40 years of medical mycology at JAC.

In the past 40 years, medical mycology has gone from a curiosity in the basements of medical schools to a mainstream branch of clinical microbiology and infectious diseases. Long gone are the days of carefully curated collections of organisms identified purely based on morphology and skill, the lack of therapeutic interventions beyond amphotericin B and the occasional strange case in the ward of a diabetic patient with mucormycosis. We highlight advances in medical mycology as reflected in the past 40 years of JAC.

Methods for the isolation and identification of Listeria spp. and Listeria monocytogenes: a review.

Listeria monocytogenes is an important food-borne pathogen and is widely tested for in food, environmental and clinical samples. Identification traditionally involved culture methods based on selective enrichment and plating followed by the characterization of Listeria spp. based on colony morphology, sugar fermentation and haemolytic properties. These methods are the gold standard; but they are lengthy and may not be suitable for testing of foods with short shelf lives. As a result more rapid tests were developed based on antibodies (ELISA) or molecular techniques (PCR or DNA hybridization).

A Rare Case of Primary Supraclavicular Lymphadenitis due to Cryptococcus Neoformans in an HIV Infected Patient.

Cryptococcosis caused by encapsulated yeast Cryptococcus neoformans most commonly presents as disease of the central nervous system. Cryptococcus is a non-mycelial budding yeast found in soil, pigeon droppings and their nesting places. The three 'classic' virulence factors of cryptococci are: polysaccharide capsule, melanin production and growth at 37°C. Here, we present a rare case of cryptococcosis affecting left supraclavicular lymph node in a Human Immunodeficiency virus (HIV) infected individual. Culture of fine needle aspirate of the lymph node yielded Cryptococcus neoformans which was i

Variables influencing extraction of nucleic acids from microbial plankton (viruses, bacteria, and protists) collected on nanoporous aluminum oxide filters.

Anodic aluminum oxide (AAO) filters have high porosity and can be manufactured with a pore size that is small enough to quantitatively capture viruses. These properties make the filters potentially useful for harvesting total microbial communities from water samples for molecular analyses, but their performance for nucleic acid extraction has not been systematically or quantitatively evaluated. In this study, we characterized the flux of water through commercially produced nanoporous (0.02 μm) AAO filters (Anotop; Whatman) and used isolates (a virus, a bacterium, and a protist) and natural sea

[The explicative power of the rules: the case of the Koch's postulates].

This article traces the emergence of the standard for evidence of causation in infectious diseases, from the scientific discoveries on contagion by Agostino Bassi, in the first half of the nineteenth century. However, only after the work of Robert Koch on anthrax and tuberculosis, a set of practical guidelines (Koch's postulates) were acknowledged, to establish rigorous criteria before a microbiologist could claim a strictly causal relationship between microbes and infectious diseases. The methodological and epistemological meaning of these guidelines, and their limits, are discussed in the li

Myths and facts: What is the evidence for routine environmental sampling---A commentary.

BACKGROUND: Before 1970, US hospitals conducted scheduled culturing of the air and environmental surfaces. Currently, routine environmental culturing is not recommended. Targeted microbiologic sampling is only recommended for research purposes, outbreak investigations, and when there are validated culture methods and action level based on culture results. METHODS: The paper is a commentary. The literature was reviewed to find published guidelines that provide recommendations for appropriate microbiologic sampling of the hospital environment based on validated culture methods and provide actio

Factors Impacting the Yield of Image-Guided Biopsy in Native Vertebral Osteomyelitis: A 10-Year Retrospective Study.

BACKGROUND: Image-guided biopsies in patients with suspected native vertebral osteomyelitis (NVO) are recommended to establish the microbiological diagnosis and guide antibiotic therapy. Despite recent advances, the microbiological yield of this procedure remains between 48% and 52%. A better understanding of factors associated with this low yield may lead to improved microbiological diagnosis. METHODS: We retrospectively identified patients with suspected NVO undergoing image-guided biopsies from January 2011 to June 2021 at our institution. Two hundred nine patients undergoing 248 percutane

Rapid detection, characterization, and enumeration of foodborne pathogens.

As food safety management further develops, microbiological testing will continue to play an important role in assessing whether Food Safety Objectives are achieved. However, traditional microbiological culture-based methods are limited, particularly in their ability to provide timely data. The present review discusses the reasons for the increasing interest in rapid methods, current developments in the field, the research needs, and the future trends. The advent of biotechnology has introduced new technologies that led to the emergence of rapid diagnostic methods and altered food testing prac

Sensitivity-enhanced hydrogel digital RT-LAMP with in situ enrichment and interfacial reaction for norovirus quantification in food and water.

Low levels of human norovirus (HuNoV) in food and environment present challenges for nucleic acid detection. This study reported an evaporation-enhanced hydrogel digital reverse transcription loop-mediated isothermal amplification (HD RT-LAMP) with interfacial enzymatic reaction for sensitive HuNoV quantification in food and water. By drying samples on a chamber array chip, HuNoV particles were enriched in situ. The interfacial amplification of nucleic acid at the hydrogel-chip interface was triggered after coating HD RT-LAMP system. Nanoconfined spaces in hydrogels provided a simple and rapid

Massive Sequencing of V3-V4 Hypervariable Region in Pyogenic Liver Abscesses Reveals the Presence of Unusual Bacteria Not Detected by Classical Culture Methods.

Pyogenic liver abscesses (PLAs) are serious infections in which doctors often fail in identifying the causative agent due to microbiological limitations. These limitations in detecting uncommon pathogens complicate the treatment and recovery. Molecular techniques, like massive sequencing, enable the detection of uncommon pathogens and highlight the shortcomings of traditional cultures. The aim of this work was to characterise the bacterial composition of PLAs through massive sequencing of the V3-V4 hypervariable region of the 16S rRNA gene in cases where conventional culture methods were negat

Comparison of curet and paper point sampling of subgingival bacteria as analyzed by real-time polymerase chain reaction.

BACKGROUND: The outcome of microbiological diagnostics may depend on the sampling technique. It was the aim of the present study to compare two widely used sampling techniques for subgingival bacteria using quantitative real-time polymerase chain reaction. METHODS: Twenty patients with chronic periodontitis were randomized into two groups. In group A, samples were taken first with a paper point and then with a curet at the same site (single-rooted teeth with probing depth >5 mm) before scaling and root planing and after 10 weeks. The sampling sequence was reversed in group B. The analysis ena

A Pilot Feasibility Study in Establishing the Role of Ultrasound-Guided Pleural Biopsies in Pleural Infection (The AUDIO Study).

BACKGROUND: Pleural infection is a common complication of pneumonia associated with high mortality and poor clinical outcome. Treatment of pleural infection relies on the use of broad-spectrum antibiotics because reliable pathogen identification occurs infrequently. We performed a feasibility interventional clinical study assessing the safety and significance of ultrasound (US)-guided pleural biopsy culture to increase microbiological yield. In an exploratory investigation, the 16S ribosomal RNA technique was applied to assess its utility on increasing speed and accuracy vs standard microbiolo

Culture-Based Isolation of Salmonella from Different Matrices.

While Salmonella is a leading bacterial cause of foodborne illness, its relative abundance in food, animals, and the environment is low. This necessitates the use of cultural enrichment to reduce the background flora and enable Salmonella to proliferate so that it can be subsequently isolated on agar or detected via molecular methods. Further, successful Salmonella isolation is often dependent on the sample type, or matrix, from which it is being isolated. Particularly where matrices have high microbial background, or high levels of organic material, attention should be paid to culture methods

Environmental Microbial Forensics and Archaeology of Past Pandemics.

The development of paleomicrobiology with new molecular techniques such as metagenomics is revolutionizing our knowledge of microbial evolution in human history. The study of microbial agents that are concomitantly active in the same biological environment makes it possible to obtain a picture of the complex interrelations among the different pathogens and gives us the perspective to understand the microecosystem of ancient times. This research acts as a bridge between disciplines such as archaeology, biology, and medicine, and the development of paleomicrobiology forces archaeology to broaden

An Overview of the Skin Microbiome, the Potential for Pathogen Shift, and Dysbiosis in Common Skin Pathologies.

Recent interest in the diverse ecosystem of bacteria, fungi, parasites, and viruses that make up the skin microbiome has led to several studies investigating the microbiome in healthy skin and in a variety of dermatological conditions. An imbalance of the normal skin flora can cause some skin diseases, and current culture techniques are often unable to detect a microorganism to further our understanding of the clinical-microbiological correlates of disease and dysbiosis. Atopic dermatitis and rosacea are presentations that GPs often manage that may have an infective or microbiological componen

[Microbiological diagnosis of emerging bacterial pathogens: Anaplasma, Bartonella, Rickettsia, and Tropheryma whipplei].

Ehrlichia/Anaplasma, Bartonella, Rickettsia and Tropheryma whipplei (formerly called whippelii) are fastidious bacterial organisms, considered the causative agents of potentially severe emerging and re-emerging diseases with repercussions on public health. The recent availability of advanced molecular biology and cell culture techniques has led to the implication of many of these species in human pathologies. These issues are extensively covered in number 27 of the SEIMC microbiological procedure: Diagnóstico microbiológico de las infecciones por patógenos bacterianos emergentes: Anaplasma, Ba
