BACKGROUND: Many pathologies of the mediastinum can be diagnosed using standard radiographs. Correlation of radiographic findings with computed tomography (CT) is instructive for a better understanding and can help improve detection rates of mediastinal lesions.
OBJECTIVES: To identify the most common mediastinal lesions and to correlate their features in chest radiographs and CT.
METHODS: The International Thymic Malignancy Interest Group (ITMIG) classification in the anterior, middle, and posterior mediastinum is based on anatomic landmarks. Used as a tool to characterize mediastinal lesio

The last several years have witnessed an increase in reports regarding the pathology and classification of mediastinal neoplasms that heretofore have received relatively scant attention in comparison to tumors from other anatomic locations. Some of these, such as thymoma, remain controversial and contentious subjects. The sampling and diagnosis of mediastinal tumors using the technique of fine-needle aspiration biopsy has not gained acceptance among radiologists, surgeons, and pathologists to the degree it has for tumors of the thyroid or breast. Thus, the literature regarding aspiration cytop

The mediastinum contains vital vascular and nonvascular organs and other structures, and a wide variety of neoplasms and other abnormalities may originate from this anatomic region of the chest. Division of the mediastinum into distinct compartments helps narrow the differential diagnosis of mediastinal abnormalities detected on imaging studies, assists the planning of biopsy and surgical procedures, and facilitates communication between healthcare providers in the multidisciplinary setting. Numerous different models have been developed and used by radiologists, surgeons, and anatomists. Recen

Mediastinal seminomas are uncommon primary mediastinal neoplasms, and most are solid in appearance. Cystic mediastinal seminoma is an unusual type of extragonadal germ cell tumor that has rarely been reported in the literature. Here we describe a 36-year-old man with a 7.5-cm cystic mediastinal seminoma. The tumor was excised successfully by video-assisted thoracoscopic surgery. No recurrence was noted during 28 months of follow-up.

Mediastinal neoplasms are rare in the elderly, and clinical suspicion is the first and most important step of differential diagnosis. Mediastinal tumors can be misdiagnosed because their symptoms or signs can overlap with cardiovascular diseases, which have a higher prevalence among the older population. The diagnostic process should be managed with multimodality imaging and clinical judgement. Here, the case of a 74-year-old male patient, who presented with shortness of breath, is examined. A chest X-ray revealed an increased cardiothoracic ratio, and he was diagnosed with hemopericardium fol

Needle aspiration biopsy of hilar and mediastinal masses was attempted in 100 patients and completed in 91. There were no major complications. In the group with completed biopsies, there were 83 patients with malignant neoplasms; a positive diagnosis of malignancy was established in 80 (96%). Two of the three false-negative results occurred in patients with Hodgkin disease. In all 8 patients with benign masses, the biopsy results were correct. Five of the nine incomplete biopsies occurred in patients with aortic aneurysms that simulated lung or mediastinal masses. In the other 4 patients, the

Teratoma is the second most common mediastinal neoplasm, but malignant transformation in mature teratomas is uncommon at both gonadal and extragonadal sites. A 35-year-old woman presented with an anterior mediastinal mass. Computed tomography suggested a teratoma. Meticulous pathological examination helped in detecting a neuroendocrine neoplasm (carcinoid) within the mature teratoma. Malignant transformation in a teratoma is difficult to diagnose on clinical and radiological assessment; histopathology is essential to detect the malignant nature of these neoplasms.

Dr. Juan Rosai is one of the most prolific contributors to the literature on mediastinal pathology, and he has added steadily to that body of work over a 50-year period. Rosai has written several landmark articles in this topical area, including articles on thymic epithelial lesions, mediastinal neuroendocrine tumors, mediastinal lymphoma and other hematopoietic lesions, thymolipoma, thymoliposarcoma, mediastinal solitary fibrous tumor, intrathymic langerhans-cell histiocytosis, mediastinal germ cell neoplasms, and multilocular thymic cyst. This review recounts his role as one of the principal

Primary mediastinal tumors, with the exception of lymphoma, are generally rare neoplasms. The diagnosis, classification, and treatment of these tumors still cause some degree of difficulty due to their low incidence and morphologic heterogeneity. This is particularly true for thymic epithelial neoplasms, that is, thymoma and thymic carcinoma, and also applies to primary neuroendocrine carcinomas of the thymus and primary mediastinal germ cell tumors. The appropriate staging of these tumors, likewise, has been a matter of debate over the years and numerous proposals for the staging of thymic ep

Mediastinal paracardiac tumors may cause both cardiovascular complications and problems in differential diagnosis of cardiac diseases. Transesophageal echocardiography (TEE) may give an additional new window to mediastinal neoplasms, but only a few studies have been reported. TEE was performed in 70 patients with paracardiac neoplastic masses. The procedure was indicated to solve particular clinical problems in 20 patients, and as a prospective study on 50 unselected patients with mediastinal neoplasms. Twenty-three patients underwent follow-up studies; a total of 101 echocardiograms were reco

The mediastinum contains vital vascular and nonvascular structures and organs, and a wide variety of abnormalities may arise from this region of the thorax. Although mediastinal masses may be initially detected on chest radiography, cross-sectional imaging plays an important role in the identification and evaluation of mediastinal lesions, enabling the formulation of focused differential diagnoses and ultimately guiding management. Computed tomography (CT) is considered the imaging modality of choice for evaluating most mediastinal masses; however, the role of magnetic resonance (MR) imaging c

In 20 of 62 patients (32%) with presumably operable cancer of the left lung, metastatic cancer in the anterior mediastinal lymph node was detected by left anterior mediastinoscopy without division or spreading of costal cartilage. Fourteen of 41 patients (33%) with left upper lobe and four of 19 (21%) with left lower lobe cancer had positive anterior mediastinal node biopsies. In a comparable group having cervical mediastinoscopy for cancer of the right lung, 39 of 103 (38%) had positive paratracheal or subcarinal lymph nodes. In both mediastinoscopy groups six of 45 squamous carcinomas (13%),

Though thymomas are the commonest neoplasms of the anterior mediastinum, metaplastic thymoma is an extremely rare subtype, with only a few reported cases. It was included in the 2004 WHO classification of thymic epithelial tumors and exhibits a biphasic morphology distinct from the conventional thymomas. It has a benign clinical course with extremely rare cases showing recurrence and malignant transformation. However, metaplastic thymoma needs to be histologically distinguished from other more aggressive mediastinal neoplasms displaying biphasic patterns. Here we report a case of an incidental

Mediastinal fine needle aspirations are routinely encountered in cytopathology practice. Mediastinal lesions may pose diagnostic challenges owing to their rarity and locations associated with the complexity of the mediastinal anatomic structures in the thoracic cavity. Diagnosing mediastinal lesions and guiding patient management usually require correlating with clinical and radiologic findings, being familiar with cytomorphologic features and appropriately triaging the diagnostic material for ancillary testing. This review proposes a practical approach to interpret mediastinal fine needle asp

Acute mediastinal hemorrhage is rarely caused by a rupture of mediastinal neoplasms. We herein report a 70-year-old man who presented with mediastinal parathyroid adenoma manifested by the sudden onset of mediastinal bleeding. Preoperatively, he showed no symptoms or complications associated with hypercalcemia. No particular findings were found in the thorax except for a small nodule in the upper mediastinum. The nodule was resected through a collar incision, and pathohistology showed a parathyroid adenoma, with an intracapsular hemorrhage.

Intrathoracic neoplasms other than lung cancer represent a diverse group of diseases. This review focuses on the recent literature addressing mediastinal neoplasms, unusual primary lung neoplasms, and pulmonary metastases from other locations. Advances in classification, imaging, diagnosis, surgical issues, and treatment issues shown in case series and individual case reports are presented and discussed.

UNLABELLED: Transesophageal sonography was originally used to assess esophageal-gastric neoplasm. The technique may also be useful in the evaluation of pulmonary neoplasms for possible mediastinal involvement, with regard to both T and N factors.
OBJECTIVE: To consider the validity of this minimally-invasive technique for assessing possible mediastinal involvement by direct tumoral invasion (T4) and to help obtain the most accurate staging of lung cancer.
PATIENTS AND METHODS: Sixteen patients with confirmed histopathological diagnoses of pulmonary neoplasm were examined by transesophageal s

Mesenchymal neoplasms of the thymus and mediastinum account for only 2 % of neoplasms of the mediastinum and are therefore very rare. With very few exceptions the histology, immunohistochemistry and (based on current knowledge) molecular biology of mediastinal soft tissue tumors are not different from their counterparts in other organs. Characteristic features are more concerned with clinical epidemiological and therapeutic aspects as well as the multitude of possible differential diagnoses. With the exception of organ-specific tumors, such as gastrointestinal stromal tumors (GIST), virtually

Hamartomas are benign neoplasms most commonly arising in the lung, typically identified as incidental solitary pulmonary nodules. Occurrence in the mediastinum is exceedingly rare. Because anterior mediastinal lesions are most often malignant, hamartomas in this location pose a diagnostic challenge. We present the case of a 53-year-old female with a background of endometrial carcinoma who was evaluated for chest pain and palpitations. A chest radiograph revealed a large opacity in the left upper hemithorax, corresponding to a well-circumscribed anterior mediastinal mass on computed tomography.

BACKGROUND: Mediastinal neurilemmoma is an infrequent and a big size neoplasm in the posterior mediastinum. There are usually no symptoms. When there is a pressure against nerves or vascular structures, it can be dangerous for the life. The main problem of this tumors is the diagnosis: surgical approach and the complete resection without injuring vital mediastinal structures.
CLINICAL CASE: We report a case in a 29-year-old male with a giant mediastinal neurilemmoma. He had chest pain for two months. Chest rays showed opacity in the left chest. A broncofiberscopy, computed tomography and tran
