Providing an acceptable quality of life for patients after reconstruction of mandibular segmental defects is challenging because the surgical techniques available have limitations. The use of methylmetacrylates has not been widely accepted for the repair of frontal bony defects due to the possibility of secondary infection. The development of a new type of methylmetacrylates into which antibiotics, such as gentamicin, are incorporated has reduced the risk of secondary infection. The surgical technique and the results are illustrated on three patients. Favourable results were obtained in small and in large bony defects and even after previous frontal osteomyelitis.

We describe a 73-year-old woman who had a right atrial-inferior vena caval thrombus and pulmonary thromboembolism develop after percutaneous vertebroplasty with methylmethacrylate. Our patient subsequently underwent open-heart surgery to effectively remove the bulk of the foreign material. This case illustrates the need for close monitoring of patients undergoing percutaneous vertebroplasty and emphasizes the importance of prompt diagnosis and treatment.

A 59-year-old woman developed a histiocytic response to methylmethacrylate debris in a loose total hip arthroplasty. This reaction eroded through the medial wall of the acetabulum, producing a pelvic mass and presenting with urinary symptoms. A combination of an abdominal approach for removal of the pelvic mass and a lateral approach for revision of the total hip arthroplasty was required. This condition should be considered in the differential diagnosis of a mass adjacent to a loose prosthesis.

It is not uncommon to observe bone cement in the pelvis on radiographs after total hip replacement, a finding which is generally considered to be benign. This paper reviews some catastrophic late complications from intrapelvic methylmethacrylate. We also describe a case of progressive, unbearable dyspareunia beginning three years after total hip replacement. A possible explanation of the pathophysiology is suggested. Recommendations for prevention, diagnosis, and treatment of these late complications are offered.

BACKGROUND: Surgical implantation of the intraocular sustained-release ganciclovir device is a safe and effective treatment for cytomegalovirus (CMV) retinitis. Failure of total hip arthroplasty with central migration of prosthetic components is uncommon. Various pelvis and visceral complications have been reported from intrapelvic migration of the acetabular cup or the cement or from the heat generated by methylmethacrylate polymerization. To our knowledge, we are the first to report intrapelvic migration of a femoral stem causing ipsilateral lower-extremity swelling, pressure sores, and severe bowel symptoms after the removal of the acetabular component.

Unexplained septic shock was ultimately shown to be caused by Candida mediastinitis after perforation of the cervical esophagus by a dislodged cervical methylmethacrylate construct in a 25-year-old patient with traumatic quadriplegia. Communication between the prevertebral abscess and pleural space further led to the formation of a esophageal-pleural-cervical fistula. Despite antibiotics, surgical removal of the construct, and drainage of the esophagus and mediastinum, the patient died from refractory shock and respiratory failure.

Phosphaturic mesenchymal tumour is an extremely rare mesenchymal neoplasm of bone or soft tissue. It is associated with paraneoplastic oncogenic osteomalacia through secretion of fibroblast growth factor-23 (FGF-23) which inhibits renal proximal tubule phosphate re-uptake. We report a case of a 36-year-old woman with a large chest wall tumour completely excised by extensive chest wall resection and reconstruction with polypropylene mesh-methylmethacrylate composite and pedicled muscle flaps.

We report on 14 patients with instability of the cervical spine secondary to rheumatoid arthritis treated by fusion using a combination of traditional bone grafting techniques and methylmethacrylate bone cement. Successful fusion was achieved in all cases with few complications. The high infection rate reported by other authors was avoided by reducing the bulk of the cement mass and the use of antibiotic impregnated cement. We recommend this method of fusion to all orthopaedic surgeons involved in the treatment of patients with rheumatoid arthritis.

A variety of agents have been used to coat aneurysms as an alternative to clipping.
