Symptomatic ring-shaped lateral meniscus.

Abnormalities of the lateral meniscus in humans are a long-known fact. Three different types of discoid menisci are described in Watanabe's atlas, being considered as congenital malformations. For the second time in the literature, a fourth ring-shaped type of discoid lateral meniscus was described as an incidental finding in this Journal by Monllau et al. in 1998. The present case is the first description of a symptomatic, ring-shaped lateral meniscus with a concomitant, recurrent cyst at the lateral aspect of a child's knee.

Ultrasound guided arthroscopic meniscus surgery.

Ultrasound is a useful adjunctive intra-operative imaging modality for soft tissue pathologies. It is readily available in the operating theater and has the distinct advantage of being more portable than an MRI, with no harmful radiation. It is especially useful for determining if any extra articular cysts have been decompressed through arthroscopic surgery, if a parameniscal cyst has been removed completely or if extrusion has been reduced adequately in a meniscus root repair. In our series, intra-operative ultrasound was used for two cases of medial meniscus horizontal tear with meniscus cys

Discoid lateral meniscus: current concepts.

The discoid meniscus is a congenital morphological abnormality encountered far more commonly on the lateral than the medial side. The discoid lateral meniscus (DLM) is more prevalent in Asia with an incidence of 10%-13%, than in the Western world with an incidence of 3%-5%. DLM can be bilateral in more than 80% cases. Due to its abnormal shape and size, the discoid meniscus is prone to tearing and has an impact on gait mechanics. The discoid meniscus has deranged collagen arrangement and vascularity which can have implications for healing after a repair. Patients with a DLM may or may not be s

Augmentation Techniques for Meniscus Repair.

Menisci display exquisitely complex structure and play an essential weight-bearing role in the knee joint. A torn meniscus is one of the most common knee injuries which can result in pain and mechanical abnormalities. Tear location is one aspect which determines the endogenous healing response; tears that occur in the peripheral densely vascularized zone of the meniscus have the potential to heal while the healing capacity is more limited in the less vascularized inner zones. Meniscectomy was once widely performed, but led to poor radiographic and patient-reported mid- and long-term outcomes.

Discoid lateral meniscus in children.

The discoid meniscus is the most common abnormal meniscal variant in children. It is most likely a congenital deviation and usually occurs laterally. The discoid shape results in greater coverage of the tibia and usually is associated with increased thickness of the meniscus that may lead to abnormal shearing forces across the knee joint. The Watanabe classification divides this anomaly into three distinct types: complete, incomplete, and Wrisberg ligament. The complete and incomplete types are often asymptomatic; unless symptomatic, they generally should be left alone. Tears should be treated

The "all-inside" meniscus repair.

The arthroscopic "all-inside" meniscus suturing technique offers the arthroscopist a way of placing vertically oriented sutures through peripheral posterior horn tears located posterocentral without the risks of nerve, vessel, or posterior capsular entrapment inherent in both the "outside-in" and the "inside-out" arthroscopic methods. This technique introduces new instrumentation that allows the surgeon to both place sutures and tie suture knots intraarticularly under arthroscopic control.

Current status of meniscus salvage.

The direct repair of meniscus tears with rasp preparation of all tear surfaces, stable suture fixation, and exogenous clot injection is effective for single longitudinal tears with peripheral white rims of 4 mm and less. Radial split and flap tears at the posterior horn of the lateral meniscus can be directly repaired as well. Single longitudinal tears typically in chronic knees with peripheral white rims of 5 mm and greater may have better reliability with use of the fascia sheath. The sheath is indicated in complex tears including flaps and radial splits. A structured rehabilitation program

Comprehensive Arthroscopic Characterization of Discoid Meniscus Tears and Instability Using the PRiSM Discoid Meniscus Classification.

The discoid meniscus is a congenital abnormality, with the vast majority occurring in the lateral meniscus. More commonly seen in pediatric populations, patients present with acute or chronic knee symptoms such as joint line pain, audible or palpable mechanical symptoms, and the inability to achieve terminal extension. The classic discoid classification system by Watanabe excludes anterior and horizontal instability and tearing that commonly occur with this pathology. A comprehensive classification, the Pediatric Research in Sports Medicine (PRiSM) Discoid Meniscus Classification, was develope

Association between discoid lateral meniscus and medial meniscus posterior root tear: A retrospective cohort study.

PURPOSE: Medial meniscus (MM) posterior root tear (PRT) has gained increasing attention because of its biomechanical impact and rapid progression to osteoarthritis. Identifying the risk factors for MMPRT is essential for early diagnosis. Although the discoid lateral meniscus (DLM) is a common congenital variant, no studies have reported on the relationship between MMPRT and DLM. This study aimed to examine the relationship between MMPRT and DLM. METHODS: This retrospective cohort study included 94 matched knees that underwent arthroscopic surgery between 2015 and 2021 (58 with MMPRT and 36 wi

Lateral meniscus "PASTA injury": Partial-thickness radial tear of the lateral meniscus: A case report.

INTRODUCTION: We report a relatively rare case of partial-thickness radial tear in the inferior surface of lateral meniscus, while the superior surface is intact. This situation was similar to PASTA rotator cuff tear. Meanwhile, there is a full-thickness radial tear in the edge. CASE PRESENTATION: A 17-year-old boy twisted the left knee while playing basketball. Magnetic resonance imaging (MRI) revealed radial tear of the lateral meniscus. During arthroscopy, it was found that there was a full-thickness radial tear of about 2 mm located in the edge. Partial meniscectomy was performed to treat

Biomechanical comparison of meniscus-suture constructs for pullout repair of medial meniscus posterior root tears.

BACKGROUND: Transtibial pullout repair for posterior meniscus root tear is widely performed to restore meniscal function. However, it is sometimes technically difficult to pass the suture through the posterior medial meniscus root in narrow joint space. To address this limitation, a new suture technique using an all-inside meniscal suture device through the tibial tunnel was proposed. The purpose of the present study was to compare the biomechanical properties of a meniscus-suture construct prepared using an all-inside meniscal suture device and those of the construct prepared using convention

Medial meniscus posterior root repair decreases posteromedial extrusion of the medial meniscus during knee flexion.

BACKGROUND: Medial meniscus (MM) medial extrusion in the coronal plane does not always improve, even after repair. This study aimed to determine the extent of posteromedial extrusion of the MM during knee flexion before and after MM pullout repair using three-dimensional magnetic resonance imaging (MRI). METHODS: Data from 14 patients (mean age, 63.4 years; 86% female) who had undergone MM pullout repair at the current institution between August 2017 and October 2018 were retrospectively reviewed. The MRIs were performed pre-operatively and ≥3 months postoperatively. Three-dimensional MRIs of

Surgical management of the discoid meniscus.

BACKGROUND: Discoid meniscus is the most frequent intraarticular paediatric knee congenital condition. Only symptomatic patients require surgical attention with arthroscopy. The incidence of discoid meniscus repair is increasing. DIAGNOSIS: Typical presentation is an insidious popping or snapping when mobilizing the knee. The older individuals can experience pain and effusion. Restricted knee motion suggests a meniscal rim instability. MRI confirms the diagnosis, with shape and volume criteria, and helps planning surgery. The most common tear pattern of the discoid meniscus is a horizontal te

Ramp Lesions of the Medial Meniscus.

PURPOSE OF REVIEW: To provide an overview of the recent scientific literature about ramp lesions of the medial meniscus and to summarise the current evidence on their prevalence, classification, biomechanics, surgical techniques and clinical outcomes. RECENT FINDINGS: Ramp lesions may be present in more than 1 patient undergoing ACL reconstruction out of 5 and almost half of the medial meniscal tears observed in this population. Due to the risk of persistent anterior and rotational laxity after ACL reconstruction, their repair has been advocated. There is no general agreement to date on wheth

The Natural History of Meniscus Tears.

BACKGROUND: In order to determine whether treatments are effective in the treatment of meniscus tears, it is first necessary to understand the natural history of meniscus tears. The purpose of this paper is to review the literature to ascertain the natural history of meniscus tears in children and adolescents. METHODS: A search of the Pubmed and Embase databases was performed using the search terms "meniscus tears," "natural history of meniscus tears," "knee meniscus," "discoid meniscus," and "natural history of discoid meniscus tears." RESULTS: A total of 2567 articles on meniscus tears, 28

Formation and maturation of the murine meniscus.

Meniscal injuries are commonplace, but current surgical repair procedures do not prevent degenerative joint changes that occur after meniscal injury and often lead to osteoarthritis. Successful tissue regeneration in adults often recapitulates events that occur during embryogenesis, suggesting that understanding the regulatory pathways controlling these early processes may provide clues for developing strategies for tissue repair. While the mouse is now widely used to study joint diseases, detailed knowledge of the basic biology of murine meniscus is not readily available. Here, we examine men
