[Critical analysis of fetal risk recognition by serum estriol determination].

In 901 pregnancies between 1978-1981 a total of 5281 serum estriol determinations were performed. 596 gravidas were high-risk patients (diabetes mell., toxemia, IUGR, premature labour, prolonged pregnancy, Rh incompatibility). In 142 of these high-risk pregnancies the estriol values were below the 10th percentile or the estriol decrease extended 40%. These cases were analysed individually, in order to answer the question, how often serum estriol values were decisive for active obstetrical management. Our data show that a drop of estriol over 40% was very rare (0.53%). For the newborn such an e

An encyclopedia of the cord blood metabolome reveals maternal-fetal interactions and disease risk.

Metabolites present in the mother traverse the placenta to supply energy, essential nutrients, and communication signals to the fetus. To gain a deeper understanding of fetal metabolism and the impacts of maternal metabolic health and medications on the fetus, we have created CordDB. Using mass spectrometry, we systematically document the metabolites and medications that enter and leave the fetus during birth, as well as the associated health records of the mother and newborn. These data reveal the metabolites consumed by the fetus, microbial metabolites (e.g., 3-indolepropionic acid), metabol

Microbes, molecules, and the maternal-fetal interface: rethinking the gut-placenta axis.

Placental biology is increasingly framed through a signaling paradigm in which maternal microbiome-derived mediators-rather than microbial colonization-affect the function of the interface. This review synthesizes evidence that circulating microbial signals, including short-chain fatty acids, tryptophan-derived indoles, bile-acid-linked ligands, microbe-associated molecular patterns, and bacterial extracellular vesicles, are associated with placental vascular development, immune regulation, nutrient transport, and endocrine programs-processes central to pregnancy outcomes. We integrate mechani

Human placental development and function.

The placenta is a transient fetal organ that plays a critical role in the health and wellbeing of both the fetus and its mother. Functionally, the placenta sustains the growth of the fetus as it facilitates delivery of oxygen and nutrients and removal of waste products. Not surprisingly, defective early placental development is the primary cause of common disorders of pregnancy, including recurrent miscarriage, fetal growth restriction, pre-eclampsia and stillbirth. Adverse pregnancy conditions will also affect the life-long health of the fetus via developmental programming[1]. Despite its cri

Fetal growth and development.

The foetal growth is, for the greater part, determined genetically. There are, however, favouring and inhibiting factors, which influence this growth. In such a matter, the mother, the placenta and the foetus play an important part. The mother shows as well metabolic as vascular adaptations. The placenta must let through nutrients and oxygen; the uteroplacental circulation is characterized by a high flow, together with a low vascular resistance. The foetus itself plays an important part owing to its own growth factors. The principal growth factors are insulin and the growth factors analogous t

[Role of maternal and fetal plasma proteins in transplacental transport of drugs].

One of the factors influencing the rate of drug transport through the placenta is their binding by maternal and fetal plasma proteins. Concentrations of the components binding the drug on both sides of the placenta influence significantly its distribution in the fetomaternal pair. It is emphasized that in plasma of a fetus or newborn there is observed the free fraction increase for most drugs--beta-adrenoblockers, local anesthetics, narcotic analgesics, antiarrthythmic and antiepileptic agents. As for salicylates, diazepam and valproic acid, one can note their accumulation in the fetal blood d

A prospective twin cohort investigates placental transfer efficiency as a potential determinant of fetal growth across a 300-xenobiotic maternal-fetal exposure landscape.

Prenatal exposure to environmental chemicals poses risks to fetal growth, yet most epidemiological studies have examined single chemical classes in isolation, leaving the broader multi-class maternal-fetal exposure landscape inadequately characterized. In addition, fetal exposure is typically inferred from cord blood concentrations, which conflates maternal body burden with transplacental transfer (TPT), obscuring the independent contribution of placental transport. We conducted a prospective cohort study screening 300 xenobiotics spanning multiple chemical classes, including PFAS, phthalate m

Exchange transfusion in neonatal myasthenia gravis.

An infant with transient neonatal myasthenia gravis had a double-blood-volume exchange transfusion because of maternal-fetal blood group incompatibility. This seemed to accelerate both decline in antiacetylcholine antibody titer and clinical improvement.

Impact of endocrine disrupting chemicals on macrophages at the maternal-fetal interface.

Pregnancy is a complex and dynamic process, immune homeostasis at the maternal-fetal interface is one of the keys to a successful pregnancy and essential for fetal nutrient exchange and the establishment of immune tolerance. Healthy pregnant women with normally functioning immune systems can successfully maintain a semi-allogeneic fetus to full term without immune-mediated rejection, many immune cells including macrophages, NK cells, T cells, B cells and dendritic cells are involved in this process. In particular, macrophages play a vital role in the establishment of immune tolerance, infectio

Placental amino acids transport in intrauterine growth restriction.

The placenta represents a key organ for fetal growth as it acts as an interface between mother and fetus, regulating the fetal-maternal exchange of nutrients, gases, and waste products. During pregnancy, amino acids represent one of the major nutrients for fetal life, and both maternal and fetal concentrations are significantly different in pregnancies with intrauterine growth restriction when compared to uncomplicated pregnancies. The transport of amino acids across the placenta is a complex process that includes the influx of neutral, anionic, and cationic amino acids across the microvilluos

Microplastic trafficking in maternal-fetal system: a systematic review and quantitative profiling linking particle characteristics to developmental biology.

Microplastics (MPs) have been detected across multiple maternal and neonatal biological matrices, suggesting widespread exposure during critical developmental stages. However, the mechanisms shaping their distribution and accumulation within the maternal-fetal system remain poorly understood. This systematic review and quantitative synthesis provides a comprehensive quantitative profiling of MP burden across six maternal and neonatal biological matrices placenta, breast milk, endometrium, amniotic fluid, meconium, and feces. Our analysis reveals three critical insights: (1) Tissue‑specific MP

Dynamic Function and Composition Changes of Immune Cells During Normal and Pathological Pregnancy at the Maternal-Fetal Interface.

A successful pregnancy requires a fine-tuned and highly regulated balance between immune activation and embryonic antigen tolerance. Since the fetus is semi-allogeneic, the maternal immune system should exert tolerant to the fetus while maintaining the defense against infection. The maternal-fetal interface consists of different immune cells, such as decidual natural killer (dNK) cells, macrophages, T cells, dendritic cells, B cells, and NKT cells. The interaction between immune cells, decidual stromal cells, and trophoblasts constitute a vast network of cellular connections. A cellular immuno

Is oxytocin a maternal-foetal signalling molecule at birth? Implications for development.

The neuropeptide oxytocin was first noted for its capacity to promote uterine contractions and facilitate delivery in mammals. The study of oxytocin has grown to include awareness that this peptide is a neuromodulator with broad effects throughout the body. Accumulating evidence suggests that oxytocin is a powerful signal to the foetus, helping to prepare the offspring for the extrauterine environment. Concurrently, the use of exogenous oxytocin or other drugs to manipulate labour has become common practice. The use of oxytocin to expedite labour and minimise blood loss improves both infant an

Prp-c and Prp-Sc at the fetal-maternal interface.

Scrapie is a naturally occurring prion (PrP) disease causing a fatal neurodegenerative disorder in sheep and goats. Previous studies suggest that scrapie is transmitted naturally through exposure to the scrapie agent in wasted placentas of infected ewes. This study determined the distribution and biochemical properties of PrP cellular (PrP-C) and the distribution of PrP scrapie (PrP-Sc) in reproductive, placental, and selected fetal tissues and fetal fluids in sheep. Glycosylated, N-terminally truncated, proteinase K-sensitive PrP-C with apparent molecular masses of 23-37 kDa was present in re

[Failure of differentiation of the trophoblast in trisomy 21].

The syncytiotrophoblast is the major component of the human placenta as it is involved in the feto-maternal exchanges and the secretion of pregnancy-specific hormones. In normal placenta, the multinucleated syncytiotrophoblast is formed by fusion of mononuclear cytotrophoblast cells. In trisomy 21-affected placenta, we show that it exists a defect (or a delay) in the syncytiotrophoblast formation and a decrease of the production of pregnancy-specific hormones. We show that it is related to the over expression of the SOD-1 located on chromosome 21. These results will be of help for the understa

Multimatrix Biomonitoring and Transplacental Transfer of Legacy and Emerging PFAS in Korean Pregnant Women.

Maternal-fetal exposure to per- and polyfluoroalkyl substances (PFAS) cannot be adequately characterized using single biological matrices because compound-specific distribution patterns vary according to physicochemical properties and elimination pathways. We measured 33 legacy, precursor, and emerging PFAS in maternal serum, plasma, urine, and cord serum from Korean singleton pregnancies (n = 151) to characterize matrix-specific exposure profiles, transplacental transfer, and associations with maternal and neonatal factors. Matrix-specific detection patterns distinguished predominantly protei
