A stepped wedge randomised controlled trial assessing the efficacy and patient acceptability of virtual clinical pharmacy in rural and remote Australian hospitals.

BACKGROUND: Despite medication being the most common healthcare intervention and medication-related incidents being common in hospitals, many rural and remote hospitals in Australia lack onsite pharmacy services due to resource constraints. A Virtual Clinical Pharmacy Service (VCPS) staffed by two senior, rural generalist hospital pharmacists assigned to four hospitals each was implemented in rural and remote facilities to determine whether the VCPS increased adherence to National Safety and Quality Health Service Standards (NSQHS). METHODS: A stepped-wedge randomised controlled trial was emp

Medication Management Strategies at Hospital Admission: A Systematic Review and Meta-Analysis.

Hospital admission is high risk for medication harm due to transfer errors, delays, and charting mistakes. Up to 70% of admission medication lists contain errors, 59% potentially harmful, highlighting the need for safety strategies. The aim of the study is to systematically review the effectiveness of strategies for medication management at hospital admission. Following PRISMA guideline, three databases were searched for RCTs. Subgroup analyses assessed outcomes including mortality, readmissions/Emergency Department (ED) visits, length of stay (LOS), medication errors, and adverse drug events

Atropine-Induced Anticholinergic Syndrome Following Off-Label Sublingual Administration for Refractory Sialorrhea: A Case Report.

A sublingual atropine ophthalmic solution is increasingly used as an off-label therapy for the symptomatic management of refractory sialorrhea because of its rapid onset of action, ease of administration, and generally favourable safety profile when used at recommended doses. Although generally well-tolerated at therapeutic doses, systemic anticholinergic toxicity may occur following medication administration errors or unintentional overdose. We report the case of a 76-year-old man with recurrent head and neck squamous cell carcinoma receiving palliative systemic therapy who presented with acu

Medication discrepancies in the dental record and impact of pharmacist-led intervention.

BACKGROUND: Patients frequently use medications with potential implications for oral health and dental procedures, yet little is known about the accuracy of medication lists available to dentists. The aims of this study were to describe the frequency and clinical implications of medication discrepancies in the dental record (phase 1) and to evaluate the impact of pharmacist intervention on medication reconciliation processes in dental practice (phase 2). METHODS: A prospective, single-centre study evaluating adults receiving dental care was conducted. Discrepancies between the dental record a

Update on the diagnosis and treatment of neuromyelitis optica: recommendations of the Neuromyelitis Optica Study Group (NEMOS).

Neuromyelitis optica (NMO, Devic's syndrome), long considered a clinical variant of multiple sclerosis, is now regarded as a distinct disease entity. Major progress has been made in the diagnosis and treatment of NMO since aquaporin-4 antibodies (AQP4-Ab; also termed NMO-IgG) were first described in 2004. In this review, the Neuromyelitis Optica Study Group (NEMOS) summarizes recently obtained knowledge on NMO and highlights new developments in its diagnosis and treatment, based on current guidelines, the published literature and expert discussion at regular NEMOS meetings. Testing of AQP4-Ab

Medication Errors and Blood Pressure Control Among Patients Managed for Hypertension in Public Ambulatory Care Clinics in Botswana.

Background The prevalence of hypertension in low- and middle-income countries is rapidly increasing, with most cases undiagnosed and many poorly controlled among those diagnosed. Medication reconciliation studies from high-income countries have demonstrated a high occurrence of antihypertensive medication errors and a strong association between medication errors and inadequate blood pressure control, but data from low- and middle-income countries are lacking. Methods and Results We conducted a cross-sectional study from April to October 2018 of adult patients on pharmacologic management for kn

Management of Polypharmacy in Dialysis Patients.

Most patients receiving dialysis have other common chronic conditions in addition to end-stage renal disease, including hypertension, diabetes, cardiovascular disease, and mineral and bone disorder, all of which require long-term medication management. Dialysis patients take an average of 10-12 prescribed and over-the-counter medications from an average of 4.7 prescribers, and an average of 19 pills per day. Thus, reducing polypharmacy is not adequate as a medication therapy goal for these patients. Instead, the dialysis community should focus on ensuring that all patients receive medications

Rationale and design of the IMProving Adherence to medications for breast Cancer and cardiovascular disease equiTably (IMPACT) randomized clinical trial.

BACKGROUND: Cardiovascular disease (CVD) is a leading cause of mortality in patients with early-stage breast cancer (BC), many of whom are prescribed endocrine therapy (ET) to prevent BC recurrence. Yet, nonadherence to ET and CVD medications is common among BC survivors, particularly in underserved populations at high risk for adverse outcomes. Little is known about how to equitably improve medication adherence for multiple chronic conditions. METHODS: The IMProving Adherence to Medications for Breast Cancer and Cardiovascular Disease EquiTably (IMPACT) trial is an investigator-initiated, si

Formulary Reconciliation: Implementation of a Comprehensive Approach to Formulary Maintenance and Standardization.

At our institution UC San Diego Health, formulary qualifiers such as indication expansions and restrictions based on provider specialty, patient location, or patient characteristics are input as free text into an online formulary platform. Inconsistency in formulary categories and their descriptions since the implementation of the electronic system have led to confusion and inconsistent formulary application amongst staff. We reviewed 880 unique medications with formulary qualifiers to standardize both categories and language. There were 537 items with inpatient restrictions (eg, restricted to

Renal Drug Dosing and Medication Safety in Patients Receiving Dialysis: A Practical Guide for Nephrology Nurses and Advanced Practice Providers.

Patients receiving dialysis are among the most medically complex populations in health care and are at high risk for medication-related problems. Polypharmacy, frequent hospitalizations, multiple comorbid conditions, and care from numerous health care providers contribute to medication discrepancies and adverse drug events. In addition, advanced kidney disease significantly alters drug absorption, distribution, metabolism, and elimination, requiring careful application of renal drug dosing principles. Nephrology nurses and advanced practice providers play critical roles in medication reconcili

Antipsychotic-Associated Hypothermia With Bradycardia and QT Prolongation in an Older Adult.

Second-generation antipsychotics may cause rare but potentially life-threatening disturbances in thermoregulation and cardiac conduction. We report a 92-year-old man with severe frailty, cognitive impairment, pre-existing sinus bradycardia, and right bundle branch block who presented on three occasions with recurrent hypothermia, progressive bradycardia, and altered mental status. The initial episode was attributed to an Enterobacter cloacae complex urinary tract infection. However, symptoms recurred despite negative repeat urine cultures, culminating in profound hypothermia, hypotension, a he
