Associations of Physical Activity With Mortality Across Cardiovascular-Kidney-Metabolic Syndrome Stages: Evidence From NHANES 1999 to 2018.

BACKGROUND: Cardiovascular-kidney-metabolic (CKM) syndrome integrates cardiovascular, renal, and metabolic dysfunction and is associated with high mortality. Stage-stratified evidence linking physical activity (PA) to mortality remains limited. METHODS: We analyzed 19 930 adults with CKM stages 0 to 4 from the NHANES (National Health and Nutrition Examination Survey, 1999-2018), with mortality follow-up through 2019. PA was assessed by duration (minute/week) and volume (metabolic equivalent of task-min/week) across recreational, transport, and occupational domains. Survey-weighted Cox models

Energy requirements of pregnancy in rural Thailand.

The energy cost of pregnancy was measured in rural Thai women between 10 wk gestation and term. The energy cost of pregnancy, which includes the increase in basal metabolic rate (BMR), the energy equivalent of maternal fat laid down, and the assumed energy equivalent of fetal fat and fetal and maternal protein, was 202 MJ (1 MJ = 239 kcal). BMR increased by 100 MJ overall and maternal fat gain averaged about 1.3 kg, equivalent to 60 MJ. The energy cost was more than covered by an overall increase in food intake of 238 MJ with little evidence of any complementary reduction in total energy expen

Impact of Cardiac Resynchronization Therapy on Daily Physical Activity in Heart Failure Patients.

PURPOSE: Cardiac resynchronization therapy (CRT) has emerged as a beneficial therapy for heart failure (HF) patients. It has been shown to enhance cardiac pump function and increase exercise capacity in patients with HF who display wide QRS complex on their electrocardiogram. To date, few studies have assessed daily physical activity (PA) in CRT patients. The objective of this pilot study was to assess the impact of CRT on the physical function and daily PA of HF patients who met the standard indications for CRT implantation. METHODS: The daily PA of 18 CRT patients was measured using a Sense

Saccharomyces cerevisiae and Bacillus subtilis show modest, node-dependent effects on insulin sensitivity in periparturient cows.

Periparturient immunometabolic adaptations yield small, node-dependent shifts in endotoxin proxies and insulin-sensitivity indices. Direct-fed microbials (DFM) are mechanistically plausible but often produce modest metabolic readouts. Test whether Saccharomyces cerevisiae (SC), Bacillus subtilis (BS), or SC + BS alter endotoxin (blood, milk) and metabolic families versus Control (Ctrl) across prespecified nodes, and evaluate prespecified equivalence where applicable. A 4-arm trial enrolled 20 Holsteins from -60 to + 21 d around calving (Ctrl, SC, BS, SC+BS). Blood and milk were sampled at mult

Importance of exercise capacity in predicting outcomes and determining optimal timing of surgery in significant primary mitral regurgitation.

BACKGROUND: In primary mitral regurgitation (MR), exercise echocardiography aids in symptom evaluation and timing of mitral valve (MV) surgery. In patients with grade ≥3 primary MR undergoing exercise echocardiography followed by MV surgery, we sought to assess predictors of outcomes and whether delaying MV surgery adversely affects outcomes. METHODS AND RESULTS: We studied 576 consecutive such patients (aged 57±13 years, 70% men, excluding prior valve surgery and functional MR). Clinical, echocardiographic (MR, LVEF, indexed LV dimensions, RV systolic pressure) and exercise data (metabolic e

Metabolic alkalosis mimicking the acute sequestration of HCl in rats: bucking the alkaline tide.

Loss or sequestration of HCl induces an acute metabolic alkalosis. The purpose of these experiments was to examine the renal handling of bicarbonate (HCO3-) in awake, euvolemic rats to determine if a significant degree of bicarbonaturia would develop because, if present, it could lead to large negative balances for sodium (Na+) and/or potassium (K+). Metabolic alkalosis was induced acutely by creating the equivalent of an acute and large loss of HCl; the net effect was to lose Cl- and gain equimolar HCO3- in rats that were in Na+ and K+ balance. A loop diuretic induced the loss of 1,860 mumol

Metabolic Effects of Cortisone Acetate vs Hydrocortisone in Patients With Secondary Adrenal Insufficiency.

CONTEXT: Pharmacokinetic properties of cortisone acetate (CA) and hydrocortisone (HC) differ because CA needs to be converted into cortisol to become active. OBJECTIVE: This work analyzed the metabolic consequences of switching CA to an equivalent daily dose of HC in patients with secondary adrenal insufficiency (SAI). DESIGN: This was a post hoc analysis from a prospective study including individuals with hypopituitarism receiving growth hormone replacement. Data were collected before and after a switch from CA to an equivalent dose of HC (switch group). Two control groups were included: pa

Accelerometer Output and Oxygen Uptake in Adults With and Without Down Syndrome: METs vs. Percent VO2Reserve.

BACKGROUND: The estimation of physical activity (PA) and sedentary behaviour with accelerometers is typically based on the relationship between accelerometer output and metabolic equivalents (METs)-an index of PA intensity. But for adults with Down syndrome (DS), PA intensity may be better reflected in the percent oxygen uptake reserve (%VO2Reserve), as it accounts for their lower aerobic fitness. This study examined if the relationship between accelerometer output and METs or %VO2Reserve across various PAs and sedentary behaviours differs between adults with and without DS. METHODS: Forty-on

Evaluation of Light Physical Activity Measured by Accelerometry and Mobility Disability During a 6-Year Follow-up in Older Women.

IMPORTANCE: Almost 1 in 4 women older than 65 years is unable to walk 2 to 3 blocks, and mobility disability is a key factor associated with loss of independence. Lack of moderate to vigorous-intensity physical activity is associated with mobility disability, but whether lighter physical activity is associated with mobility disability is unknown. OBJECTIVE: To determine the association of light-intensity physical activity and incident mobility disability among older women. DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort study included women enrolled in the Objectively Measured Phy

Comparative study on the neurotoxicity of five bisphenols using zebrafish embryos/larvae models.

Bisphenols (BPs) are known to exert neurotoxic effects, but comparative studies of their neurotoxicity using equivalent concentrations (instead of identical ones) remain limited. This study assessed the neurotoxicity of bisphenol A (BPA), bisphenol S (BPS), bisphenol F (BPF), fluorine-9-bisphenol (BHPF), and bisphenol AF (BPAF) in zebrafish embryos/larvae exposed to concentrations equivalent to 2 % of each BP's LC50. Results showed all five BPs significantly decreased 72 h post-fertilization (hpf) body length and increased 96 hpf mortality but had no notable impacts on hatching rate, heart rat

Metabolic and Alcohol-Associated: Diagnosis and Management.

Metabolic and alcohol/associated liver disease was introduced as a new term to describe those with steatotic liver disease who meet criteria for metabolic dysfunction-associated steatotic liver disease but exceed the allowable limits for alcohol within an upper limit of the equivalent of 50 to 60 g for females and males, respectively. Consumption beyond these limits would be characterized as alcohol-associated liver disease. Clinicians should focus on alcohol cessation counseling, metabolic risk factor management, as well as early referral to hepatology in those suspected to have significant f
