Enhancing participation in cardiac rehabilitation: Focus on underserved populations.

Participation in cardiac rehabilitation (CR) significantly decreases morbidity and mortality and improves quality of life following a wide variety of cardiac diagnoses and interventions. However, participation rates and adherence with CR are still suboptimal and certain populations, such as women, minorities, and those of lower socio-economic status, are particularly unlikely to engage in and complete CR. In this paper we review the current status of CR participation rates and interventions that have been used successfully to improve CR participation. In addition, we review populations known t

Preventing corneal blindness caused by keratitis using artificial intelligence.

Keratitis is the main cause of corneal blindness worldwide. Most vision loss caused by keratitis can be avoidable via early detection and treatment. The diagnosis of keratitis often requires skilled ophthalmologists. However, the world is short of ophthalmologists, especially in resource-limited settings, making the early diagnosis of keratitis challenging. Here, we develop a deep learning system for the automated classification of keratitis, other cornea abnormalities, and normal cornea based on 6,567 slit-lamp images. Our system exhibits remarkable performance in cornea images captured by th

Impact of clinical trial virtualization on recruitment in underserved communities for a type 2 diabetes mHealth intervention.

OBJECTIVE: Approaches to randomized clinical trial (RCT) implementation changed during the COVID-19 pandemic as clinical trials transitioned to largely virtual implementation which may continue in the future. Without careful consideration, virtual RCTs may exacerbate the historical under-representation of women, the elderly, and racial and ethnic minorities that has occurred in the past. The following study presents an approach to virtualizing an RCT for an mHealth intervention. MATERIALS AND METHODS: Study participants were recruited from Federally Qualified Health Centers in medically under

Free hernia surgery for the underserved is possible in the United States.

INTRODUCTION: Inguinal hernia is one of the most common ailments known to mankind. When symptomatic it can severely affect the patient's quality of life. Nevertheless, the vast majority of inguinal herniorrhaphies are elective and, therefore, not available to uninsured patients who do not have the financial wherewithal to pay for the operation. Using the Surgery on Sunday model developed in Kentucky, hernia repair for the underserved developed a free clinic for hernia surgery, based on institutional commitment to the poor as well as the volunteer efforts of medical students and hospital person

Comparison of small-area deprivation measures as predictors of chronic disease burden in a low-income population.

BACKGROUND: Measures of small-area deprivation may be valuable in geographically targeting limited resources to prevent, diagnose, and effectively manage chronic conditions in vulnerable populations. We developed a census-based small-area socioeconomic deprivation index specifically to predict chronic disease burden among publically insured Medicaid recipients in South Carolina, a relatively poor state in the southern United States. We compared the predictive ability of the new index with that of four other small-area deprivation indicators. METHODS: To derive the ZIP Code Tabulation Area-Lev

Innovative Pathways to Oral Health Workforce Resilience: Introducing the CHC-MIF for Mentorship-Driven Strategies in Underserved Settings.

OBJECTIVES: To reflect on personal experiences in community health centers (CHCs) and federally qualified health centers to highlight mentorship as an underutilized tool for addressing oral health workforce shortages, recruitment, and retention. METHODS: Narrative reflection based on the author's lived experiences as a dentist in CHCs since graduation and as an adjunct faculty mentor, synthesized with literature from 2020 to 2025. RESULTS: Financial incentives dominate CHC strategies, but mentorship fosters confidence and retention for new graduates. The proposed CHC Mentorship Integration F

How can engagement with underserved communities be enhanced? A co-inquiry informed model of stop smoking outreach.

AIMS: This co-inquiry project aimed to develop a qualitatively informed model of professionally led stop smoking outreach. It involved 13 staff from a Stop Smoking Service (SSS) which operates across three Local Authorities in England (Central Bedfordshire, Bedford Borough and Milton Keynes). Staff's outreach sought to engage people from the most deprived areas who smoked but were not engaging with the service. METHODS: The co-inquiry comprised six reflection sessions and ethnographic research which aimed to explicate and examine staff's assumptions about how outreach works, conducted over 12

An assessment of the telehealth needs and health-care priorities of Tanna Island: a remote, under-served and vulnerable population.

We surveyed eight Canadian physicians who had each provided medical care for six months on the remote and under-served island of Tanna in Vanuatu. The most frequently encountered medical problems on Tanna were infectious diseases (tuberculosis, hepatitis, abscesses, malaria, pneumonia, typhoid fever, meningitis and skin infections). When physicians were asked about the top three health-care priorities, they ranked tuberculosis control, clean water and improved health-care delivery/communication between hospital and outposts as most important. The key issues were: (1) basic public health needs

Increasing Care for Underserved Communities Through a Global Health Residency Training Program.

Background: Global health education is important for addressing health inequities nationally and internationally. Physician shortages in underserved areas suggest more exposure during training is needed. Objective: To study the impact of a global health training program on residents' perceived preparedness and intention to care for underserved populations. Methods: Observational mixed method evaluation of the impact of an educational intervention, the Global Health Scholars Program (GHSP), on perceived knowledge and intention to practice in underserved settings. The intervention consisted of a

The Association between Mental Health-Related 911 Calls and the Mental Health Professional Shortage in New York City.

This study examines the relationship between designated Mental Health Professional Shortage Areas (MH HPSAs) and mental health-related 911 calls in New York City. Negative binomial regression models were used to estimate the relationship between MH HPSAs and MH 911 calls after adjusting for the population size and other neighborhood characteristics. The study found that neighborhoods designated as MH HPSAs had higher MH 911 calls compared to non-shortage areas, with a 27% increase in expected MH 911 calls after adjustment. Moreover, the results indicated that neighborhoods with higher rates of

The quality of screening colonoscopy in rural and underserved areas.

BACKGROUND: Screening colonoscopy effectiveness depends on procedure quality; however, knowledge about colonoscopy quality in rural and underserved areas is limited. This study aimed to describe the characteristics and quality of colonoscopy and to examine predictors of colonoscopy quality at rural and underserved hospitals. METHODS: Adults undergoing colonoscopy from April 2017 to March 2019 at rural or underserved hospitals across the Illinois Surgical Quality Improvement Collaborative were prospectively identified. The primary outcome was colorectal adenoma detection, and secondary outcome

Georgia Leverages Telehealth to Expand HIV Care Management in Underserved Areas.

INTRODUCTION: The Southeast U.S. and Georgia in particular are disproportionally affected by HIV. A major barrier in Georgia to meeting the goals of Ending the HIV Epidemic in the U.S is lack of proximity to HIV care. To address this and other barriers, Georgia implemented a comprehensive HIV telehealth system, including telemedicine to provide remote care through clients' local medical homes. In addition, a telementoring program (Project Extension for Community Healthcare Outcomes) was initiated to strengthen the HIV provider workforce in Georgia. METHODS: A total of 60 telemedicine solution

Brief behavioral interventions at free medical fairs.

Free medical fairs have emerged to compensate for the lack of access to affordable health care in rural areas of the United States. Mental health services are offered less frequently than other medical services, despite a documented need, perhaps due to a belief that mental health interventions could not be effective in a single session. We examined the types of problems presented at three rural medical fairs, and whether single session mental health interventions affected participants' health confidence, distress, or progress toward health-related goals. Problems presented included mental hea

Social accountability in a medical school: is it sufficient? A regional medical school curriculum and approaches to equip graduates for rural and remote medical services.

BACKGROUND: Social accountability is increasingly integral to medical education, aligning health systems with community needs. Universitas Pattimura's Faculty of Medicine (FMUP) enhances this through a curriculum that prepares graduates for rural and remote (RR) medical practice, exceeding national standards. The impact of this curriculum on graduate readiness in actual work settings remains unassessed. OBJECTIVE: This study was conducted to capture the perspectives of FMUP medical graduates in a rural-centric curriculum, focusing on the teaching and learning opportunities afforded to them du

The role of a student-run clinic in providing primary care for Calgary's homeless populations: a qualitative study.

BACKGROUND: Despite the increasing popularity of Student-Run Clinics (SRCs) in Canada, there is little existing literature exploring their role within the Canadian healthcare system. Generalizing American literature to Canadian SRCs is inappropriate, given significant differences in healthcare delivery between the two countries. Medical students at the University of Calgary started a SRC serving Calgary's homeless population at the Calgary Drop-In and Rehabilitation Centre (CDIRC). This study explored stakeholders' desired role for a SRC within Calgary's primary healthcare system and potential

Building Community With Community: Collaborative Reflections on the Rural and Urban Community Orienting Experience (RUCOE).

Short-term intensive orientation programs have existed at medical schools for decades, yet there is very little published on these programs and the impact they have on medical students, particularly those at the very beginning of their medical training. The annual Rural/Urban Community Orienting Experience (RUCOE) at the Ohio University Heritage College of Osteopathic Medicine is a three-day immersive orientation before the start of the academic year with the goals to build community among students interested in underserved practice, faculty, and staff; foster a curious mind as a practitioner-

Teleophthalmology Programs in Underserved Communities in the United States: A Narrative Review.

Underserved communities in the United States continue to face substantial barriers to accessing eye care, leading to preventable vision loss. This review synthesizes United States-based teleophthalmology programs designed to expand eye care access among under-resourced populations, including older adults, Indigenous communities, children, veterans, people experiencing homelessness, and patients in safety-net systems. Most programs used asynchronous nonmydriatic fundus photography captured by trained technicians in community or primary care settings, with remote specialist interpretation and fr
