New meniscus repair technique for peripheral tears near the posterior tibial attachment of the posterior horn of the medial meniscus.

We introduce a suture technique to repair a peripheral tear near the posterior tibial attachment of the posterior horn. A suture hook was inserted through the posteromedial portal, and the peripheral capsular rim was penetrated from superior to inferior by the sharp hook. Both relay limbs were brought out through the posteromedial portal. The outer limb of the superior peripheral capsular rim was identified with a hemostat. An 18-gauge spinal needle loaded with a No. 0 polydioxanone suture (PDS) was introduced into the joint from the anteromedial portal; it was passed through the joint space u

[Adult lateral meniscus].

Meniscal lesion does not mean meniscectomy and this is particularly true for the lateral meniscus. The reputation of mildness of the meniscectomy is usurped. The rate of joint space narrowing after lateral meniscectomy is of 40% at a follow-up of 13 years compared to 28% for the medial meniscus (symposium SFA 1996). Several arguments explain those results: biomechanical: the lateral meniscus contributes to the congruence; particularly the lateral meniscus is the zone where antero-posterior translational during knee flexion is 12 mm. The pejorative effects of lateral meniscectomy have conducted

Medial meniscus interposition in proximal tibial epiphyseal fracture: a case report and review of the literature.

Proximal tibial fractures are rare in children, and most are treated conservatively. We report a case of a minimally displaced proximal tibial epiphyseal injury (Salter-Harris type III) in an 11-year-old child. We initially thought to treat this fracture conservatively, but the presence of massive haemarthrosis and considerable pain made us choose arthroscopy under general anaesthetic. During examination under anesthesia no instability could be detected, but the knee was locked in 5 degrees of flexion. On screening the fracture was not reducible anatomically. At arthroscopy the medial meniscus

Surgical treatment for meniscal injuries of the knee in adults.

BACKGROUND: Injuries to the knee menisci are common and operations to treat them are among the most common procedures performed by orthopaedic surgeons. OBJECTIVES: To evaluate the effects of common surgical interventions in the treatment of meniscal injuries of the knee. The four comparisons under test were: a) surgery versus conservative treatment, b) partial versus total meniscectomy, c) excision versus repair of meniscal tears, d) surgical access, in particular arthroscopic versus open. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries Group trials register, MEDLINE and

[Lesions of the extensor muscles of the knee caused by meniscus injury].

The damage of the knee extensors was examined with integrated EMG and with histochemical and electron microscopical methods in 23 patients with meniscal injury. The integrated DMG has proved in 2/3 of the patients tonic, in 1/3 both tonic and phasic damage of the muscle function. With histochemical examination the decrease of the ratio of the fibers of type 1 was found and also the atrophy of the same fibre type cells in 18-36 per cent. Ultrastructural examinations have shown out of the damage of the contractive elements an increase of the interstitial connective tissue together with alteratio

Reduced anterior tibial translation associated with adaptive changes in the anterior cruciate ligament-deficient joint: goat model.

Adaptive changes in the menisci and adjacent posterior capsule were documented within anterior cruciate ligament-deficient knee (stifle) joints in the goat model. These physical changes in the menisci and capsule developed over time and were associated with reduction in the initial (time zero) abnormal anterior tibial translation following transection of the anterior cruciate ligament. At 50 N of applied force, the normal goat knee joint has a total anterior-posterior translation of 0.6+/-0.1 mm (+/- SEM) at 45 degrees of flexion and 0.3+/-0.1 mm at 90 degrees. The translation immediately afte

Wrisberg variant of the lateral discoid meniscus in children: review of the literature and presentation of case series.

INTRODUCTION: The Wrisberg variant of the discoid lateral meniscus is a very rare disorder and is characterized by the hypermobility and instability of the meniscus caused by the absence of its posterior tibial attachment, with only its meniscofemoral junction (Wrisberg's ligament) maintained, and inserted in the posterior horn of the meniscus. As a result, the posterior horn of the lateral meniscus is mobile; often subluxing into the joint. MATERIALS AND METHODS: A total of eight skeletally immature patients with symptomatic Wrisberg variant of the discoid lateral meniscus were included in t

Patterns of cartilage degeneration in knees with medial tibiofemoral offset.

OBJECTIVE: To determine if radiographic medial tibiofemoral offset (MTFO) is associated with: (1) magnetic resonance imaging (MRI) pathology of cartilage, meniscus, and ligament; and (2) a distinct pattern of lateral cartilage degeneration on MRI. MATERIALS AND METHODS: Three hundred consecutive adult knee MRIs with anteroposterior (AP) radiographs were retrospectively reviewed, and 145 studies were included. MTFO was defined as a medial extension of the medial femoral condyle beyond the articular surface of the medial tibial plateau on weight-bearing AP radiographs. The patients were then di

A comparative anatomical study of the human knee and six animal species.

PURPOSE: Animal models are an indispensable tool for developing and testing new clinical applications regarding the treatment of acute injuries and chronic diseases of the knee joint. Therefore, the purpose of this study was to compare the anatomy of the intra-articular structures of the human knee to species commonly used in large animal research studies. METHODS: Fresh frozen cow (n=4), sheep (n=3), goat (n=4), dog (n=4), pig (n=5), rabbit (n=5), and human (n=4) cadaveric knees were used. Passive range of motion and intra-articular structure sizes of the knees were measured, the structure s

Principles and decision making in meniscal surgery.

This article provides a review of the basic science and clinical information available to the orthopedist on which a systematic approach to meniscal surgery can be based. Attitudes toward the meniscus have changed dramatically in the last 50 years. Laboratory investigations show that the menisci participate in many important functions, including tibiofemoral load transmission, shock absorption, lubrication, and passive stabilization of the knee joint. Histologic/structural analyses reveal the menisci to be annular structures, with the ability to transmit and properly distribute load over the t

Finite Element Modelling Simulated Meniscus Translocation and Deformation during Locomotion of the Equine Stifle.

We developed a finite element model (FEM) of the equine stifle joint to identify pressure peaks and simulate translocation and deformation of the menisci. A series of sectional magnetic resonance images (1.5 T) of the stifle joint of a 23 year old Shetland pony gelding served as basis for image segmentation. Based on the 3D polygon models of femur, tibia, articular cartilages, menisci, collateral ligaments and the meniscotibial ligaments, an FEM model was generated. Tissue material properties were assigned based on data from human (Open knee(s) project) and bovine femoro-tibial joint available

Magnetic resonance evaluation in long term follow up of operated lateral tibial plateau fractures.

BACKGROUND: A lack of data exists on the long-term magnetic resonance imaging (MRI) findings after surgical repair of tibial plateau fractures (TPFs). We evaluated the MRI findings 13 to 31 years after surgical repair of TPFs, focusing especially on the pathological changes in the ligaments, menisci, and cartilage. METHODS: Twenty-three patients with 24 TPFs underwent open reduction and internal fixation with the same fork-shaped surgical plate that was used in our institution until 1999. No patient underwent preoperative or immediately postoperative MRI. The knees of all patients who underwe

Degenerative arthritis after tibial plateau fractures.

Secondary osteoarthritis after tibial plateau fracture was found in 44% of 131 cases at 7.6 (3.3-13.4) years follow-up. Narrowing of the joint space was noted during the first 7 years after injury, usually in the same compartment as the fractured plateau. The incidence slightly increased with the age of patients. Removal of a meniscus during the fracture surgery resulted in secondary degeneration in 74% of the cases. When a meniscus was intact or repaired, the proportion of degenerative cases was only 37%. Normal or slight valgus alignment of the tibial plateau with intact menisci protected be

Anterior approach to the knee with osteotomy of the tibial tubercle for bicondylar tibial fractures.

Eight patients--six who had a bicondylar fracture of the tibia and two who had a complex fracture-dislocation--were treated by open reduction and internal fixation that was achieved through an anterior approach to the knee. The approach included elevation of the tibial tubercle, proximal retraction of the extensor mechanism (patellar tendon, retropatellar fat pad, and patella), and transection and detachment of the anterior horn of one or both menisci. The extent of the approach depended on the specific need for exposure. The quality of reduction was better and the rate of complications was lo

High tibial osteotomy combined with joint debridement: a long-term study of results.

Combined high tibial osteotomy and joint debridement have been performed in the treatment of unicompartmental osteoarthritis of the knee. A specially designed angled compression plate was used for fixation after a wedge upper tibial resection, and this permitted early joint motion and eliminated cast immobilization. Knee stiffness was thereby prevented. The adjunctive joint debridement enabled us to remove torn menisci, loose bodies and marginal osteophytes. Seventy-one patients were operated upon between 1966 and 1978. An average follow-up period of six years was recorded. Critical analysis o

MR imaging in congenital lower limb deformities.

Treatment for children with congenital deformities of the lower extremities may vary, depending on the state of the unossified skeletal structures and surrounding soft tissues. The purpose of our study was to demonstrate the spectrum of the osteochondral and extrasosseous abnormalities as depicted with MR imaging. We retrospectively reviewed MR examinations of 13 limbs of ten children (aged 1 month-9 years, mean 2.1 years) with longitudinal and transverse deformities of the lower extremities. The lesions imaged were fibular hemimelia (n = 5), tibial hemimelia (n = 5), and congenital constricti

Posterior Horn Meniscus Centroid Position Is Altered Soon After Noncontact ACL Injury in Males and Females.

Anterior cruciate ligament (ACL) injury and subsequent changes in the magnitude and distribution of contact stress about the articular surfaces of the knee are associated with post-traumatic osteoarthritis. Soon after ACL injury, changes in tibial articular cartilage thickness occur that can be explained, in part, by abnormal positioning of the tibia relative to the femur. However, little is known about the effects of ACL injury on the positions of the menisci. The purpose of this case-control study was to determine the effect of ACL injury on posterior horn meniscus centroid (PHMC) position r

Arthroscopy of the normal cadaveric ovine femorotibial joint: a systematic approach to the cranial and caudal compartments.

OBJECTIVES: Preclinical studies using large animal models play an intergral part in translational research. For this study, our objectives were: to develop and validate arthroscopic approaches to four compartments of the stifle joint as determined via the gross and arthroscopic anatomy of the cranial and caudal aspects of the joint. METHODS: Cadaveric hindlimbs (n = 39) were harvested from mature ewes. The anatomy was examined by tissue dissection (n = 6), transverse sections (n = 4), and computed tomography (n = 4). The joint was arthroscopically explored in 25 hindlimbs. RESULTS: A cranio-

Discoid Lateral Meniscus Evaluation and Treatment.

Discoid lateral meniscus (DLM) is a variant of meniscal histomorphology that often presents in young, active pediatric populations. Patients may present with mechanical symptoms, joint line pain, swelling, and loss of motion especially with lack of terminal knee extension. DLM are prone to tearing, most frequently with horizontal patterns in children and complex patterns in adults. The PRiSM classification and assessment of DLM provide a comprehensive approach to evaluating DLMs arthroscopically, focusing on the following four factors: (1) meniscal width, (2) meniscal height, (3) stability, an
