The "boomerang" malleus-incus complex in congenital aural atresia.

"Boomerang" malleus-incus fusion deformity is identified on axial high-resolution CT in a subset of patients with congenital aural atresia, and it is associated with an absent incudostapedial joint and stapes capitulum and attachment of the incus to the tympanic segment of the facial nerve canal. Twelve patients with this deformity were identified on a retrospective review of imaging from a cohort of 673 patients with congenital aural atresia, with surgical confirmation in 9 of these patients. Eight of 9 patients underwent partial ossicular replacement prosthesis reconstruction with improvemen

Epitympanic malleus fixation: correction without disrupting the ossicular chain.

The preoperative diagnosis of malleus fixation can be made consistently with a pneumatic otoscope. Nontympanosclerotic epitympanic malleus fixation, for the most part, has been corrected by disrupting the ossicular chain and then reconstructing the sound-conducting mechanism. Once the diagnosis of malleus fixation is established, the surgical approach should be modified to cope with the fixed malleus. A wide, inferiorly based tympanotomy flap affords ample access to the epitympanum and permits definitive resolution of the associated conductive hearing loss. Atticotomy and discreet osteotomy ca

Endoscopic Management of Malleus Head Fixation in Revision Stapes Surgery.

OBJECTIVE: Malleus ankylosis is an uncommon finding during stapes surgery, which can result in functional failure if undetected during primary surgery. Its management can be difficult and different approaches and solutions have been described to date, most of which require interruption of the ossicular chain. We report the management of such condition through a transcanal endoscopic technique, with preservation of ossicular chain integrity. PATIENTS: The procedure was carried out in a patient affected by otosclerosis with persistent hearing loss after primary stapes surgery. INTERVENTION: Su

Isolated Traumatic Fracture of the Malleus Handle Causing Hearing Fluctuation.

INTRODUCTION: Handle of malleus fracture is a rare condition with <100 cases having been reported. The clinical presentation is conductive hearing loss following a history of trauma, typically, during manipulation of the external auditory canal. The diagnosis of the condition is clinical and radiological. The options for treatment are either a hearing aid or ossiculoplasty. We describe an isolated case of malleus handle fracture after trauma associated with manipulation of the external auditory canal. CASE REPORT: A 56-year-old female, reported a right ear trauma. She suffered immediate otalg

[Verification of malleus head fixation : computed tomography as diagnosis standard].

Malleus head fixation and otosclerosis causing conductive hearing loss are often difficult to differentiate by clinical features. Up to now exploratory anterior tympanotomy was necessary to distinguish these pathologies. The case of a 22-year-old male suffering from conductive hearing loss of the right ear is presented. Otosclerosis was suspected and high-resolution CT scanning of the petrous bone was performed. The diagnosis was confirmed radiologically by the detection of an isolated malleus head fixation. Instead of explorative anterior tympanotomy, radiological and audiological controls we

A new self-fixing and articulated malleus grip stapedectomy prosthesis.

A new prosthesis for malleus-grip stapedectomy is presented: the Clip(R) Piston MVP according to Häusler. The titanium piston is equipped with a self-fixing clip mechanism for automatic fixation of the prosthesis on the proximal malleus handle as well as a ball and socket articulation allowing easy introduction of the piston at an optimal angle into the oval window as well as adjustment of the insertion depth. A first series of malleus-grip stapedectomies performed with the Clip Piston MVP shows a hearing gain of 20 to 50 dB and a residual air-bone gap of < or =20 dB in all cases. In one patie

Ossicular osteoma of the malleus-A rare diagnosis of middle ear mass.

Osteoma of the middle ear (MEO) is a rarity. Due to the benign nature and slow growth rate, MEO are often occasionally found by chance. Possible unspecific clinical symptoms may be conductive hearing loss, tinnitus, effusion, and a sense of fullness. If the osteoma is small and not visible in the otoscopic inspection, it may be confused with other conductive hearing loss etiologies, such as otosclerosis. Nevertheless, one should be aware of this rare but important differential diagnosis of middle ear lesions. This article presents an MEO case causing conductive hearing loss and outlines the di

Ossiculoplasty for Malleus Bar with Incudostapedial Disconnection and Fused Incudomalleolar Joint.

Malleus bar is an abnormal bony connection between the malleus handle and the posterior wall of the tympanic cavity. We report a patient with a malleus bar and another malformation of the ossicles. An 11-year-old boy presented with hearing impairment since early childhood. Computed tomography (CT) revealed a malleus bar with an incudostapedial disconnection in the right ear. At tympanoplasty, the malleus bar was first identified and removed. A fused malleus-incus, not visible on the preoperative CT, was found intraoperatively. Therefore, the fused malleus-incus was removed; then, the ossicular

A method to measure sound transmission via the malleus-incus complex.

BACKGROUND: The malleus-incus complex (MIC) plays a crucial role in the hearing process as it transforms and transmits acoustically-induced motion of the tympanic membrane, through the stapes, into the inner-ear. However, the transfer function of the MIC under physiologically-relevant acoustic stimulation is still under debate, especially due to insufficient quantitative data of the vibrational behavior of the MIC. This study focuses on the investigation of the sound transformation through the MIC, based on measurements of three-dimensional motions of the malleus and incus with a full six degr

Histopathological differences in bony destruction of malleus and incus following mastoidectomy.

OBJECTIVES: Upon direct inspection of surgically removed ossicles from the ears of patients with long-term post-mastoidectomy cavity problems, the extent of malleus destruction often appears greater in patients with a longer duration of cavity problems, whereas the extent of incus destruction does not appear to correlate with the duration of cavity problems. This study aimed to investigate this impression. MATERIALS AND METHODS: As a result of total middle-ear reconstruction, 41 ossicles (21 malleus and 20 incus bones) were obtained from 31 patients with post-mastoidectomy cavity problems. Th
