An Unusual Case of Pylephlebitis With the Involvement of Portal and Superior Mesenteric Veins.

Pylephlebitis is a low-incidence condition worldwide, more commonly affecting the adult population. Its clinical presentation is nonspecific, thus requiring a detailed clinical history and confirmation through imaging studies (contrast-enhanced computed tomography). The literature recommends antibiotic treatment for at least six weeks and the use of anticoagulants. When diagnosed and treated early, the prognosis is generally favorable. This report presents a case of pylephlebitis with clinical symptoms of fever, vomiting, and diarrhea. The patient was diagnosed with pylephlebitis involving the

Idiopathic myointimal hyperplasia of mesenteric veins: a rare mimic of idiopathic inflammatory bowel disease.

Idiopathic myointimal hyperplasia of mesenteric veins (IMHMV) is a rare and poorly understood disease that occurs in the rectosigmoid colon of predominantly young, previously healthy male patients. IMHMV typically requires segmental resection due to complications after a relatively protracted clinical course. This disease presents a challenging diagnostic dilemma for the clinician because it is initially often confused with chronic idiopathic inflammatory bowel disease. We report a case of IMHMV, illustrate endoscopic and histopathologic features, and review key characteristics of this rare en

[Combined Resection of the Superior Mesenteric Vein and Revascularization for Cecal Cancer with Extensive Invasion and Tumor Thrombosis in the Superior Mesenteric Vein-A Case Report].

The patient was a 76-year-old woman. When she visited a nearby clinic for pedal edema and palpitations, she was found to have severe anemia, which led to a referral to our hospital. Cecal cancer with regional lymph node metastasis and tumor thrombosis in the superior mesenteric vein were diagnosed. However, because no distant metastasis was detected, extended right hemicolectomy was performed. Only the first jejunal vein was preserved because the tumor thrombosis extended from the ileocolic vein to the superior mesenteric vein, and complete(R0)resection was achieved. When ileocolic anastomosis

Techniques of Oncovascular Reconstruction of Portal and Mesenteric Veins during Pancreatic and Hepatobiliary Surgery.

Major vessel invasion, particularly involving the portal and superior mesenteric veins, poses significant challenges during the radical resection of hepatobiliary and pancreatic cancers. Oncovascular surgery is essential for curative outcomes, and often requires portomesenteric vein reconstruction. Techniques, such as lateral venorrhaphy, patch repair, end-to-end anastomosis, and interposition grafting, have been employed. Autogenous veins such as the internal jugular, left renal, external iliac, or femoral veins are options, although not always available. Alternatives include great saphenous

Focal myointimal hyperplasia of mesenteric veins is associated with previous trauma in surgical specimens.

Idiopathic myointimal hyperplasia of mesenteric veins (IMHMV) is a rare and poorly understood disease that occurs in the rectosigmoid colon of predominantly young, previously healthy, male patients. This disease is often confused with chronic idiopathic inflammatory bowel disease clinically, and pathologists may miss the diagnosis unless elastin stains are performed because diseased veins may readily be mistaken for arteries. The etiology of IMHMV is unclear, but a traumatic pathomechanism resulting in arterialization of the veins has been proposed. Review of bowel resection specimens (n=68) f

Mucosal biopsy findings and venous abnormalities in idiopathic myointimal hyperplasia of the mesenteric veins.

Idiopathic myointimal hyperplasia of the mesenteric veins (IMHMV) is a rare cause of intestinal ischemia secondary to venous compromise. A patient with this condition who presented with crampy abdominal pain, diarrhea, and rectal bleeding initially attributed to inflammatory bowel disease had several colonoscopies and ultimately a sigmoid colectomy. The colonic mucosa in biopsies performed at initial presentation and subsequently and in the resection specimen contained numerous hyperplastic, thick-walled, hyalinized vessels in the lamina propria, which have not been described in this entity pr

Reactivity of isolated bovine mesenteric and hepatic veins to vasoactive agents and specific antigen.

In an attempt to investigate the possible contribution of the mesenteric and hepatic vasculature to the haemodynamic changes occurring during bovine systemic anaphylaxis, the pharmacological reactivity of isolated bovine mesenteric and hepatic veins was studied. Both mesenteric and hepatic veins contracted in the presence of histamine, 5-hydroxytryptamine (5HT), acetylcholine, dopamine and noradrenaline, and relaxed in the presence of isoprenaline. The mesenteric veins contracted in the presence of concentrations of isoprenaline greater than 1 mug/ml. By the use of antagonists, the receptor po

Delayed development of inferior mesenteric vein in human foetuses.

BACKGROUND: The superior mesenteric vein appears as a fusion between irregularly-shaped slits of the midgut mesentery tissue at 5-6 weeks. In contrast, there has been no report identifying when and how the inferior mesenteric vein (IMV) develops. So we aimed to find the human initial IMV. MATERIALS AND METHODS: We examined 1) sagittal histological sections of seven human foetuses with 45-75 mm crown rump length or CRL (10-12 weeks); 2) horizontal sections of 15 foetuses with 70-155 mm CRL (12-18 weeks); and 3) horizontal sections of 12 late-term foetuses with 225-328 mm CRL (28-41 weeks). Spontaneous Pancreatic Pseudocyst-Superior Mesenteric Vein Fistula.

Pancreatic pseudocyst formation is a common complication of chronic pancreatitis. Rarely, a fistula develops between the pseudocyst and the portal venous system. We present a case of a 50-year-old man who was found to have a pancreatic pseudocyst-superior mesenteric vein fistula after being evaluated for several months of abdominal pain and weight loss. The patient was treated with endoscopic stenting of the pancreatic duct along with early enteral nutrition and suppressive antibiotics, which resulted in improvement in his condition. This case report highlights clinical presentation and the co

Pylephlebitis of a variant mesenteric vein complicating sigmoid diverticulitis.

Pylephlebitis--suppurative thrombophlebitis of the portal and/or mesenteric veins--is a rare complication of abdominal infections, especially diverticulitis. It can lead to severe complications such as hepatic abscess, sepsis, peritonitis, bowel ischemia, etc., which increase the mortality rate. Here we present a case of suppurative thrombophlebitis of the inferior mesenteric vein, as a complication of sigmoid diverticulitis. The epidemiology, clinical and radiological features as well as treatment strategies are discussed. We also review the anatomy of the mesenteric vein given its anatomic v

Clinical Management of Acute Portal/Mesenteric Vein Thrombosis.

BACKGROUND: Acute thrombosis of the portal vein (PV) and/or the mesenteric vein (MV) is a rare but potentially life-threatening disease. A multitude of risk factors for acute portal vein thrombosis (PVT)/mesenteric vein thrombosis (MVT) have been identified, including liver cirrhosis, malignancy, coagulation disorders, intra-abdominal infection/inflammation, and postoperative condition. METHODS: This article analyses the treatment options for acute PVT/MVT. RESULTS: Initially, the clinical management should identify patients with an intra-abdominal focus requiring immediate surgical interven

Effects of inferior mesenteric vein flow in patients with cirrhosis.

BACKGROUND & AIMS: The inferior mesenteric vein (IMV) is detected in more than 90% of computed tomography images. Little is known about the hemodynamic features of IMV as a collateral vessel in portal hypertension, or its effects in clinical presentation and outcome. We investigated the roles of the IMV in portal hemodynamics, clinical presentation, and outcomes of patients with cirrhosis. METHODS: We performed a prospective study of 467 patients with cirrhosis (274 men; age, 64.6 ± 10.9 y). We assessed hemodynamics in the IMV using Doppler sonography and compared these data with patients' cl

Direct thrombolytic therapy in portal and mesenteric vein thrombosis.

A 34-year-old female from Laos presented to the emergency department with a 3-week history of worsening abdominal pain; she subsequently developed an acute abdomen requiring emergent exploratory laparotomy. An intraoperative angiography was performed, which revealed complete portal vein thrombosis. A 5F 20-cm infusion catheter was placed through an omental vein, and tissue plasminogen activator was administered directly into the catheter with successful decrease in thrombus burden. There are few controlled data on which to base clinical decisions in patients with portal vein thrombosis. Our ca

The anatomic course of the first jejunal branch of the superior mesenteric vein in relation to the superior mesenteric artery.

Introduction. The purpose of this study is to determine the anatomic course of the first jejunal branch of the superior mesenteric vein (SMV) in relation to the superior mesenteric artery (SMA). Methods. Three hundred consecutive contrast-enhanced computed tomography (CT) scans were reviewed by a surgical oncologist with confirmation of findings by a radiologist. Results. The overall incidence of a first jejunal branch coursing anterior to the SMA was 41%. There was no correlation between patient gender and position of the jejunal branch. In addition, there was no correlation between size of t

Mesenteric Vein Thrombosis and Its Interventional-Radiological Management.

OBJECTIVE AND BACKGROUND: Mesenteric venous thrombosis (MVT) is a rare but clinically important cause of mesenteric ischemia. It most often affects the superior mesenteric vein and is associated with high morbidity and mortality. Nonspecific symptoms often delay diagnosis. Severe disease courses may occur in the presence of intestinal infarction. OBJECTIVE AND BACKGROUND: The aim of this article is to provide a structured overview of the etiopathogenesis, imaging findings, and interventional radiological treatment options for MVT. MATERIALS AND METHODS: Narrative, literature-based review tak

Pylephlebitis With Splenic and Mesenteric Vein Thrombosis in a Patient With Diverticulitis.

Diverticulitis is a common gastrointestinal complaint that refers to inflammation of colonic diverticula. Its incidence has increased partly due to the increase in prevalence of diverticulosis, which results from poor dietary habits and chronic constipation. An acute diverticulitis episode can vary in severity, ranging from outpatient management of mild abdominal discomfort to inpatient admission requiring emergent surgery. Some common complications associated with diverticulitis include bowel wall perforation, microperforation, abscess formation, bowel obstruction, and colonic fistulas. A les
