A history of medical reports on the Boston Marathon: 112 years and still running.

INTRODUCTION/METHODS: We performed a systematic search for medical reports on the Boston Marathon, run annually since April 19, 1897 and studied medically since 1899. RESULTS: We identified 66 articles: 25 were related to cardiology; 10, exercise physiology; 8, metabolism; 5, neurology; 4, gastroenterology; 3, hematology; 3, several disciplines; and 8, nephrology, orthopedics, and general topics. The predominance of cardiology articles reflects concerns about the cardiac risks of exercise present in the early 20th century and persistent to this day. The authors and contributors included lumin

[Muscle cell proteins are selectively released into the blood stream by marathon running].

In 19 marathon runners of both sexes, plasma concentrations of total creatine kinase (CK) activity, CKMB mass, myoglobin and troponin I were determined before and immediately after the race. Total CK activity and myoglobin increased significantly in all runners and showed neither a correlation with the individual age of the runners nor with the time they needed to reach the goal. In 12 of the runners, CKMB mass increased during the race to a level suggesting myocardial necrosis. However, the runners did not show any detectable deterioration of cardiac function after the race. The appearance of

Circulating Antimicrobial Peptides as Biomarkers of Inflammation and Airway Dysfunction After Marathon Running.

Marathon running exerts physical stress and may lead to transient immune dysregulation, increasing susceptibility to airway inflammation and exercise-induced bronchoconstriction (EIB). This study investigated systemic levels of antimicrobial peptides in athletes and their association with EIB. Serum concentrations of angiogenin, human beta-defensin 2 (hBD-2), major basic protein (MBP), S100A8, and S100A8/A9 were measured in 34 marathoners and 36 half-marathoners at baseline, immediately after a race, and seven days postrace using enzyme-linked immunosorbent assays and compared with 30 sedentar

Biomechanical Responses to Level, Uphill, and Downhill Running Segments During an Outdoor Marathon.

This study investigated running biomechanics across varying slopes during a mass-participation road marathon. Although laboratory studies suggest specific gait modifications occur in response to varying slopes, data from outdoor competitive environments remain scarce. We monitored 39 distance runners using shoe-mounted inertial measurement units to capture spatiotemporal, footstrike, and wearable-derived loading data across level, uphill (+3%), and downhill (-3%) segments of the 2025 Hong Kong Marathon. Results demonstrated slope-dependent modifications, with uphill running characterized by sl

Plasma volume changes in middle-aged male and female subjects during marathon running.

Circulatory fluid shifts were studied in middle-aged runners (6 males and 5 females, ages 32-58 yr) during a 42.2-km marathon race run in mild weather (dry-bulb temperature = 17.5-20.4 degrees C). Running times for the subjects were 3:12-4:40 (mean values were 3:34 for males and 4:10 for females). Venous blood samples were taken without stasis in all subjects seated at rest before the start of the race and within 3 min of finishing; eight of the subjects also paused for samples at 6 and 27 km during the race. At 6 km, body weight loss averaged less than 1%, whereas plasma volume (PV) had decre

Associations between environmental factors and running performance: An observational study of the Berlin Marathon.

Extensive research has delved into the impact of environmental circumstances on the pacing and performance of professional marathon runners. However, the effects of environmental conditions on the pacing strategies employed by marathon participants in general remain relatively unexplored. This study aimed to examine the potential associations between various environmental factors, encompassing temperature, barometric pressure, humidity, precipitation, sunshine, cloud cover, wind speed, and dew point, and the pacing behavior of men and women. The retrospective analysis involved a comprehensive

The impact of advanced footwear technology on running performance and pacing in world marathon majors.

INTRODUCTION: This study investigated the impact of Advanced Footwear Technology (AFT) on marathon performance and pacing strategies within the World Marathon Majors (WMM). While AFT is known to improve running economy (RE), it remains unclear whether AFT enhances overall running velocity across the race or primarily alters pacing strategies. METHODS: We analyzed the top 10 performances from the Berlin, Chicago, London, New York City, and Tokyo marathons across two periods: the non-AFT era (2013-2015) and the standardized AFT era (2022-2025). There were 221 male athletes and 211 female athlet

Acute impact of first-time marathon running on the heart in middle-aged men.

BACKGROUND: Middle-aged men seem to be particularly susceptible to adverse cardiovascular effects of endurance exercise. The acute impact of training, marathon finishing and recovery on the heart has not been extensively investigated in such at-risk novice marathon runners. METHODS: We prospectively quantified cardiac changes in middle-aged men (41.2±4.5 years; n=17) who participated in their first marathon run at four time points: at baseline, after 16 weeks of training, within 10 hours after completing the 2021 Amsterdam Marathon run and after 4 weeks of recovery. The influence of marathon running on resting-state EEG activity: a longitudinal observational study.

Physical activity (PA) has positive effects on various health aspects and neuronal functions, including neuronal plasticity. Exceeding a certain exercise frequency and duration has been associated with negative effects. Our study investigated the effects of excessive PA with a marathon run (MA) and regular PA (training and recovery phases) on electrocortical activity, as measured by electroencephalography (EEG). Thirty healthy marathon runners (26 male, 45 ± 9 yrs) were enrolled in the study. Four resting-state 32 channel EEG recordings were conducted: 12-8 weeks before MA (T-1), 14-4 days pri

24-Hour Ultra-Marathon Running: A Narrative Review of Performance Factors and Physiological Impacts.

The 24-hour ultra-marathon is a specific race format with a long tradition and high popularity. To date, no comprehensive review has systematically summarized the scientific literature on 24-hour ultra-marathon running. We performed a comprehensive search in the PubMed and Scopus databases, covering studies published until the end of 2025. The participation of runners and finishers in 24-hours has increased in the past decades. Most participants in 24-hours are age group or master runners older than 35 years. 24-hour ultra-runners typically cover distances exceeding 100 km per event, with an a

Assessment of serum free light chain levels in healthy adults immediately after marathon running.

BACKGROUND: Immunoglobulin κ and λ free light chains (FLC) are important serum biomarkers for diagnosing and monitoring plasma cell dyscrasias (via the κ:λ FLC ratio), and assessing immune competence and activation status (via ∑FLC). FLCs are produced, in excess of heavy chains, from healthy plasma cells during immunoglobulin production, but unlike intact immunoglobulins that are cleared by cellular catabolism over a number of weeks, FLC are rapidly cleared from the bloodstream by the renal glomerulus with a half-life of 3 (κ FLC)-6 (λ FLC) hours. Marathon running has been shown to acutely and
