Eosinophilia in systemic mastocytosis.

The diagnosis of systemic mastocytosis depends on the proper recognition of extensive mast cells infiltration in tissues. Accurate identification of the mast cells in tissues may be difficult when there is no clinical suspicion to initiate a special search for these cells. Four patients with atypical clinical features and eosinophilia in tissues and three with eosinophilia in blood had hematologic or histologic findings suggestive of myelofibrosis with myeloid metaplasia, and malignant lymphoma. It was the presence of blood and tissue eosinophilia in these patients that led to further investig

Systemic Mastocytosis: Multidisciplinary Approach.

Systemic mastocytosis (SM) is a heterogeneous group of diseases that affect almost exclusively adults and are defined by the proliferation and accumulation of clonal mast cells (MC) in various tissues. Disease subtypes range from indolent to rare aggressive forms. Although SM is classified as a rare disease, it is believed to be likely underdiagnosed. Major signs and symptoms mainly depend on MC activation and less frequent organ infiltration, typical of more aggressive variants. Diagnosis may be challenging, and symptoms can be aspecific and involve several organs. Therefore, it is advisable

CD30 in systemic mastocytosis.

CD30 is a transmembrane receptor, normally not expressed by mast cells, which regulates proliferation/apoptosis and antibody responses. Aberrant expression of CD30 by mastocytosis mast cells and interaction with its ligand CD30L (CD153) appears to play an important role in the pathogenesis and clinical presentation of systemic mastocytosis. This article highlights the expression profile and role of CD30 and CD30L in physiologic and pathologic conditions, the applicability of CD30 as a marker for systemic mastocytosis, the consequences of mast cell-expressed CD30, and the possibility of future

Systemic mastocytosis complicating pregnancy.

BACKGROUND: Systemic mastocytosis is a rare medical disorder in which an increased number of mast cells can precipitate immediate hypersensitivity reactions, leading to hypotension, shock, and death. It is characterized by persistent elevated serum tryptase levels. The few published reports on pregnancy complicated by systemic mastocytosis indicate favorable maternal and fetal outcomes in gravidas known to have systemic mastocytosis. CASE: A pregnant woman treated with terbutaline at 31 weeks of gestation developed severe hypotension which resulted in fetal demise; this was initially diagnose

[Systemic mastocytosis: clinical case].

We report a 38 years old female who, since her childhood, had a history of precisely limited, fixed maculo papular dark brown cutaneous lesions in the trunk and extremities. These lesions become erythematous or urticarial after rubbing, medication intake or scratching. She also had frequent episodes of tachycardia, flushing, headache, abdominal pain, arthralgia, diarrhea and vomiting. She was hospitalized in three occasions due to high frequency tachycardia, hypotension, generalized urticarial erythema and clouding of consciousness. Three of these episodes occurred after the ingestion of antii

[Systemic mastocytosis with eosinophilia].

A 67-year old female patient presented with marked eosinophilia in the peripheral blood. The patient had asymptomatic systemic mastocytosis which she had probably had for many years. At the time of diagnosis, the eosinophilia had already produced symptoms of cardiac insufficiency. The patient was treated with prednisone and hydroxyurea which reduced the eosinophil count. In spite of treatment cardiac symptoms progressed, the patient developed atrial flutter, and died in pulmonary oedema. The autopsy showed fibrotic changes in the heart as seen in the fibrotic end stage of Löeffler's endomyocar

Systemic mastocytosis. Extracutaneous manifestations.

The clinical, radiologic, ultrastructural, and histopathologic findings in 14 patients with systemic mastocytosis were evaluated. Seven patients had evidence of urticaria pigmentosa (UP) and seven patients presented with no recognizable cutaneous lesions. There were no major clinical differences between patients with or without UP except for splenomegaly, which was present in one/seven patients with UP and five/seven patients without UP and the median age, 44 in patients with UP, and 75 in patients without UP. Bone marrow involvement was present in 13/13 specimens studied. Involvement was both

Cimetidine in systemic mastocytosis.

54-year-old caucasian women with systemic mastocytosis manifested by gastrointestinal and dermatological symptoms that were treated with cyproheptadine, Lomotil (diphenoxylate-atropine), and cimetidine was studied. The patient consented to discontinue the H2-receptor antagonist, and to have the following values measured: serum and urine concentrations of cimetidine: complete blood count (CBC) with differential, serum chemistry, and urinalysis; and systemic mastocytosis symptomatology. The patient experienced prompt recurrence of gastrointestinal and dermatological symptoms after stopping the c

Urticaria pigmentosa and systemic mastocytosis.

KEY CLINICAL MESSAGE: Additional investigations for systemic involvement should be initiated once the diagnosis of cutaneous mastocytosis has been established in an adult patient. A serum tryptase can serve as a screening test for systemic mastocytosis, and persistent elevations should prompt further investigations, such as bone marrow studies. ABSTRACT: Urticaria pigmentosa (UP) is the most common form of cutaneous mastocytosis, presenting as a wide variety of macroscopic appearances. Cutaneous mastocytosis in pediatric patients usually does not present with systemic involvement, but more th
