Diagnosis and Therapy of Community-Acquired Pneumonia in the Emergency Department: A Retrospective Observational Study and Medical Audit.

BACKGROUND: Despite advances in therapy, community-acquired pneumonia (CAP) is still associated with significant morbidity and mortality. Several studies conducted in different countries have reported suboptimal adherence to the guidelines. However, there are currently no available data on adherence to CAP guidelines specifically in Switzerland. OBJECTIVES: The aim of this study was to audit the quality of diagnosis and therapy of CAP at a Swiss general hospital. METHODS: A retrospective, observational, single-center cohort study was conducted, including patients older than 18 years diagnose

A natural history of temporary tube feeding care at a children's hospital: A prospective audit of medical records.

This chart audit characterized the natural history of temporary tube feeding use at a children's hospital between 1 November 2018 and 30 November 2019. Data were collected from the decision-making phase until tube removal or 4 months post-tube insertion. Children's tube feeding journeys were described, and associations determined between patient and service-related variables and outcomes of tube feeding duration and being discharged with a feeding tube. Four hundred and 94 patients were followed with a median age of 1.0 years (IQR 3.6). Many had respiratory illnesses (n = 213, 43%) and receive

Improvement in the management of gout is vital and overdue: an audit from a UK primary care medical practice.

BACKGROUND: Gout is estimated to affect 1.4% of adults in the UK. Appropriate and timely management is essential to reduce the risk of further flares, complications, and to reduce cardiovascular disease risk. The British Society for Rheumatology and British Health Professionals in Rheumatology (BSR/BHPR) and the European League Against Rheumatism (EULAR) have published guidance regarding the management of gout, thereby providing standards against which performance can be measured. This audit was designed to assess the extent to which patients diagnosed with gout in one primary care medical pra

A Retrospective Audit of Healthcare-Associated Infections in Patients Who Are Optimised for Discharge at a Tertiary Medical Centre.

Background This audit aimed to identify patients who developed a healthcare-associated infection (HCAI) after being deemed medically optimised for discharge and to then recognise the common patterns of HCAIs alongside the reasons for delays in discharges. Methodology This audit used a retrospective observational design. The audit was conducted in a general medical ward in an NHS England tertiary medical centre between December 2023 and January 2024. The study had a sample size of 93 patients who had been deemed medically optimised for discharge (MOFD) in a general medical ward but had not yet

Reducing unwarranted variation in medication reviews following a fall: a closed-loop audit in primary care.

BACKGROUND: Falls in adults ≥65 are common, costly and preventable. Around one-third fall each year and 5-10% sustain serious injury; hip fractures carry 20-30% one-year mortality and falls cost the NHS ~£2.3bn annually. Polypharmacy and high-risk drugs (e.g. benzodiazepines, anticholinergics, antihypertensives) increase risk. NICE CG161/NG5 and NHS England guidance recommend timely, structured post-fall medication reviews, yet local data suggested unwarranted variation in documentation and follow-up. AIM: To increase guideline-concordant documentation of medication reviews within 4 weeks aft

Transition of pediatric patients with bronchiectasis to adult medical care in the Northern Territory: A retrospective chart audit.

BACKGROUND: Bronchiectasis is increasingly being recognized to exist in all settings with a high burden of disease seen in First Nations populations. With increasing numbers of pediatric patients with chronic illnesses surviving into adulthood, there is more awareness on examining the transition from pediatric to adult medical care services. We undertook a retrospective medical chart audit to describe what processes, timeframes, and supports were in place for the transition of young people (≥14 years) with bronchiectasis from pediatric to adult services in the Northern Territory (NT), Australi

Reducing inappropriate medical practices: a scenario-based experiment on fiduciary duty and audit enforcement in China's healthcare sector.

BACKGROUND: China's recent anti-corruption efforts in the healthcare sector have focused on senior hospital administrators and on procurement processes for pharmaceuticals and medical supplies, with comparatively little attention paid to frontline clinical physicians. Yet the everyday practices of frontline physicians-including excessive testing, overtreatment, and overprescription-constitute a major source of cost inflation and have contributed to a substantial erosion of patient trust. Drawing on principal-agent theory and on responsive regulation theory, this study examines whether introduc

Perioperative anaesthetic management of high-order repeat caesarean section: audit of practice in a university-affiliated medical centre.

BACKGROUND: High-order (five or more) repeat caesarean sections (HORCS) are associated with increased rates of placenta praevia, placenta accreta and peripartum hysterectomy and prolonged surgical time secondary to intra-abdominal adhesions. This study summarizes our experience in the anaesthetic management of HORCS. METHODS: The files of all parturients undergoing HORCS between January 1995 and August 2007 were reviewed to determine surgical times, rates and causes of conversion from neuraxial to general anaesthesia and the need to supplement neuraxial anaesthesia with intravenous sedation.

Prevalence and Severity of Guillain-Barré Syndrome in Two Regional Areas of Australia: A Retrospective Audit of Medical Records.

INTRODUCTION: Guillain-Barré syndrome (GBS) is an autoimmune disorder affecting the peripheral nervous system, with an annual worldwide incidence of 1-2 cases per 100,000 people. The incidence of GBS in Australia has not been studied in the past 38 years, and existing Australian literature presents a different severity profile compared to that observed in the Barwon South West Region (BSWR). The aim of this study was to describe and compare the prevalence of GBS in two proximate regional areas of Australia. METHODS: A retrospective file audit was conducted for patients residing in the BSWR or

Using electronic clinical practice audits as needs assessment to produce effective continuing medical education programming.

BACKGROUND: The traditional needs assessment used in developing continuing medical education programs typically relies on surveying physicians and tends to only capture perceived learning needs. Instead, using tools available in electronic medical record systems to perform a clinical audit on a physician's practice highlights physician-specific practice patterns. AIM: The purpose of this study was to test the feasibility of implementing an electronic clinical audit needs assessment process for family physicians in Canada. METHOD: A clinical audit of 10 preventative care interventions and 10

Early detection of relapse after treatment for metastatic germ cell tumour of the testis: an exercise in medical audit.

The relapse patterns of 29 patients who recurred following treatment for metastatic germ cell tumours of the testis (seminoma n = 7, non-seminomatous germ cell tumour n = 22) have been analysed and the relative effectiveness of clinical follow-up and routine investigations in detecting relapse at an early stage have been examined. The analysis shows that routine estimation of the serum tumour markers human chorionic gonadotrophin and alpha-foetoprotein (HCG and AFP) is the single most important follow-up procedure. This is so, even in patients who were previously marker negative; it was the fi

Barrier to Entry: How Health Canada's Medical Device Single Audit Program Limits Patient Access to New Technology in Canada.

In this article, we look at the approval process for medical devices in Canada, and how the Medical Device Single Audit Program (MDSAP) has made it unfeasible for small manufactures to enter the Canadian market. Specifically, we explore how MDSAP has been a barrier in Canada for the entry of hypoglossal nerve stimulators (HGNS), a proven technology with a strong track record in other markets for the treatment of obstructive sleep apnea. This is ultimately preventing Canadian patients access to life-saving medical devices. We hope this commentary will shed light on this barrier and prompt Healt
