Acute Median Neuropathy Due to Persistent Median Artery Thrombosis Following Cupping Therapy: The Diagnostic Value of High-Resolution Ultrasound.

Persistent median artery (PMA) thrombosis is a rare cause of acute median neuropathy that may mimic typical carpal tunnel syndrome. We report the case of a 45-year-old man with several months of intermittent numbness without weakness affecting the radial three-and-a-half digits of the right hand who developed rapidly worsening pain, forearm erythema and swelling, distal coldness, and finger weakness following acupuncture and cupping therapy. High-resolution ultrasonography revealed a bifid median nerve and an enlarged PMA containing a long-segment hypoechoic thrombus. Spectral Doppler demonstr

Case report: Traumatic median neuropathy in the distal forearm after massage therapy.

Median nerve damage caused by trauma is rare, especially after the massage therapy. There have been no reports of median neuropathy in the distal forearm following massage therapy. A 61-year-old man developed paresthesia and numbness in the right hand after two sessions of massage therapy. Electrophysiologic studies, ultrasound, and magnetic resonance imaging were used to localize and confirm the median nerve lesion in the distal forearm. Ultrasound-guided perineural steroid injection and oral pregabalin were administered. At the 1-month follow-up, more than 80% of the sensory symptoms had res

Proximal Entrapment Neuropathy of the Median Nerve above the Elbow-Case Report.

We report an extremely rare case of proximal entrapment neuropathy of the median nerve above the elbow in a 44-year-old man who presented with paresthesia with median nerve distribution. Tinel's sign was located in the upper arm medial to the biceps and 5 cm proximal to the elbow. The patient underwent microsurgery under local anesthesia. The fascia covering the brachial- and the biceps brachii muscle entrapped median nerve. After operation, he reported symptom improvement. Lesions above the elbow should be considered as possible causative factors of entrapment neuropathy of the median nerve.

Median nerve neuropathy after flexor pollicis longus tendon reconstruction: A case report.

BACKGROUND: To treat flexor pollicis longus (FPL) muscle function loss, the 4th flexor digitorum superficialis (FDS) to the FPL tendon transfer is preferred as a reconstruction method. Various complications can occur during transfer. However, median nerve neuropathy has not been reported yet. We present a case of median nerve neuropathy caused by irritation of suture knots of the 4th FDS to the FPL tendon transfer with a review of the literature. CASE SUMMARY: A 52-year-old male patient presented with paresthesia along median nerve distribution of right hand after tendon transfer. He complain

Superficial brachial artery: a possible cause for idiopathic median nerve entrapment neuropathy.

Nerve entrapment syndromes occur because of anatomic constraints at specific locations in both upper and lower limbs. Anatomical locations prone to nerve entrapment syndromes include sites where a nerve courses through fibro-osseous or fibromuscular tunnels or penetrates a muscle. The median nerve (MN) can be entrapped by the ligament of Struthers; thickened biceps aponeurosis; between the superficial and deep heads of the pronator teres muscle and by a thickened proximal edge of flexor digitorum superficialis muscle. A few cases of MN neuropathies encountered are reported to be idiopathic. Th

Brachialis syndrome: a rare consequence of patient positioning causing postoperative median neuropathy.

BACKGROUND: Poor positioning of patients can result in devastating permanent neurologic deficits. We describe a previously unreported cause of median nerve compression that we have termed the brachialis syndrome, associated with patient positioning that results in permanent median nerve damage. METHODS: We identified this condition affecting 6 median nerves. All patients underwent surgical decompression of the proximal median nerve at the level of the antecubital fossa. RESULTS: Five patients presented with symptoms of median nerve compression; 6 affected median nerves manifested brachialis

Median neuropathy after multiple punctures of the forearm for catheterization: A case report.

BACKGROUND: Neuropathy may occur at some sites after catheterization for close examination of cardiac disease. Although the radial artery is considered a relatively uncom-plicated site for catheterization, the radial artery and median nerve are in relatively close proximity, with the risk of median nerve injury depending on the angle of puncture. The purpose of this study was to report the outcomes of surgery performed for conservative therapy-resistant median neuropathy following forearm catheterization. CASE SUMMARY: A 50-year-old woman experienced palsy from the right thumb to the radial s

Accuracy of in-office nerve conduction studies for median neuropathy: a meta-analysis.

PURPOSE: Carpal tunnel syndrome is the most common focal neuropathy. It is typically diagnosed clinically and confirmed by abnormal median nerve conduction across the wrist (median neuropathy [MN]). In-office nerve conduction testing devices facilitate performance of nerve conduction studies (NCS) and are used by hand surgeons in the evaluation of patients with upper extremity symptoms. The purpose of this meta-analysis was to determine the diagnostic accuracy of this testing method for MN in symptomatic patients. METHODS: We searched the MEDLINE database for prospective cohort studies that e

Peripheral neuropathies of the median, radial, and ulnar nerves: MR imaging features.

The median, radial, and ulnar nerves of the upper limbs may be affected by various peripheral neuropathies, each of which may be categorized according to its cause, as either an entrapment or a nonentrapment neuropathy. Entrapment neuropathies, also referred to as nerve compression syndromes, include the supracondylar process syndrome, pronator syndrome, anterior interosseous nerve syndrome, carpal tunnel syndrome, posterior interosseous nerve syndrome, cubital tunnel syndrome, and Guyon canal syndrome. Nonentrapment neuropathies include traumatic nerve injuries, infectious and inflammatory co

The incidence and decompression rates of median and ulnar neuropathies following shoulder surgery.

BACKGROUND: Upper extremity peripheral neuropathy is a known, but uncommon complication that can occur after shoulder surgery. The incidence rate is well documented, and most of these cases historically have been treated conservatively. However, we hypothesize peripheral compression neuropathy requires a much higher need for surgical decompression than originally reported. The purpose of this study was to evaluate the incidence, decompression rates, and prognostic factors for developing median and ulnar neuropathies following shoulder surgery. METHODS: A retrospective chart review was perform

Computer protocol for the electrodiagnostic evaluation of patients with suspected median neuropathy at the wrist.

Patients with median entrapment neuropathy at the wrist (MEW) present a large burden to electrodiagnostic (EDx) services. We report here our experience with a computer protocol for EDx standardized evaluation of these patients. The computer program guiding the examiners through the history, focused clinical examination and nerve conduction studies (NCSs), and automatically generating the report was employed in patients with suspected MEW referred to our unit in year 2010. Included were 1,935 patients (3,870 hands) with suspected MEW. Median motor study had to be performed in 100 %, a ring fing

Risk factors for carpal tunnel syndrome and median neuropathy in a working population.

OBJECTIVE: To assess whether work-related physical activities are associated with Carpal tunnel syndrome (CTS), even when controlling for personal risk factors. METHODS: A cross-sectional assessment of 1108 workers from eight employers and three unions completed nerve conduction testing, physical examination, and questionnaires. CTS was defined by median neuropathy and associated symptoms. RESULTS: Eighteen workers had CTS and 131 had evidence of median neuropathy. CTS was highest among construction workers (3.0%) compared to other subjects (<1%). Logistic regression models for median neurop

Neuropathy of motor branch of median or ulnar nerve induced by midpalm ganglion.

Two cases of neuropathy of a motor branch caused by a midpalmal ganglion are presented. In the first case the ganglion originated from the midcarpal joint, protruded into the thenar muscle, and compressed the motor branch of the median nerve. In the second case the ganglion, distal to the fibrous arch of the hypothenar muscles, originated from the third carpometacarpal joint and compressed the motor branch of the ulnar nerve. In both cases muscle weakness and finger deformity recovered well after resection of the ganglion. This clinical condition is rare compared with carpal tunnel syndrome an
