B-cell markers and lacked T-lineage markers. Clinical features were more in keeping with large B-cell lymphoma than with classical t(2;5)-positive anaplastic large-cell lymphoma, and immunostaining for anaplastic lymphoma kinase (ALK) protein provided no evidence for the (2;5) transloca

BACKGROUND: Anaplastic lymphoma kinase (ALK)-positive primary CNS anaplastic large cell lymphoma (ALCL) is an extremely rare pediatric malignancy. Its radiological appearance often mimics infectious or glial lesions, complicating diagnosis and delaying treatment.
OBSERVATIONS: The authors report the case of a 10-year-old immunocompetent female who presented with absence seizures and vomiting. Brain MRI revealed a left frontal lesion with peripheral contrast enhancement and diffusion restriction, suggestive of a cerebral abscess. She was started on antibiotics and underwent resection. Histopat

Anaplastic large cell lymphoma (ALCL), a subgroup of T-cell non-Hodgkin's lymphoma, is an uncommon tumor exhibiting CD30 positivity and a characteristic immunophenotypic profile. Histologically, ALCL is characterized by the proliferation of large, anaplastic lymphoid cells with eccentric horseshoe- or kidney-shaped nuclei and one or more prominent nucleoli. Rare cases have been cited in the literature of ALCL presenting primarily in the oral cavity. The purpose of this article was to present 2 instructive cases of CD30+, anaplastic lymphoma kinase-negative ALCL with oral and systemic involveme

Unlike systemic anaplastic large cell lymphoma, the vast majority of primary cutaneous anaplastic large cell lymphomas (C-ALCL) do not carry translocations involving the ALK gene and do not express ALK. Expression of ALK protein therefore strongly suggests secondary cutaneous involvement of a systemic anaplastic large cell lymphoma. Recent studies described a small subgroup of ALK-positive C-ALCL, but information on frequency, prognosis, and translocation partners is virtually lacking. A total of 6/309 (2%) C-ALCL patients included in the Dutch registry for cutaneous lymphomas between 1993 and

Anaplastic large cell lymphoma expressing anaplastic lymphoma kinase (ALK+ ALCL) is a distinct subtype of non-Hodgkin lymphoma. In this review, we discuss the historical findings that led to its classification as a unique disease, despite its varied clinical presentation and histology. We discuss the molecular mechanisms underlying ALK+ ALCL pathology and the questions that remain in the field. Finally, we visit how decades of ALK+ ALCL research has yielded more precise drugs that hold promise for the future.

Anaplastic large cell lymphoma (ALCL) is a rare CD30-positive T-cell non-Hodgkin lymphoma, primarily classified into two subtypes: ALK-positive ( ALK +) and ALK-negative ( ALK -). ALK + ALCL is commonly seen in younger patients and characterized by rapid progression, with the majority of patients achieving complete remission after chemotherapy. In contrast, ALK -ALCL generally affects older patients, presenting with similar clinical features but a poorer prognosis. Genetic rearrangements, such as DUSP22 and TP63, have a significant impact on patient survival. The current treatment regimen prim

Anaplastic large cell lymphoma (ALCL) is a rare form of mature T-cell non-Hodgkin lymphoma. In pediatric patients, most cases are anaplastic lymphoma kinase (ALK) positive. Despite intensive multiagent chemotherapy regimens, treatment failure rates remain at 25% to 30%. Recent advancements in targeted therapies, notably ALK inhibitors and the anti-CD30 antibody-drug conjugate brentuximab vedotin have demonstrated substantial activity in relapsed and refractory settings. Molecular detection of minimal disseminated disease (MDD) and minimal residual disease (MRD) offers improved prognostic strat

Anaplastic large cell lymphoma (ALCL), ALK negative (ALK-) is an aggressive lymphoproliferative disorder of mature T lymphocytes characterised by hallmark cells, CD30 positivity and lacking ALK protein expression. ALCL, ALK- has to be differentiated from peripheral T-cell lymphoma-not otherwise specified and classical Hodgkin's lymphoma. ALK- anaplastic large cell leukaemia should be considered in a patient with a history of ALCL, ALK- presenting with new leukaemia. We report a rare presentation of relapsed ALCL, ALK- with leukaemia after autologous stem cell transplantation in a 57-year-old m

Anaplastic Large Cell Lymphomas (ALCL) are clinically aggressive and pathologically distinct lymphoid neoplasms that originate from a mature post-thymic T-cell. The contemporary World Health Organization (WHO) Classification of Haematologic Malignancies recognizes two distinct subtypes of systemic ALCL: Anaplastic Lymphoma Kinase (ALK)-negative, and ALK-positive. An additional unique subtype of ALCL is known to arise after prolonged exposure to breast implants, known as Breast Implant Associated ALCL (BIALCL). While histologic features of ALCL subtypes have significant overlap, genomic studies

History A 57-year-old woman presented with right breast swelling and enlargement and a palpable mass in the right lower inner breast. There was no history of fever, trauma, skin redness, or nipple discharge. The patient had a history of breast augmentation, with retropectoral silicone implants placed 2.5 years ago. Screening mammography (images not shown) 7 months prior revealed bilateral breast implants but no other abnormality. Mammography and US of the right breast were performed at the initial evaluation. Subsequently, fluorine 18 fluorodeoxyglucose PET/CT and MRI were performed.

Anaplastic large cell lymphoma (ALCL) is a T-cell lymphoma, accounting for <5% of non-Hodgkin's lymphoma. Cutaneous involvement can be primary or secondary arising in systemic ALCL. The diagnostic feature in both is the presence of pleomorphic, CD30 positive hallmark cells. We present a case of ALCL in a 19-year-old male presenting as an ulcerated scalp swelling. Clinical impression was actinomycosis or scrofuloderma. Cytology smears showed large dispersed pleomorphic cells with hyperlobated nuclei and multinucleated giant cells. The differentials considered were ALCL, rhabdomyosarcoma, and po

Conjunctival tumors in children are uncommon and behave differently from those in adults. The vast majority of tumors are benign. Of the malignant lesions in this age group, lymphoma and melanoma are most commonly reported. Most lymphoid tumors in children represent lymphoid hyperplasia, not lymphoma. Clinical differentiation of these two entities is not possible and biopsy is required to establish the diagnosis. We present an unusual case of primary anaplastic large cell lymphoma of the conjunctiva in a young boy.

A 43-year-old woman presented with a few weeks' history of discomfort and swelling in her left breast. She had undergone bilateral breast augmentation 8 years previously. There were no risk factors for breast cancer. Clinical examination, mammography and breast ultrasound revealed a large left breast mass adjacent to the breast implant with enlarged axillary lymph nodes. Owing to diagnostic uncertainty, core biopsies were sent to a specialist unit which confirmed breast implant-associated anaplastic large cell lymphoma with involved lymph nodes. Staging investigations confirmed no distant dise

Lymphoma is a malignant neoplasm of lymphoid tissue which is divided into 2 groups: Hodgkin and non-Hodgkin. About 85% of non-Hodgkin lymphomas are B-cell lymphomas, and T-cell lymphomas are unusual. Anaplastic large cell lymphoma (ALCL) is a kind of non-Hodgkin lymphoma with T or NK cell origin that is rarely seen in the oral cavity and only 10 cases have been reported up to now. Here we present a case of ALCL metastasized to the oral cavity in a 32- year- old man with pervious history of ALCL which caused an ulceration in the posterior area of the hard palate. Radiography showed irregular re
