The Versatile Vascularized Second Metacarpal-Base Bone Graft.

Vascularized bone grafts of the wrist and hand help to achieve bone union in various clinical settings such as bone fusion or nonunion treatment. Among the multiple possible donor sites, the base of the second metacarpal is convenient because of its relatively straightforward harvesting, lack of impairment of the hand vascularization, sufficient bone supply for small joint fusion and phalanx or metacarpal nonunion management. The flap is versatile as it can reach any carpal and metacarpal bone as well as the proximal phalanx of all fingers. The arc of rotation of the flap depends on which side

Metacarpal bone reconstruction by a cementless induced membrane technique.

Gunshot wounds to the hand often produce complex injuries and large segmental bone defects. Bone reconstruction remains a challenge in this context. The induced membrane technique is a simple and effective procedure for reconstruction of segmental bone defects. The technique is straightforward but must be performed rigorously. Usually polymethylmethacrylate (PMMA) cement is required for the first stage of the surgery. We describe four cases of metacarpal bone reconstruction after gunshot wounds in a limited-resource setting. Two patients were treated using the induced membrane technique with a

[The surgical treatment of fractures of the base of the fifth metacarpal bone].

Fractures of the base of the fifth metacarpal bone are relatively uncommon injuries. In contrast to similar fractures of the first metacarpal bone, various methods of treatment have been advocated. Some believe that cast immobilization or immediate unrestricted motion is a reliable method, while others recommend surgery and pinning or plate fixation. Fractures of the base of the fifth metacarpal bone are inherently unstable and inadequate reduction may result in pain, functional disability and osteoarthritic changes. We retrospectively reviewed 14 cases with intra-articular or extra-articular

Distraction-fixation in the primary treatment of metacarpal bone loss.

Nine patients seen in civilian practice with severe open injuries of the hand, including loss of portions of some or all of the metacarpals, were treated by primary restoration of metacarpal length and alignment by distraction-fixation with the use of transverse intermetacarpal Kirschner wires. An external fixation device was added in two of the nine patients. Severe associated soft tissue injuries were present in all but one patient. Two injuries were caused by firearms, and the other seven by heavy machinery. Contractures of the joints were prevented by the use of a second set of wires to po

Iliac Crest Bone Graft With Intramedullary Headless Implant for Metacarpal Bone Loss.

ABSTRACT: Metacarpal bone loss presents a challenging reconstructive dilemma for hand surgeons. While multiple bone grafting techniques have been described, complications including nonunion, graft resorption, fixation requiring prolonged immobilization, stiffness, and the need for multiple procedures are well-documented. We present a technique for managing metacarpal bone loss utilizing a tri-cortical iliac crest graft and an intramedullary metacarpal nail for the treatment of metacarpal fractures and nonunions that is technically simple, fast, and allows for early initiation of motion to decr

Cellular Bone Matrix for the Treatment of Considerable Bone Loss in Two Metacarpal Fractures.

High-energy mechanisms that lead to hand fractures can create substantial bony defects that are not amenable to traditional treatments. Management options include autograft, allograft, and bone graft substitutes such as calcium sulfate/phosphate cements, which provide good results when used appropriately however each strategy has limitations. Cellular bone matrices (CBMs) are newer graft substitutes designed to supply osteogenic cells along with scaffold and inductive factors. We present a case series of metacarpal fractures with considerable ballistic-related defects treated with CBM augmenta

[Measurement of bone mineral density of metacarpal and phalangeal bones of the hand by dual X-ray absorptiometry].

Bone mineral density (BMD) of the metacarpal and phalangeal bones of the hand was evaluated using dual X-ray absorptiometry (DXA) in 104 healthy subjects, 35 patients with rheumatoid arthritis (RA) and 6 young male volunteers with a short arm cast. BMD was positively correlated with the bone volume of metacarpal and phalangeal bones in skeletal specimens. In healthy subjects, the mean values for BMD of metacarpals of the index and middle fingers were significantly higher than those of the ring and little fingers (p < 0.001). The BMD of the middle proximal phalanx was the highest among the prox

Use of a Reverse Metacarpal Bone Flap for the Treatment of Segmental Bone Defects of the Proximal Phalanges.

PURPOSE: The purpose of this retrospective study was to report the results of reconstruction of segmental bone defects of the proximal phalanges using a reverse metacarpal vascularized bone flap harvested from the third metacarpal bone. METHODS: From August 2012 to May 2017, 17 patients with segmental osteomyelitis or necrotic bone of the proximal phalanges were treated. There were 15 male and 3 female patients, with a mean age of 36 years (range, 19-65 years). The mean size of bone defects was 26 × 9 × 9 mm (range, 16 × 6 × 7 mm to 35 × 10 × 7 mm); and the mean size of bone flaps was 27 × 8

Abnormal bone geometry at the metacarpal bone shaft of rheumatoid arthritis patients with maintained muscle-bone relationship.

OBJECTIVE: Metacarpal juxtaarticular bone is altered in rheumatoid arthritis (RA). However, a detailed analysis of disease-related geometric adaptations of the metacarpal shaft is missing. The aim of the present study was to assess the role of RA disease, forearm muscle cross-sectional area (CSA), age, and sex on bone geometry at the metacarpal shaft. METHODS: In 64 RA patients and 128 control subjects, geometric properties of the third metacarpal bone midshaft and forearm muscle CSA were measured by peripheral quantitative computed tomography (QCT). Linear models were performed for cortical

The Use of External Fixator and Iliac Crest Bone Graft in Traumatic Metacarpal Bone Defect.

INTRODUCTION: Metacarpal bone fractures often lead to significant shortening in the phalanges; it can be caused by variable conditions, ranging from idiopathic, and infective to traumatic causes. There are very rare reports of metacarpals shortening in idiopathic primary hypoparathyroidism cases in the literature. Traumatic metacarpal shortening differs from congenital agenesis [2]. This condition leads to both functional and cosmetic limitations which affect the patient's quality of life. CASE REPORT: A 22-year-old male patient was admitted to our outpatient clinic with a traumatic left-hand

Chondroblastoma of a metacarpal bone mimicking an aneurysmal bone cyst: a case report and a review of the literature.

Chondroblastoma of the metacarpal bone has been extremely rare and only seven cases have been reported in the English literature. Here we reported the eighth case of a chondroblastoma that developed on the first metacarpal bone of the right hand of a 21-year-old man. Radiographs showed an expansile osteolytic lesion with a multilocular appearance. In MR images, the lesion showed low intensity in T1 and high intensity in T2-weighted images with multiple fluid-fluid levels, which are findings resembling those of an aneurysmal bone cyst. From the pathological findings, however, it was recognized

Osteoid osteoma of the phalanx and metacarpal bone: report of 17 cases.

BACKGROUND: Osteoid osteoma (OO) is a slowly progressing benign osteoblastic tumour. In the hand, this tumour is rare. We propose a review of seventeen cases of OO of the phalanx and metacarpal bone. PATIENTS AND METHODS: During the period, between 1998 and 2011, a total of seventeen OO in the hand were confirmed by the pathological examination. They were 10 women and 7 men with an average age of 29 years (age range 17-76). The most common site of the tumour was the proximal phalanx in ten cases, followed by middle phalanx in four cases and metacarpal bone in three cases. The diagnosis of OO
