The purpose of the present study was to predict inferior alveolar nerve (IAN) exposure and associated neurosensory deficits after mandibular cyst surgery based on imaging findings. The study includes 193 sites in 184 consecutive patients who underwent enucleation of cystic lesions around a mandibular third molar (MM3) and extraction of the associated MM3. Absence/presence of white lines on panoramic radiographs (PR) and absence/presence of cortication surrounding the mandibular canal on computed tomography (CT) are evaluated as predictor variables. Outcome variables are operative IAN exposure

PURPOSE: The purpose of the present systematic review was to assess the added value of panoramic radiography in predicting postoperative injury of the inferior alveolar nerve (IAN) in the decision-making before mandibular third molar (MM3) surgery.
MATERIALS AND METHODS: MEDLINE and EMBASE were searched electronically to identify the diagnostic accuracy of studies that had assessed the predictive value of 7 panoramic radiographic signs, including root-related signs (darkening of the root, deflection of the root, narrowing of the root, and dark and bifid apex of the root) and canal-related sig

Injury to the marginal mandibular nerve (MMN) as an isolated complication of posterior cervical spine surgery performed in the prone position is exceedingly rare, with only a handful of documented instances in indexed surgical literature. We present the case of a 60-year-old gentleman who developed unilateral lower facial deviation immediately following a seven-hour posterior cervical decompression and multilevel instrumented stabilization procedure. The deficit was confined entirely to the distribution of the MMN, with no involvement of other facial nerve branches or adjacent peripheral nerve

PURPOSE: A surgical procedure to minimize the incidence of inferior alveolar nerve injury (IANI) in deeply impacted mandibular third molars (IMTMs) has been proposed. Our study compared the near-term outcomes between coronectomy and traditional extraction of IMTMs and evaluated the long-term complications after coronectomy using cone-beam computed tomography (CBCT).
PATIENTS AND METHODS: A prospective study was performed of patients with IMTMs at high-risk of IANI using radiographic examination and CBCT. The patients were divided into 2 groups: a coronectomy group and an extraction group. The

The extraction of mandibular third molars can lead to injury to the inferior alveolar nerve. Hence, it is important to assess the proximity of the root to the inferior alveolar canal before extraction. The classification system of Pell and Gregory and the Winter classification are commonly used to evaluate the positional relationship of the third molar based on radiographs. This retrospective study involving 105 mandibular third molars was performed to assess whether these systems reflect the proximity of the root to the canal (based on computed tomography images), and to identify risk factors

OBJECTIVES: The aim of this retrospective study was to determine the incidence of inferior alveolar nerve (IAN) injury in patients with mandibular fractures by correlating fracture displacement and mandibular canal involvement on preoperative computed tomography (CT) imaging with clinically documented nerve deficits - and to develop the first CT-based displacement classification for predicting the likelihood of IAN injury.
MATERIALS AND METHODS: We retrospectively reviewed mandibular fractures and available preoperative CT scans. Fracture location, cortical and canal displacement, and nerve f

Carotid endarterectomy (CEA) is a frequently used surgical treatment for carotid artery stenosis. Cranial and peripheral nerve injuries are among the intraoperative complications. Diagnosing isolated injuries of the marginal mandibular branch of the facial nerve after CEA is challenging and leads to oral motor dysfunction that affects the patient's quality of life. Early diagnosis and treatment are valuable, since delayed diagnosis requires a surgical restoration of the affected nerve or muscle. In this article, we present a case of marginal mandibular nerve injury after CEA to increase the aw

The objective of this study was to investigate the incidence of sensory impairment of the lingual nerves following lower third molar removal and to compare the outcome with various operative variables. A total of 1200 mandibular third molars were removed under local anaesthesia. Predictor variables were categorized as lingual flap retraction, tooth sectioning, and buccal guttering. The outcome variable was the presence or absence of lingual nerve impairment. Different operative techniques were performed to identify independent predictors. Of the 1200 patients, 67 (5.6%) experienced transient s

The present study aimed to evaluate the effect of Piezosurgery on histopathologic features of the inferior alveolar nerve (IAN) damage after osteotomy of the buccal cortex of the mandible using piezoelectric devices in Hamdani sheep. A total of ten healthy mature female sheep were included. Each side of the mandible underwent two different experiments: the first experiment operated directly on the mental nerve by touching and activating the piezo tip on the nerve for ten seconds for the left side and thirty seconds for the right side. In the second experiment, the inferior alveolar nerve was t

The diagnostic value of several clinical, quantitative sensory tests (brush-stroke directional discrimination (BSD), touch detection threshold (TD), warm/cold (W/C) and sharp/blunt discrimination (S/B)), and electrophysiologic tests (mental nerve blink reflex (BR), nerve conduction study (NCS), cold (CDT), and warm (WDT) detection thresholds) in the recovery of inferior alveolar nerve (IAN) injury was evaluated in a prospective 1-year follow-up study of 20 patients after bilateral sagittal split osteotomy (BSSO). The subjective sensory alteration was assessed from patients' drawings. The predi

BACKGROUND: Impaction of mandibular third molars predisposes to pathological conditions including periodontal disease. Extraction of these teeth also may lead to damage to the nerve and to periodontal involvement of the second molars. This report describes a series of cases in which the third molars were orthodontically induced to erupt to prevent the sequelae associated with extraction.
METHODS: Impacted mandibular third molars in 18 patients were surgically exposed following placement of an orthodontic appliance. Depending on the individual case, 1 of 3 approaches was used: attachment of a

The management of impacted mandibular third molars can be a difficult clinical issue due to the proximity of the tooth to critical neurovascular structures. Surgical removal is routinely performed, but the technique carries a known risk of injury to the inferior alveolar and lingual nerves. The complication may induce temporary or permanent sensory impairment. Orthodontic traction can be an alternative to reposition the tooth in a more desirable location before surgical removal. The selection of the appropriate treatment is important to avoid a mechanical injury and to allow the adjacent bone

The aim of the study was to compare the accuracy of conventional and digital panoramic radiography (OPG) in relation to 4 specific high-risk signs (interruption of the superior cortical line, diversion, narrowing of the canal, and dark band of the root), which would indicate a close anatomic relationship between third molar roots and the inferior alveolar canal.Four hundred mandibular third molar surgical removals after conventional and 272 after digital radiographs were evaluated in the study. The association between postoperative inferior alveolar nerve (IAN) paresthesia and the presence of

We randomised 128 patients who required operations on mandibular third molars and who had radiological evidence of proximity of the third molar to the canal of the inferior alveolar nerve to one of two operations: extraction [n=102], and coronectomy [n=94]. Some roots were dislodged during intended coronectomy and were therefore removed, resulting in two subgroups (successful coronectomy n=58, and failed coronectomy n=36). The mean (S.D.) follow up was 25 (13) months. Nineteen nerves were damaged (19%) after extraction, none after successful coronectomy, and three (8%) after failed coronectomy

BACKGROUND: Reconstruction of the marginal mandibular nerve (MMN) is important for achieving optimal outcomes in the treatment of facial paralysis. However, the heterogeneity of injuries, ranging from extensive proximal facial nerve injuries to isolated distal MMN injuries, complicates meaningful outcome comparisons. This study assessed a new anatomical classification system for stratifying facial nerve injuries by injury location. The aim was to study MMN outcomes in proximal versus distal injuries and determine whether this system could provide a more reliable way to compare surgical results

This case report describes the conservative management of an incompletely erupted, subgingivally positioned mandibular right second molar with extensive caries and periapical pathosis in close anatomical relationship to the IAN canal. Complete extraction was initially considered as a provisional treatment option; however, CBCT revealed partial encirclement of the IAN canal by the roots, leading to a conservative treatment plan comprising root canal treatment followed by coronectomy. The initial treatment involved the extraction of tooth 48, followed by root canal treatment and tooth restoratio

OBJECTIVES: Extraction of impacted mandibular third molars (IMTMs) is the most common surgery performed in the Department of Oral and Maxillofacial Surgery. Inferior alveolar nerve (IAN) injury is a rare but severe complication, and the risk is significantly higher in cases of IMTM near the inferior alveolar canal (IAC). The existing surgical method to extract such IMTMs is either not safe enough or is time-consuming. A better surgical design is needed.
MATERIALS AND METHODS: From August 2019 to June 2022, 23 patients underwent IMTM extraction by Dr. Zhao at Nanjing Stomatological Hospital, A

OBJECTIVES: The present study aimed to assess whether anatomical variations of the mandibular canal are associated with neurosensory disturbances of the inferior alveolar nerve (IAN) following mandibular third molar removal.
METHODS: Two observers compared the detection of third molar root-nerve relations and bifurcations of the mandibular canal on panoramic radiographs and CBCT images of 201 patients undergoing removal of 357 mandibular third molars. Potential neurosensory disturbances of the IAN were surveyed ten days after surgery. Fisher's Exact was performed to correlate presence of cana
