Retro- and prospective histologic and immunohistochemical analysis of safety margins following complete excision of less than 1 mm melanomas.

Reexcisions for melanoma do rarely present residual melanoma. To analyze the number of positive margins in reexcisions of in situ and <1 mm melanomas. To see whether the immunohistochemical (IHC) panel (Preferentially expressed antigen of melanoma (PRAME), Sry-related HMg-Box gene 10 (SOX 10), Human melanoma black 45 (HMB45), and Melan A) detected additional cases of melanoma. Three pilot cohorts (retrospective, prospective, and direct safety margins) were analyzed on the persistence of melanoma in reexcisions. Among the 97 cases of the retrospective cohort (27 in situ and 69 invasive melanoma

Cutaneous squamous cell carcinoma: predictors of positive and close margins and outcomes of re-excision in Northland, New Zealand.

BACKGROUND: Non-melanoma skin cancer (NMSC) is the most commonly diagnosed and costly cancer in Australasia. Cutaneous squamous cell carcinoma (cSCC) accounts for approximately 25% of NMSC. A better understanding of predictors of close and positive margins following surgical excision will help guide treatment. METHODS: A retrospective study was carried out of all primary cSCC histologically diagnosed in Northland, New Zealand in 2015. The cohort was identified by searching the regional pathology database. The primary outcome of interest was positive and close (≤1mm) margin rate following surg

Significance of disease extent in high-grade cervical intraepithelial neoplasia excised with negative margins by loop electrosurgical excision procedure.

Approximately 15% of patients with high-grade cervical intraepithelial neoplasia (CIN 2-3), treated by the loop electrosurgical excision procedure (LEEP) will experience a persistence or recurrence of their disease. The full spectrum of pathologic factors that may be predictive of an increased probability of this event remains to be elucidated. In this work, we evaluate the significance of disease extent in a group of patients with CIN 2 to 3 that was completely excised by LEEP (ie, endocervical and ectocervical margins were negative). From 102 patients whose LEEP specimens had negative margin

Margin status after loop electrosurgical excision procedure (LEEP) and laser excision in patients with high-grade cervical neoplasia.

INTRODUCTION: Early management of high-grade cervical intraepithelial neoplasia is one of the key steps in cervical cancer prevention after vaccination and screening. It relies on lesion removal by surgical excision, and the French National Cancer Institute (INCa) in 2016 recommend using a loop electro excision procedure (LEEP). However, the laser excision method seems to be as effective with similar rates of negative excision margins and postoperative clearance of HPV. OBJECTIVE: The main objective of our study was to compare the laser excision method to LEEP by evaluating the quality of the

Impact of age on surgical excision margins for vulvar squamous cell carcinomas: A multicenter study by the francogyn group.

INTRODUCTION: Vulvar cancer is a rare cancer, it most often affects older women, with tumours of more advanced size and stage than in younger patients. The first-line treatment for vulvar cancer is surgery. Current European and American guidelines recommend negative histological margins. As tumor size is greater in older patients, the aim of this study was to assess the impact of patient age on surgical excision margins in squamous cell carcinomas of the vulva. MATERIAL AND METHOD: This was a retrospective multicenter observational study. A descriptive analysis of the population was performed

Negative margins and negative HPV tests after large loop excision of the transformation zone: A nationwide historical cohort study.

INTRODUCTION: Adequate treatment of cervical precancer, defined as negative margins and a negative HPV test post-treatment, is essential for cervical cancer prevention, as inadequate excision of cervical precancer and a positive HPV test are associated with increased risk of recurrence. Here, we aimed to describe trends in the proportion of women receiving adequate treatment in Denmark. MATERIAL AND METHODS: Nationwide historical cohort study including Danish women ≥18 years treated with large loop excision of the transformation zone (LLETZ) from 2013 to 2022. Using the Danish Pathology Regis

The clinical relevance of excisional margins after large loop excision of the transformation zone for the persistence of cervical dysplasia.

OBJECTIVE: A new guideline on population-screening cervical cytology was introduced to improve diagnosis and management of (pre-)malignant cervical lesions. Subsequently, more colposcopies and more large loop excision of the transformation zone (LLETZ) were performed. There is little information about the relevance of positive margins for cervical intraepithelial neoplasia (CIN) after LLETZ. This study assesses the clinical relevance of margins on the presence of CIN. METHODS: In this retrospective study, 567 women who had undergone LLETZ due to cervical dysplasia between January 2017 and Dec

Excision margins and prognosis in cutaneous melanoma: retrospective population-based cohort study.

BACKGROUND: The impact of surgical excision margin size on the survival of patients with cutaneous melanoma (CM) is controversial. The aim of this study was to assess the impact of excision margins on patient outcomes. METHODS: This retrospective population-based follow-up study analysed data from patients presenting with CM without extranodal disease. The linear distance of CM from the excision margin was assessed microscopically. Regarding overall and melanoma-specific survival estimates, Kaplan-Meier analysis, the Cox proportional hazards model, and the subdistribution hazard ratio of the

Recurrence Patterns after Primary Excision of Invasive Melanoma with Melanoma in situ at the Margin.

The significance and management of melanoma in situ (MIS) at the margin of excision of invasive melanoma is debated. Patients undergoing excision of invasive melanoma from 2000 to 2016 with MIS at the margin were identified. A cohort without MIS was matched for age, gender, location, and Breslow depth. Thirty-two patients with 33 cases of MIS at the margin were identified. Melanoma was located on the head/neck (66.7%), extremities (24.2%), and trunk (9.1%). Median Breslow depth was 1.0 mm (range 0.25-10.80). Margin treatment included re-excision (45.5%), re-excision plus imiquimod (3.0%), imiq

Positive pathological margins after loop electrosurgical excision procedure - Management and outcome.

OBJECTIVE: Pathological involvement of cervical conization margins is a risk factor for recurrence, although management of these patients is controversial. We aimed to define risk factors for positive margins and compare recurrence following additional surgical intervention compared to conservative management. METHODS: A retrospective study of all conizations at our center between 2010 and 2019. Univariate analysis identified characteristics associated with positive margins. Women were stratified by mode of management comparing three groups (surveillance, repeat conization or hysterectomy) th

Surgical excision margins in primary cutaneous melanoma: A systematic review and meta-analysis.

BACKGROUND: The main treatment of primary cutaneous melanoma is surgery. This review aims to assess the width of excision margin that minimises the risk of adverse outcome from surgery, locoregional recurrence, distant recurrence, and death. METHODS: PRISMA guidelines were followed. MEDLINE, EMBASE, and four other databases were searched by using the term "melanoma", "margin", and limiting the search to randomised clinical trials (RCTs). RESULTS: Seven RCTs involving 4579 patients data were analysed. No statistically significant difference was found in locoregional recurrence RR 1.09 (95%CI

Arthroscopic Excision of Infraspinatus Calcific Tendinitis With Double-Row Margin Convergence Repair.

Calcific rotator cuff tendinitis is a common cause of shoulder pain. It is thought to be an active cell-mediated process although the exact pathophysiology remains unclear. It commonly affects the supraspinatus tendon. The condition is generally self-limiting and can be managed with appropriate nonoperative treatment; however, some cases may need surgical excision. Complete removal of the calcific deposits may result in large significant defects. Infraspinatus calcific deposits were seldomly described. This report illustrates a double-row suture bridge technique with margin convergence of a la

Margin comments in dermatopathology reports on dysplastic nevi influence re-excision rates.

BACKGROUND: Dermatopathology reports influence clinical management, but it is not clear to what extent comments on margin involvement of histopathologically dysplastic nevi (HDN) influence decisions about re-excision or complete excision. OBJECTIVE: We sought to determine if standardized margin comments (MCs) on HDN influence re-excision rates. METHODS: By reviewing medical records, we compared re-excision rates of HDN reported with (May 2011 to December 2012) and without (January 2007 to December 2010) standardized MCs, and surveyed clinicians to assess perceptions of the impact of MCs on t

Second intention healing after shave excision of benign tumors on the lid margin.

BACKGROUND: Defects caused by excision of benign lid margin tumors are conventionally repaired by reconstructive surgery. However, second intention healing is another option for managing wounds on the lid margin. OBJECTIVE: To evaluate the effectiveness of second intention healing after a shave excision of benign tumors on the lid margin. METHODS: Lid defects following a shave excision of the lid margin tumor were allowed to heal by second intention in 25 patients (26 lesions). The epithelialzation period was calculated, and cosmetic and functional results and complications were evaluated by

Optimal excision margins for primary cutaneous melanoma: a systematic review and meta-analysis.

OBJECTIVE: To determine in patients with localized primary melanoma of the trunk or extremities the optimal excision margin that achieves the highest disease-free survival and overall survival and the lowest local recurrence rate. DATA SOURCES: Trials comparing 2 different excision margins were identified by searching MEDLINE from 1966 to May 2002 using the term "melanoma," subheading "surgery," and limiting the search to human studies and randomized controlled trials (RCTs). Additional studies were found using the MeSH term "surgical procedures, operative," combining with "melanoma," and lim

Can a more detailed evaluation of excision margins refine cytologic follow-up of women post-LLETZ for high-grade dysplasia?

The relationship between dysplastic changes in the cervical epithelium and progression to in situ carcinoma and invasive carcinoma has been extensively studied. The removal of dysplastic epithelium through the long loop excision of the transformation zone (LLETZ) in 95% of the cases is curative. About 18% to 37% of LLETZ specimens with dysplasia at the margins have recurrent/residual disease. Earlier small studies suggest that the degree of dysplasia at the margins could predict for recurrence and allow a risk-based stratification of follow-up. We tested this hypothesis in a large group of wom

Margin matters: analyzing the impact of circumferential margin involvement on survival and recurrence after incomplete total mesorectal excision for rectal cancer.

BACKGROUND: Incomplete mesorectal excision during rectal cancer surgery often leads to positive circumferential margins, with uncertain prognostic impacts. This study examines whether negative margins can mitigate the poorer prognosis typically associated with incomplete total mesorectal excision (TME) in rectal cancer surgery, thus potentially challenging the prevailing emphasis on complete mesorectal excision. PATIENTS AND METHODS: A retrospective analysis was conducted on patients who underwent proctectomy for rectal adenocarcinoma with incomplete TME at a single center from 2010 to 2022.

Assessment of mesorectal fascia status in MRI compared with circumferential resection margin after total mesorectal excision and predictors of involved margins.

BACKGROUND: Circumferential resection margin is an important prognosticator for total mesorectal excision outcome. We investigated the status of mesorectal fascia on magnetic resonance imaging compared with circumferential resection margin on pathology and factors associated with status change. METHODS: This was a retrospective analysis of a prospective database of rectal cancer patients who underwent surgery. Mesorectal fascia status on magnetic resonance imaging done before neoadjuvant therapy and circumferential resection margin status on pathology were compared. The study outcomes were fa
