Does Hallux Valgus Exhibit a Deformity Inherent to the First Metatarsal Bone?

Hallux valgus is a common condition, and it still poses some challenges. The identification of factors associated with the development of the deformity is of paramount importance in obtaining a full correction of the disorder. Hallux pronation is one of the frequently found components, especially in larger deformities, but the cause and exact location of this condition are not fully understood. The aim of the present study was to investigate whether there is a rotational deformity inherent to the first metatarsal bone. A case-control study was conducted on patients with and without hallux valg

Association of Metatarsalgia After Hallux Valgus Correction With Relative First Metatarsal Length.

BACKGROUND: Metatarsalgia is frequently associated with hallux valgus. The aim of this study was to evaluate how the relative length and position of the first metatarsal head influenced metatarsalgia and plantar callosities beneath the lesser metatarsal heads. METHODS: A retrospective analysis of the clinical data and radiographs of 102 cases was performed at a mean follow-up of 16 months after biplane interlocking osteotomies. Clinical evaluation was made using the Japanese Society for Surgery of the Foot (JSSF) hallux scale. Radiologic evaluation was made with standard weight-bearing antero

Hallux valgus and the first metatarsal arch segment: a theoretical biomechanical perspective.

Hallux valgus is a progressive foot deformity characterized by a lateral deviation of the hallux with corresponding medial deviation of the first metatarsal. Late-stage changes may render the hallux painful and without functional utility, leading to impaired gait. Various environmental, genetic, and anatomical predispositions have been suggested, but the exact cause of hallux valgus is unknown. Evidence indicates that conservative intervention for hallux valgus provides relief from symptoms but does not reverse deformity. Part 1 of this perspective article reviews the literature describing the

Lengthening of the shortened first metatarsal after Wilson's osteotomy for hallux valgus.

Metatarsalgia is a recognised complication following iatrogenic shortening of the first metatarsal in the management of hallux valgus. The traditional surgical treatment is by shortening osteotomies of the lesser metatarsals. We describe the results of lengthening of iatrogenic first brachymetatarsia in 16 females. A Scarf-type osteotomy was used in the first four cases and a step-cut of equal thicknesses along the axis of the first metatarsal was performed in the others. The mean follow-up was 21 months (19 to 26). Relief of metatarsalgia was obtained in the six patients in whom 10 mm of leng

Chevron or Wilson metatarsal osteotomy for hallux valgus. A prospective randomised trial.

We compared the chevron and the Wilson metatarsal osteotomy for hallux valgus in a prospective randomised trial on 87 feet in 51 patients, reviewed at averages of 5.5 and 38 months after operation. The patients in the chevron group returned to work earlier and mobilised faster, but, at the later review, those in the Wilson group had better functional results and were more satisfied with the appearance of the foot. Correction of the hallux valgus angle was better maintained in patients in the Wilson group and they had a better range of motion at the metatarsophalangeal joint; fewer complained o

First Metatarsal Hemiepiphysiodesis for the Treatment of Juvenile Hallux Valgus: A Systematic Review.

INTRODUCTION: Juvenile hallux valgus (JHV) is a pediatric deformity characterized by the varus deviation of the first metatarsal and valgus deviation of the proximal phalanx. Among the several surgical techniques available, hemiepiphysiodesis consists of the unilateral growth arrest of the first metatarsal physis. Despite this technique has been proposed over 70 years ago, only a few studies including clinical and radiological outcomes have been published, making the procedure unclear in terms of results. This systematic review aimed to evaluate the outcomes of hemiepiphysiodesis of the first

The Use of a Triplanar Metatarsal Rotational Osteotomy to Correct Hallux Valgus Deformities.

Proximal rotational metatarsal osteotomy (PROMO) is a technique that allows the surgeon to correct the varus and pronation of the first metatarsal that is observed in most patients with hallux valgus deformity. Persistent metatarsal pronation is a recognized recurrence factor for operatively treated hallux valgus. The indication for this technique is a mild-moderate hallux valgus deformity (i.e., intermetatarsal angle <18° and hallux valgus angle <40°) in which pronation is present. (Note that according to the literature, 10% to 20% of patients do not have pronation.) The PROMO is performed vi

Proximal opening-wedge osteotomy of the first metatarsal for correction of hallux valgus.

Osteotomy of the proximal metatarsal in combination with a distal soft tissue procedure for the correction of moderate to severe hallux valgus deformity is commonly performed. All described techniques have complications such as non-union and malunion, and many are extremely technically demanding. The purpose of this study is to review the results of a novel technique for the correction of hallux valgus, an opening-wedge osteotomy of the proximal first metatarsal with plate fixation. A review was performed of the results of 23 patients who underwent correction of hallux valgus with proximal met

[The results of treatment of hallux valgus with subcapital osteotomy of the first metatarsal bone].

This paper presents results of wedge subcapital osteotomy of the first metatarsal bone with lateral and plantar translation of the distal fragment in 47 feet (44 women age 37-72) operated for hallux valgus deformity. Bone fragments were stabilized with Kirschner wire introduced obliquely from lateral side of the head to the shaft. Protruding out of the skin part of the wire between I and II toe was shaped parallel to the lateral side of hallux preventing from valgisation. The mean AOFAS score improved from the preoperative value of 49 points to 86 points postoperatively. Hallux valgus angle im

Metatarsophalangeal fusion for hallux valgus: indications and effect on the first metatarsal ray.

Arthrodesis of metatarsophalangeal joint was performed in 31 cases of severe hallux valgus. With the surgical technique used, which is described in detail, internal fixation and plaster cast immobilization were not necessary. Analysis of the results suggested that the operation is indicated for severe deformities in elderly patients, preferably women. The basic metatarsal deformity was corrected by the procedure.

Correction of Severe Hallux Valgus Deformity Using a Percutaneous Metatarsal Distal Osteotomy.

BACKGROUND: Severe hallux valgus (HV) presents a significant surgical challenge. Traditional methods are being questioned because of their invasiveness, whereas recent minimally invasive techniques raise concerns because of their associated complications. This study evaluates the percutaneous distal metatarsal osteotomy technique, generally found to be effective for mild-to-moderate cases, and tests the hypothesis that it provides effective clinical and radiologic correction for severe deformities. METHODS: This retrospective study analyzed 116 feet that underwent percutaneous distal metatars

First metatarsal length change after basilar closing wedge osteotomy for hallux valgus.

BACKGROUND: Hallux valgus deformities with large intermetatarsal angles require a more proximal metatarsal procedure to adequately correct the deformity. Due to the relative ease of a closing wedge osteotomy, this technique was adopted but with concern over first metatarsal shortening. In this study, we primarily evaluated angular correction and first metatarsal shortening. MATERIALS AND METHODS: We evaluated 70 feet in 57 patients (average age, 54 years) with 52 female and five male. The average followup was 14 (range, 6 to 45) months. The charts were reviewed for the presence of metatarsalg

Minimally invasive distal metatarsal osteotomy for mild-to-moderate hallux valgus deformity.

Minimally invasive surgery has recently been introduced for foot and ankle surgery, and hallux valgus surgery is no exception. The purpose of our study was to analyze the early results and to present our experience of minimally invasive distal metatarsal osteotomy in correcting mild-to-moderate hallux valgus deformities. Between September 2005 and December 2006, 31 consecutive patients (47 feet) with mild-to-moderate hallux valgus deformities underwent minimally invasive distal metatarsal osteotomies. The clinical and radiographic outcomes were assessed. The satisfaction rate was 90.32%. The m

Role of medical capsule and transverse metatarsal ligament in hallux valgus deformity.

The role of the medial capsule and transverse metatarsal ligament in hallux valgus deformity including stability of the first metatarsophalangeal and adjacent joints was investigated in vitro. The three-dimensional positions of the proximal phalanx, first metatarsal, and second metatarsal before and after sectioning the medial capsule and metatarsal ligament were measured using a magnetic tracking system. Valgus deformity of the hallux increased with medial capsule sectioning an average of 22.3 degrees +/- 6 degrees. Valgus deformity of the hallux increased with medial capsule and metatarsal l

[Treatment of hallux valgus. Distal soft-tissue procedures and proximal metatarsal osteotomy].

Correction of the hallux valgus deformity is dependent upon recognition of the specific anatomic deformity, followed by selection of a procedure that addresses the specific deformity. A hallux valgus deformity with a subluxated metatarsophalangeal joint has a soft tissue and bony component, both of which need to be corrected, or a recurrence may result. The distal soft tissue procedure addresses the lateral soft tissue contracture by releasing the adductor hallucis, lateral joint capsule and transverse metatarsal ligament. The capsule is medially elongated and the medial eminence needs to be e
