Integrated management of mandibular fractures and traumatic dental injuries: a literature review and clinical algorithm proposal.

BACKGROUND: Mandibular fractures are among the most common maxillofacial injuries and are frequently associated with traumatic dental injuries (TDIs), creating combined skeletal, occlusal, periodontal, and pulpal treatment priorities. Integrated protocols addressing both the fracture and the dental component remain limited. METHODS: The present literature review was conducted by means of a structured search of PubMed/MEDLINE, Scopus, and Web of Science Core Collection from 2000 to 2026, complemented by manual reference-list screening. A structured Population, Intervention/Exposure, Comparison

Success of surgical interventions for direct dental implant-related injuries to the mandibular nerve: a review.

To the best of our knowledge there are no guidelines regarding the surgical management of dental implant-related injuries to the mandibular nerve. This review aims to investigate the success of different surgical interventions. Neurosensory injury to the mandibular branch of the trigeminal nerve can occur during administration of local anaesthetic, elevation of the flap, preparation for osteotomy, and placement of the implant. Surgical interventions include extraction of the implant, external decompression, internal neurolysis, excision of a neuroma, neurorrhaphy, nerve grafting, and low-level

Study of cerebrospinal injuries by force transmission secondary to mandibular impacts using a finite element model.

Brain and cervical injuries are often described after major facial impacts but rarely after low-intensity mandibular impacts. Force transmission to the brain and spinal cord from a mandibular impact such as a punch was evaluated by the creation and validation of a complete finite element model of the head and neck. Anteroposterior uppercut impacts on the jaw were associated with considerable extension and strong stresses at the junction of the brainstem and spinal cord. Hook punch impacts transmitted forces directly to the brainstem and the spinal cord without extension of the spinal cord. Dea

Anatomic relationship between impacted third mandibular molar and the mandibular canal as the risk factor of inferior alveolar nerve injury.

Our aim was to explore the relation between the site of the mandibular canal and neurosensory impairment after extraction of impacted mandibular third molars. We organised a retrospective study of 537 extractions in 318 patients in which the affected tooth was intersected by the mandibular canal. This was verified by cone-beam computed tomography (CBCT), and we analysed the relation between the site of the canal and the likelihood of injury to the inferior alveolar nerve (IAN) after extraction of the third molar. The relation between the position of the root of the tooth and the mandibular can

Injury to Anatomic Structures Due to Fixation Device in Management of Mandibular Fracture.

INTRODUCTION: ORIF is the gold standard for the management of mandibular fractures. The chances of postoperative complications are due to infection, damage of the inferior alveolar nerve canal, or tooth root during the fixation of the fracture fragments. Fixation hardware placed in the vicinity of the vital structures may encroach on the anatomic structures, leading to postoperative complications. AIM: The aim of this study is to evaluate the injury to the anatomic structures in the proximity of the fracture line due to the fixation device. MATERIAL AND METHODS: This retrospective study was

Cervical injury related to mandibular fractures. A retrospective study of 315 cases.

Objective To define the association between mandibular fractures and alterations of the cranio-atlo-axial joint (CAAJ). Methods 315 cases of displaced mandibular fractures were retrospectively evaluated by CT scan and cervical X-ray for associated alterations of the CAAJ. Statistical analysis employed the chi-square and Cohen's kappa coefficient. Results Single fractures amounted to 69.84% and multiple fractures to 30.16% of total fractures. CT scans showed a rotation of the atlas on the same side of single fractures and subluxation of the CAAJ. Vertical derangement of the joint was observed i

C-spine injury and mandibular fractures: lifesaver broken in two spots.

BACKGROUND: Trauma is a leading cause of injury and mortality and may involve mandibular fractures and cervical spine injuries. Manipulation of the spine during trauma protocols and operative treatment has the potential to cause serious spinal cord injuries. The purpose of this study was to identify risk factors associated with cervical spine injury (CSI) in patients with mandibular fractures. METHODS: The National Trauma Databank (2007-2010) was used to identify patients with mandibular fractures. RESULTS: A total of 59,028 patients were identified and separated into adult and pediatric coh

[Anatomical rationale for lingual nerve injury prevention during mandibular block].

The topographic and anatomical study of lingual nerve structural features was done. It was revealed that during mandibular anesthesia possible lingual nerve injury can occur if puncture needle is lower than 1 cm. of molars occlusal surface level. The position of the lingual nerve varies withmandible movements. At the maximum open mouth lingual nerve is not mobile and is pressed against the inner surface of the mandibular ramus by the medial pterygoid muscle and the temporal muscle tendon. When closing the mouth to 1.25±0.2 cmfrom the physiological maximum, lingual nerve is displaced posteriorl

Mandibular whiplash. Part II. An extension flexion injury of the temporomandibular joints.

The single most common contributing factor in the onset of TMJ disorders is cranio cervical trauma. This is strongly supported by the referred literature and a wide range of clinical experience. Trauma is also one of the most frequently overlooked factors when developing an occlusion related treatment plan. Failure to take into account a previous injury that has caused permanent damage to the TMJs could prove to be very embarrassing. If the symptoms are in remission at the initiation of occlusal therapy, a tense situation can arise if an old injury becomes acute during treatment and the patien

Risk factors of nerve injury during mandibular sagittal split osteotomy.

There is little objective data about whether surgical technique or mandibular anatomy are a risk for inferior alveolar nerve (IAN) injury during bilateral sagittal split osteotomy (BSSO). Orthodromic sensory nerve action potentials (SNAPs) of the IAN were continuously recorded on both sides in 20 patients with mandibular retrognathia during BSSO operation. Changes in latency, amplitude, and sensory nerve conduction velocity (SNCV) at baseline and at different stages of the operation were analyzed. The SNAP latencies prolonged, the amplitudes diminished, and the SNCVs slowed down during BSSO (P

Incidence of inferior alveolar nerve injury in mandibular third molar surgery.

This paper documents the incidence of inferior alveolar nerve injury and resultant sensory disturbance encountered in the removal of 100 consecutive impacted mandibular third molars. Five cases of anesthesia and/or paresthesia resulted and all but one of these resolved within six months. The lack of direct correlation between surgical exposure of the neurovascular bundle intraoperatively and the proximity of radiographic images of tooth root and mandibular canal, with the occurrence of sensory deficit, is noted. Our findings support the 1979 Recommendations of NIHCDCRTM*, upon which the policy

[The effect of local anesthetics and injuries on the angiographic picture of the cat mandibular blood vessels].

The authors examined the effects of local anesthetics and traumata (traumatic dental extractions) on the angiographic picture of the mandibular vessels of the cat in the 10th, 70th and 120th minutes. The application of local anaesthetics results in a reduced filling of the inferior alveolar artery in the 10th and 70th minutes. The picture returns to normal in the 120th minute. The traumatic extraction produces more marked changes up to complete spasm of the vascular system of the common carotid artery. In the 120 th minute, the blood supply is restored; and the normal number of vessels appears

Neuromuscular pre-activation attenuates mandibular impact injuries: Dynamic finite element analysis of stress modulation in craniomaxillofacial structures.

The craniomaxillofacial region is highly susceptible to high-intensity impact injuries during contact sports, however, existing passive protective devices have limited energy absorption capacity. This study aimed to quantitatively assess the protective efficacy of neuromuscular pre-activation against mandibular impact injuries. A high-precision craniomaxillofacial biomechanical model was constructed using CT and MRI data from a healthy male subject (glabello-occipital length: 176 mm; vertex-menton height: 212 mm; bizygomatic breadth: 135 mm). The model included the cranium, maxilla, mandible,

Can magnetic resonance imaging (MRI) reduce lingual nerve injuries during mandibular third molar surgery? A scoping review.

BACKGROUND: Recent advancements in MRI, with its superior soft tissue resolution and ionising radiation-free nature, provide a promising solution for the limitations of current imaging modalities. This review aims to evaluate whether MRI can be utilised to reduce the risk of lingual nerve (LN) injury during mandibular third molar surgery (M3M). METHODS: Following PRISMA guidelines, the protocol was registered in PROSPERO (CRD42024625994). A systematic literature search was employed across MEDLINE/PubMed, Scopus, Web of Science, Cochrane Library, and Science Direct without language or date res

Trigeminal nerve injuries after mandibular oral surgery in a university outpatient setting--a retrospective analysis of 1,559 cases.

BACKGROUND: In general, trigeminal nerve injury is known as a potential risk of many surgical procedures in the oral cavity. Recent literature demonstrated that the risk of nerve injury is correlated with the experience of the surgeon. Therefore, the purpose of this study was to evaluate retrospectively the incidence of trigeminal nerve injuries in a teaching university setting. MATERIAL AND METHODS: From January 2000 to December 2009, a total of 1,559 patients underwent one intervention in the postcanine region of the mandible. Interventions included extractions, osteotomies, periradicular s

[The specific panoramic radiographic signs and their relation with inferior alveolar nerve injuries after mandibular third molar surgery].

The aim of the authors was to describe the classic specific panoramic signs (indicating a close spatial relationship between dental canal and third molar's root) on panoramic radiographic images and determine their role in the risk assessment, predicting inferior alveolar nerve (IAN) paresthesia after lower third molar removal. The authors represented an informative case, where the IAN was visible during the surgery. The exact knowledge of classic panoramic radiographic signs should help the determination of "high risk" cases predicting IAN paresthesia after mandibular third molar removal. The
