New concept for total wrist replacement.

Wrist prostheses have never achieved the sort of clinical outcomes found with those of hips and knees. We have developed a novel uncemented modular wrist prosthesis with screw fixation, metal-on-metal coupling, and ball-and-socket articulation. Eight patients admitted for wrist arthrodesis to treat primary or secondary osteoarthritis (not rheumatoid) were operated on. The prosthesis reduced the amount of bone removed and spared the distal radioulnar joint. After 7 to 9 years we found that the fixed centre of the ball-and-socket articulation provided good stability and mobility, and relief of p

Metallic wear debris may regulate CXCR4 expression in vitro and in vivo.

CXCR4, the chemokine receptor for CXCL12, also known as SDF-1 (stromal cell derived factor-1), has been shown to play a pivotal role in bone metastasis, inflammatory, and autoimmune conditions but has not been investigated in periprosthetic osteolysis. We co-cultured osteoblast-like cells with increasing concentrations of metallic (Co-35Ni-20Cr-10Mo and Co-28Cr-6Mo) and Co-ions simulating wear debris. Real-time polymerase chain reaction (RT-PCR) and Western blotting were used to quantify gene and protein expression of CXCR4. The expression of tumor necrosis factor-alpha (TNF-α) and the effects

Hypersensitivity reactions to metal implants: laboratory options.

BACKGROUND: All implant compounds undergo an electrochemical process when in contact with biological fluids, as well as mechanical corrosion due to abrasive wear, with production of metal debris that may inhibit repair processes. None of the commonly-used methods can diagnose implant allergies when used singly, therefore a panel of tests should be performed on allergic patients as pre-operative screening, or when a postoperative metal sensitisation is suspected. METHODS: We analysed patients with painful prostheses and subjects prone to allergies using the Patch Test in comparison with the Ly

Ceramic-on-polyethylene hip arthroplasty reduces the risk of postoperative periprosthetic joint infection.

Metal-on-polyethylene (MoP) total hip arthroplasty (THA) prostheses are known to release metal debris. Basic science studies suggest that metal implants induce a pro-inflammatory response that ultimately chemoattracts leukocytes including macrophages and neutrophils to the surgical site. This raises concern of higher risk of infection with these prostheses through the "trojan horse" mechanism by which neutrophils and macrophages transport intracellular pathogens from a remote site. This study compared the infection occurrence between MoP and ceramic-on-polyethylene (CoP) implants to determine

[Measurement of chromium and cobalt ions in metal-on-metal total hip arthroplasty].

INTRODUCTION: New metal-on-metal hip replacement models emerged in the 1990 with modifications of the previous designs and alloys. This led to expect a lesser rate of ion and particle release in the body with the resulting decrease in wear, osteolysis and loosening. OBJECTIVE: To measure blood and urine chromium and cobalt ions in a 28 mm metal-on-metal tribology during the first postoperative year. MATERIAL AND METHODS: Blood and urine cobalt and chromium concentrations were measured in 10 patients (13 prostheses) with the atomic absorption method. The etiology was studied and they were fun

Cementless metal-on-metal versus ceramic-on-polyethylene hip arthroplasty in under-50 year-olds with 20 to 22 years' follow-up: Was it a good idea to abandon the small-diameter metal-on-metal bearing?

In 2005 and 2011, we reported results for a comparative study of two types of 28-mm bearing (metal-on-metal (MoM) and ceramic-on-polyethylene (CoP) in active patients with respectively 5 and 12 years' follow-up. The present report is an update at a mean 20 years' follow-up, addressing 2 issues: (1) implant survival at a mean 20 years; and (2) long-term complications with the metal-on-metal bearing. Twenty-eight millimeters MoM bearings show good survival at 20 years in young active subjects. Two groups of cementless hip prostheses were compared: Metasul™ 28mm MoM versus 28mm CoP. The MoM group

Fracture of a triple-polished tapered cemented femoral stem at the Morse taper after metal-on-metal primary total hip arthroplasty.

Total hip arthroplasty, one of the most successful orthopaedic procedures, is influenced by several variables. Corrosion at the modular junction is known as trunnionosis. Despite being reported infrequently, corrosion between the femoral head and the Morse taper can result in severe complications. Fracture of the femoral component in primary metal-on-metal (MoM) total hip arthroplasty at the Morse taper is an extremely rare event and can be associated with several risk factors. We report a case of corrosion at the Morse taper in a hybrid primary MoM total hip arthroplasty, resulting in Morse t

Molecular analysis of HIF activation as a potential biomarker for adverse reaction to metal debris (ARMD) in tissue and blood samples.

We aimed to find a biomarker for patients with adverse reaction to metal debris (ARMD) due to a metal-on-metal (MoM) hip implant. First, we compared molecular markers of hypoxia-inducible factor (HIF) pathway activation (BNIP3, GLUT1, HO1, VEGF, and HIF1A) and inflammatory response (IL1B and COX2) in tissue from patients undergoing revision of MoM hip implant with tissue from patients undergoing primary hip replacement (PHR). Second, we compared blood levels of the above molecular markers and additional inflammatory markers: TNFA, IL18, CASPASE1, NFKB or IKB, and TLR1-4 mRNA in patients with n

Localising ingested metallic foreign bodies to guide surgical planning: A novel use for the CT metal artefact reduction algorithm.

We present the case of a 20-year-old female patient who presented following ingestion of multiple button magnets. She remained clinically well however serial abdominal radiographs demonstrated the magnets were not passing through the gastrointestinal tract and a CT was, therefore, performed for further assessment and to aid surgical planning. Artefact from the magnets made interpretation of the CT challenging. The use of a Metal Artefact Reduction (MAR) algorithm, however, enabled accurate localisation of the magnets thus guiding subsequent surgical intervention. Whilst MAR algorithms are usua

Metal ions as inflammatory initiators of osteolysis.

Osteolysis and aseptic loosening currently contribute 75 % of implant failures. Furthermore, with over four million joint replacements projected to be performed in the United States annually, osteolysis and aseptic loosening may continue to pose a significant morbidity. This paper reviews the osteolysis cascade leading to osteoclast activation and bone resorption at the biochemical level. Additionally, the metal ion release mechanism from metallic implants is elucidated. Even though metal ions are not the predominating initiator of osteolysis, they do increase the concentration of key inflamma

Ultrasound findings in asymptomatic patients with modular metal on metal total hip arthroplasty.

OBJECTIVE: The use of metal-on-metal and modular total hip arthroplasty is associated with potentially serious local and systemic complications. The primary aim of this study was to identify the prevalence of a pseudotumor in asymptomatic patients with a particular metal-on-metal hip prosthesis after a minimum follow-up of 5 years using ultrasound evaluation. A secondary purpose was to identify associations between the presence of pseudotumor and serum metal ion levels following implantation. METHODS: We prospectively evaluated data collected from 36 asymptomatic patients who underwent implan

Levels of Cr, Co, Ni and Mo in erythrocytes, serum and urine after hip resurfacing arthroplasty.

BACKGROUND: Hip resurfacing arthroplasty is known to increase the metal ion concentration in the serum, urine and whole blood, with potentially adverse effects on the organism. However, only few data are available about the metal ion concentrations in erythrocytes, although they are directly exposed to the higher concentrations of the serum. METHODS: The ion levels of chromium, cobalt, nickel and molybdenum in erythrocytes of 25 patients with a hip resurfacing implant were analysed with high resolution ICP-sf-MS (inductively-coupled-plasma-sector-field-mass-spectrometry). The results were com

Evaluation of metallic osseous implants with nuclear medicine.

Nuclear medicine has proven to have a valuable role in the evaluation of osseous metallic implants, particularly with joint prostheses, but can assist with evaluation of other appliances as well. The nuclear arthrogram has become an invaluable adjunct to simultaneously performed radiographic contrast arthrography. This application has been best evaluated in what is one of the most common of orthopedic prosthesis problems, namely, loosening of total hip prostheses. Experience indicates that both sensitivity and specificity of loosening of the femoral component can be increased to over 90% throu

The taper disaster--how could it happen?

Corrosion of metallic implants in contact with body fluids is unavoidable, especially at interfaces where movement occurs or in gaps. Corrosion became clinically relevant with the introduction of large modular metal-on-metal total hip joint articulations (MoM THA) early in the 21st century. This review attempts to summarise the scientific knowledge about taper problems available at the time of introduction of these bearings, why this "disaster" could happen. It is speculated that changes to the taper connection made in the 1990s to increase the range of motion with small heads (28 and 32 mm) r

Clinical Outcomes and Survivorship After Conversion of Metal-on-Metal Hip Resurfacing to Total Hip Arthroplasty: A Systematic Review.

BACKGROUND: Hip resurfacing arthroplasty was developed for younger active patients to preserve bone stock and facilitate future revision; however, failure requiring conversion to total hip arthroplasty (THA) remains a recognized problem. Outcomes after conversion are variable and influenced by indication and technical factors. This systematic review evaluates survivorship, complications, and functional outcomes after conversion of hip resurfacing to THA. METHODS: A systematic review was performed according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and re

Revision of metal-on-metal hip replacements and resurfacings for adverse reaction to metal debris: a systematic review of outcomes.

PURPOSE: This systematic review assessed evidence on outcomes following revision of MoM hip resurfacings (HRs) and total hip replacements (THRs) for adverse reaction to metal debris (ARMD). METHODS: Four electronic databases were searched between January 2009 and July 2013 to identify studies reporting clinical outcomes following revision of MoM HRs and THRs for ARMD. Only studies reporting cohorts with more than 10 metal-on-metal (MoM) hips revised for ARMD were included. Outcomes of interest following ARMD revision were: (1) complication rates; (2) re-revision rates; (3) surgical interventi

Imaging of elbow replacement arthroplasty.

Elbow replacement arthroplasty has become a standard surgical treatment for a variety of diseases of the elbow. First popularized for the treatment of late-stage rheumatoid arthritis and other debilitating forms of joint disease, the current indications have expanded to include primary treatment of elbow trauma. The most commonly used total elbow replacements are linked semiconstrained chromium-molybdenum alloy or titanium alloy prostheses with polyethylene bearing surfaces. These are inserted after resection of the ulnotrochlear joint and typically cemented in place; the radial head is often
