Role of Axillary Reverse Mapping in the Prevention of Lymphedema after Modified Radical Mastectomy in Breast Cancer.

INTRODUCTION: Lymphedema is a well-established complication of modified radical mastectomy (MRM). Axillary reverse mapping (ARM) can help spare the upper limb lymph nodes thus preventing lymphedema. This study assessed the role of ARM in lymphedema prevention after MRM. MATERIALS AND METHODS: Breast cancer patients eligible for MRM were included and divided into two groups of 50 patients each. Group A underwent ARM with MRM and Group B underwent MRM only. Follow-up was done at 3 months, 6 months, and 1 year to assess for lymphedema. Preoperative and postoperative arm circumference was compare

Combined Thoracic Paravertebral Block-Interscalene Block as a Primary Anesthetic for Modified Radical Mastectomy: A Case Report.

BACKGROUND: Regional anesthesia as a primary anesthetic can offer merits over general anesthesia for patients having multiple comorbidities who are at a high risk of perioperative morbidity and mortality. Thoracic paravertebral block (TPVB) and interscalene block (ISB) have been used widely to improve the quality of postoperative analgesia after breast surgery. OBJECTIVE: There are limited data on the feasibility of combining TPVB-ISB as a sole anesthetic technique for extensive breast surgery with axillary lymph nodes dissection. CASE PRESENTATION: In this report, the author presented a suc

Nursing intervention in women who developed lymphedema after undergoing a modified radical mastectomy: a pre-experimental study.

The aim of this study was to analyse the effect of a nursing intervention in increasing the therapeutic self-care demand in patients with breast cancer-related lymphedema after a mastectomy. The pre-experiment was carried out on women in the State Cancer Institute in Colima, Mexico. Thirty women who had undergone a mastectomy were included in this study. Each woman received a nursing intervention to increase her knowledge, skills and motivation, which are the components of therapeutic self-care demand. Students' t and P values were calculated for paired means, and the McNemar X2 test was perfo

Evaluation of shoulder girdle strength more than 12 month after modified radical mastectomy and axillary nodes dissection.

BACKGROUND: Muscle strength is necessary for upper body normal function. Upper extremity function impairments have been reported in breast cancer (BC) survivors. It is not possible to know precisely if cancer adjuvant therapy such as radiation and chemotherapy had any effect on the unaffected arm. The aim of this study was to compare shoulder girdle strength among women with BC and similarly aged women without cancer. MATERIALS AND METHODS: Thirty-three postmenopausal women (51 ± 6.46 years) with BC who underwent surgery, chemotherapy, and radiation therapy and 30 healthy postmenopausal women

Anesthetic effect of ultrasound-guided multiple-nerve blockade in modified radical mastectomy in patients with breast cancer.

INTRODUCTION: Routine anesthesia modality for modified radical mastectomy (MRM) includes general anesthesia (GA), epidural blockade-combined GA and nerve blockade-combined GA. However, GA has been associated with postoperative adverse effects such as vertigo, postoperative nausea and vomiting and requirement for postoperative analgesia, which hinders recovery and prognosis. Moreover, combined blockade of thoracic paravertebral nerves or intercostal nerves and adjuvant basic sedation for massive lumpectomy provided perfect anesthesia and reduced opioid consumption, whereas the excision coverage

Comparison between ultrasound guided erector spinae plane block and paravertebral block on acute and chronic post mastectomy pain after modified radical mastectomy: randomized controlled trial.

BACKGROUND: Inadequate acute postoperative pain management is linked to the effect on the stress response and development of chronic pain. A unique regional anaesthetic method that is becoming more important for postoperative pain management is erector spinea plane block (ESP). Since its initial description, physicians have questioned weather this novel easy method can take the place of paravertebral block (PVB). Our goal was to evaluate, in contrast to control group, the effects of ESP & PVB on acute and chronic post-mastectomy pain. METHODS: One hundred and five female patients undergoing m

Predictors of early arm lymphoedema in breast cancer patients treated with modified radical mastectomy -a prospective observational study.

BACKGROUND: Breast cancer is the most common malignancy among women globally, with 2.3 million new cases in 2022. Advances in early detection and treatment have improved survival, but long-term complications like breast cancer-related lymphoedema (BCRL) remain underrecognized. Affecting 6-40% of patients following axillary lymph node dissection, BCRL leads to chronic swelling, pain, and reduced quality of life. The risk is heightened in low- and middle-income countries (LMICs), where delayed diagnosis and limited awareness necessitate more aggressive interventions. Despite its profound physica

Nocardia Nova bacteremia and viral pneumonia in a breast cancer patient post modified radical mastectomy: a case report.

BACKGROUND: Nocardia nova (N. nova) is a Gram-positive aerobic actinomycete and a rare pathogen that can cause severe infections, particularly in immunocompromised patients. CASE PRESENTATION: We report here a rare case of N. nova bacteremia combined with viral pneumonia in a 54-year-old woman who underwent a modified radical mastectomy for breast cancer. Post-surgery, she developed high fever and lung inflammation. Initial empirical therapy with levofloxacin was administered for the pulmonary infection. Upon identification of N. nova through blood cultures and antibiotic susceptibility testi

Thoracic Epidural Anesthesia for Modified Radical Mastectomy in a High-Risk Patient: A Case Report With Literature Review.

Surgery is one of the mainstays of treatment in breast cancers. Typically, modified radical mastectomy (MRM) is done under general anesthesia (GA). However, GA is not a reasonable choice in patients with multiple comorbidities and difficult airways. Thoracic epidural anesthesia (TEA) is a reasonable and safe alternative to GA as it involves blunting of stress response and avoidance of airway handling apart from hemodynamic stability, lower analgesic consumption, superior postoperative analgesia, reduced postoperative nausea and vomiting, earlier resumption of feeding, and shorter duration of h

Combined ultrasound-guided Pecs II block and general anesthesia are effective for reducing pain from modified radical mastectomy.

Purpose: Combined regional and general anesthesia are often used for the management of breast cancer surgery. Thoracic spinal block, thoracic epidural block, thoracic paravertebral block, and multiple intercostal nerve blocks are the regional anesthesia techniques which have been used in breast surgery, but some anesthesiologists are not comfortable because of the complication and side effects. In 2012, Blanco et al introduced pectoralis nerve (Pecs) II block or modified Pecs block as a novel approach to breast surgery. This study aims to determine the effectiveness of combined ultrasound-guid
