The biphasic ultrastructure of the meniscus and of articular cartilage provides their function in the complex biomechanics of the knee joint including load distribution, shock absorption, viscoelasticity, a smooth low friction gliding surface and resilience to compression. Meniscectomy may lead to destruction of cartilage and to osteoarthritis of the knee joint. Osteoarthritic changes after meniscectomy have been reported in up to 89% of patients. Retrospective analysis after open or arthroscopically assisted meniscectomy revealed restriction in sports to be between 2 and 50% and cessation of

Two hundred and forty arthroscopic meniscectomies have been performed between 1980 and 1982. One hundred and ninety-eight cases have been reviewed with a follow up of between 3 and 6 months and 101 cases were reviewed with a follow up of between 6 months and 2 years. On eight occasions the arthroscopy had to be completed by an arthrotomy and one of these became septic. No sepsis was observed in the absence of arthrotomy. Three cases of phlebitis and one haemarthrosis were seen. Social and functional rehabilitation was very rapid. The stay in hospital was 2 days and resumption of sport and work

BACKGROUND: Meniscal tear in older adults often accompanies knee osteoarthritis and is commonly treated with arthroscopic partial meniscectomy (APM) when patients have persistent pain after a trial of physical therapy. Cross-sectional evidence suggests that synovitis is associated with baseline pain in this patient population, but little is known about the relationship between synovitis and postoperative recovery or progression of knee osteoarthritis.
PURPOSE/HYPOTHESIS: Intra-articular extended-release triamcinolone may reduce inflammation and thereby improve outcomes and slow disease progre

BACKGROUND: Patients following meniscectomy are at increased risk of developing knee osteoarthritis in the tibiofemoral compartment and at the patellofemoral joint. As osteoarthritis is widely considered a mechanical disease, it is important to understand the potential effect of arthroscopic partial meniscectomy (APM) on knee joint mechanics. The purpose of this study was to evaluate changes in knee joint biomechanics during a forward lunge in patients with a suspected degenerative meniscal tear from before to three months after APM.
METHODS: Twenty-two patients (35-55 years old) with a suspe

We continued a prospective longitudinal follow-up study of 53 remaining patients who underwent open total meniscectomy as adolescents and who at that time had no other intra-articular pathology of the knee. Their clinical, radiological and patient-reported outcomes are described at a mean follow-up of 40 years (33 to 50). The cohort of patients who had undergone radiological evaluation previously after 30 years were invited for clinical examination, radiological evaluation and review using two patient-reported outcome measures. A total of seven patients (13.2%) had already undergone total knee

OBJECTIVE: The purpose of this report is to describe two cases of osteonecrosis that occurred after arthroscopic meniscectomy with a contact neodymium:yttrium aluminum garnet laser system. The patients developed increasing knee pain and disability 5 months and 6 months after laser meniscectomy. MR imaging showed subchondral osteonecrosis in the femoral condyle and tibial plateau immediately adjacent to the site of laser meniscectomy in both patients. One patient required a total knee replacement to alleviate knee symptoms, and the other patient required a tibial osteotomy and surgical elevatio

One hundred ACL insufficient knees were followed for an average of 3.5 years after arthroscopic partial meniscectomy had been performed. Fifty-two percent of the patients obtained a satisfactory result and 41% were able to participate in strenuous sports activities. The arthroscopic partial meniscectomy reduced pain and swelling in a high proportion of cases, but stability was only marginally improved. Negative prognostic factors were: sex (female), generalized laxity, contralateral recurvatum of more than 10 degrees, complete as opposed to partial ACL lesions, Grade III tibiofemoral or patell

Osteophytes are considered characteristic manifestations of osteoarthritis (OA) but may be seen independent of structural cartilage erosive lesions. Etiopathogenic mechanisms for osteophyte formation may involve responses to synovial inflammation, stretching of the synovial membrane, general mechanical instability, and vascular metaplasia. Experimentally induced OA following partial menisectomy in rabbits demonstrates active cellular replication at the normal joint periphery. Vascular responses are seen late in parallel with osteophyte formation rather than preceding it. Osteophyte ossificatio

The authors have reviewed 44 patients retrospectively who failed arthroscopic partial meniscectomy. The study attempts to define the chances for success and to identify prognostic factors as these patients return for reoperation. Each patient had a repeat arthroscopic examination from 2 to 60 months after partial meniscectomy (average 19 months). They were reviewed an average of 31 months after reoperation (6 to 60 months), and each completed a subjective questionnaire evaluating the efficacy of their repeat arthroscopic surgery. Seventy-one percent of the patients had improvement with reopera

PURPOSE OF REVIEW: This manuscript comprehensively reviews the most recent studies on preoperative and intraoperative factors that positively or negatively influence the clinical and radiological outcomes of arthroscopic partial meniscectomy (APM). RECENT FINDINGS: Initial research focused on baseline demographic (e.g., age, sex, BMI) and meniscal tear characteristics to predict short- and medium-term outcomes. Recent investigations, however, have broadened this scope to include previously overlooked ancillary elements, such as socio-economic and psychological factors. Furthermore, non-menisca

OBJECTIVE: The aim of this study was to evaluate the influence of partial medial meniscectomy on tibiofemoral joint space width (JSW).
DESIGN: Thirty-seven patients (mean age: 51 years), suffering from medial meniscal lesions, (post-traumatic (n=22) or degenerative (n=15)), and requiring arthroscopic partial meniscectomy, were enrolled in a prospective, controlled study. Bilateral weight-bearing radiographs in extended and flexed position were performed in the same Radiology Unit just before and 2 days after meniscectomy. During arthroscopy, the extent of meniscectomy was evaluated in percent

Twenty-four children who underwent open total meniscectomy were reviewed and followed-up for an average of 16.1 years (range 6-33 years). There were 7 boys and 17 girls aged between 7 and 16 years (average 12.5 years). Excellent and good results were noted in 62.5% of the patients. Eighty-seven of the children showed radiographic degeneration of the knee joint. This study in a Chinese population indicates that meniscectomy is not a benign procedure in children, and total meniscectomy should be avoided as far as possible.

In order to compare partial with total meniscectomy a prospective clinical study of 200 patients was carried out. At arthrotomy 100 patients were allocated to each type of operation. The two groups did not differ in duration of symptoms, age distribution, or sex ratio. The operations were performed as conventional arthrotomies. One hundred and ninety two of the patients were seen at follow up 2 and 12 months after operation. There was no difference in the period off work between the two groups. One year after operation, 6 of the 98 patients treated with partial meniscectomy had undergone furth

In a clinical follow-up study the postoperative results after arthroscopic meniscectomies are compared to conventional arthrotomies. 200 selected patients, half treated arthroscopically and half by open meniscectomy are reviewed after a postoperative period of 1.5-4 years. Each patient in the arthrotomy-group is matched with a patient in the endoscopic collective by age, sex and type of meniscus lesion. Ligamentous instability was an excluding criteria. The mean hospital stay in the arthroscopic group is 4.9 days compared to 12.3 days in the arthrotomy-group. At follow-up 46% of the arthroscop

Results of 150 arthroscopic partial medial meniscectomies were analyzed by computer to identify the factors that lead to an unsatisfactory (fair or poor) outcome. The average follow-up for the group was 36 months (range 24-60 months). One-hundred ten men and 40 women were involved, with an average age of 48 years. The overall results were 58% excellent-good, 28% fair, and 14% poor. Most tears involved the posterior horn (76%). Bucket-handle, longitudinal, and flap tears were rated 88% excellent-good, whereas horizontal cleavage and degenerative and complex tears had only 45% excellent-good sco

The results of three different types of meniscectomy have been compared in 219 knees, 71 treated by arthroscopic partial meniscectomy, 45 treated by open partial meniscectomy, and 103 treated by open total meniscectomy, with a mean follow-up of 4.3 years. Knees which had undergone previous operations or had other simultaneous operative procedures or ligamentous damage were excluded. Knees with chondromalacia were included provided that this did not amount to frank osteoarthritis. Simple indicators were used for the rate of early recovery from the operation, and the Tapper and Hoover scale was

Knee arthroscopy to address meniscus tears is among the most common orthopedic procedures performed, and technical advances in the treatment of meniscus tears have affected the treatment options available to orthopedic surgeons. The purpose of this study was to perform a large cross-sectional analysis of orthopedic patients to investigate trends in arthroscopic meniscectomy and meniscus repair in the United States. Patients who underwent arthroscopic meniscectomy (Current Procedural Terminology codes 29881 and 29880) and arthroscopic meniscus repair (Current Procedural Terminology codes 29882

Meniscal injuries are frequently associated with acute injuries to the anterior cruciate ligament (ACL). With the passage of time, this frequency increases significantly. The management of the torn meniscus varies with the type of lesion and the patient's goals. When possible, meniscus repair combined with ACL reconstruction is recommended in young, athletically active patients. Repair may be accomplished by open or closed techniques. Because of significant risks associated with arthroscopic approaches, an outside-in type of repair has been devised. Overall, the clinical success approaches 90%

Endoscopic surgery can replace operative procedures. The evolution of arthroscopy leads to realize intra articular surgery more and more complicated. However, any surgical procedure involve some risks, and surgeons have to be prudent in use of new instruments or new technics. These need a radio surgical environment adapted to all surgery.
