Sepsis and the microcirculation: the impact on outcomes.

PURPOSE OF REVIEW: Advances in the treatment of septic shock have historically focused on resuscitation endpoints, mainly mean arterial pressure and cardiac output. As the definitions of sepsis and septic shock have shifted to focus on the diversity of causes of dysregulated host-response we have seen an emerging phenotype where tissue hypoxia persists despite adequate macrocirculatory parameters. Interest in the topic of microcirculation is re-emerging as validated bedside techniques for hemodynamic monitoring, such as video microscopes, are becoming available. We review the current understan

Fluid therapy and the hypovolemic microcirculation.

PURPOSE OF REVIEW: In shock states, optimizing intravascular volume is crucial to promote an adequate oxygen delivery to the tissues. Our current practice in fluid management pivots on the Frank-Starling law of the heart, and the effects of fluids are measured according to the induced changes on stroke volume. The purpose of this review is to evaluate the boundaries of current macrohemodynamic approach to fluid administration, and to introduce the microcirculatory integration as a fundamental part of tissue perfusion monitoring. RECENT FINDINGS: Macrocirculatory changes induced by volume expa

Regulation of the human coronary microcirculation.

Atherosclerosis of conduit epicardial arteries is the principal culprit behind the complications of coronary heart disease, but a growing body of literature indicates that the coronary microcirculation also contributes substantially to the pathophysiology of cardiovascular disease. An understanding of mechanisms regulating microvascular function in humans is an essential foundation for understanding the role in disease, especially since these regulatory mechanisms vary substantially across species and vascular beds. In fact all subjects whose coronary tissue was used in the studies described h

Effect of surgical traumas on microcirculation.

BACKGROUND: Adequate microcirculation in different tissues maintains the physiological function and heals surgical wounds. In any surgical procedure, the commonly used instruments are cautery, tissue forceps, and clamps. The fact that their inappropriate use produces an adverse effect on microcirculation is often not realized. By this study, we could demonstrate live, the effect of these surgical traumas. METHODS: The study was conducted on the inferiorly based fasciocutaneous flap with a fascial extension in patients with a distal leg defect. The extended fascial flap was mounted on a glass

[Is arterial hypertension a microcirculation disease?].

Capillary and arterioles rarefaction has been reported in animal models of hypertension and in essential and borderline hypertension. Moreover, young adults with a predisposition to hypertension may exhibit microvascular rarefaction suggesting that defective angiogenesis-arteriogenesis may be an etiological component of high blood pressure. Nevertheless, in other studies, VEGF plasma levels are increased in hypertensive subjects and low birth weight predicts higher blood pressure but not dermal capillary density. In fibrin gel chambers implanted in rats, angiogenesis-arteriogenesis are increas

[Importance of microcirculation in plastic surgery].

The article summarizes distinct microcirculatory models for use in surgical research with a special interest to plastic surgery. Methods for the quantitative analysis of the microcirculation in burns, flaps and wounds are presented. Vascularization of biomaterials can be observed by means of the dorsal skinfold chamber model in hamsters and mice. Developing capillary sprouts can be assessed by means of videomicroscopy and angiogenetic drugs can be tested using this model. Hairless mice allow for direct, long-term observation of the microcirculation in burns as well as during healing of dermal

The peritoneal microcirculation in peritoneal dialysis.

This paper deals with the peritoneal microcirculation and with peritoneal exchange occurring in peritoneal dialysis (PD). The capillary wall is a major barrier to solute and water exchange across the peritoneal membrane. There is a bimodal size-selectivity of solute transport between blood and the peritoneal cavity, through pores of radius approximately 40-50 A as well as through a very low number of large pores of radius approximately 250 A. Furthermore, during glucose-induced osmosis during PD, nearly 40% of the total osmotic water flow occurs through molecular water channels, termed "aquapo

Microcirculation and hemorheology in NIDDM patients.

The authors studied 10 patients with non-insulin-dependent diabetes mellitus and 5 controls matched for age, sex, blood lipids, and smoking habit. The two groups were also comparable for hemorheologic characteristics as evaluated by viscosimetry on whole blood, plasma and serum, erythrocyte filtration and aggregation. The microcirculation was studied in the subjects of both groups by microalbuminuria determination, retinal fluorangiography, and capillaroscopic examination of the bulbar conjunctive and nail folds. None of the patients presented microalbuminuria values higher than the upper limi

[A study on cutaneous microcirculation in the human].

From the research on diseased tissues of the skin, we found that there are evidences of microcirculatory damages at the following sites: (1) Arterial limbs; (2) Venule limbs (3) All Loops (4) Top of Loops (5) Skin Papilla. On the other hand. We have observed such atypical loops, under microcirculatory microscope and recording color TV, as: (1) big fishhook-like (2) dumb-bell like (3) glomerular (4) hemorrhagic (5) vascular (6) big tadpole-like (7) net-like (8) papillar edema. For clinical diagnosis and treatment, studies on cutaneous microcirculation may prove to be helpful.

Tumour microcirculation as a target for hyperthermia.

A great number of investigators have, independently, shown that tumour blood flow is affected by a hyperthermic treatment to a larger extent than normal tissue blood flow. While the majority of the studies on experimental tumours show a decrease and even a lapse in blood flow within the microcirculation during or after hyperthermia, the data on human tumours are less conclusive. Some of the investigators do not find a decrease in circulation, while others do. Obviously, this is an important field of investigation in the clinical application of hyperthermia because a shut down of the circulatio

Effect of glycerol on cochlea microcirculation.

Microcirculation in the cochlea of l2 normal anesthetized guinea pigs was investigated after intravenous infusion of 2.4 ml/kg of 40% glycerol. In these experiments endolymphatic PO2, blood pressure, cochlear microphonics, and endocochlear potentials were recorded. Administration of glycerol caused a significant increase in endocochlear PO2 for about 30 minutes. The improvement of hearing in patients with Meniere's disease is thus not only a result of the reduced hydrostatic pressure, as has been demonstrated by others, but may also be the result of vasodilatation, which improves microcirculat

Histamine receptors in the gastric microcirculation.

The types and functions of histamine receptors in the submucosal arterioles of the corpus and antrum of the cat and rat stomach were studied using an in vivo microscopy technique. Change in arteriolar diameter in response to superfusion of histamine with and without antagonists was measured by an image-splitting technique. H1 and H2 histamine receptors subserving vasodilatation were demonstrated in both the antral and corpus submucosal arterioles of the cat and rat. However, the H1 effect was predominant in the antrum (the H2 antagonist inhibited histamine dilatation only in the presence of th

Conjunctival microcirculation is associated with cerebral cortex microcirculation in post-resuscitation mild hypothermia: A rat model.

OBJECTIVE: This study aimed to compare the changes in sublingual and conjunctival microcirculation occurring with cerebral cortex microcirculation changes during mild hypothermia in a rat model of cardiac arrest. METHODS: Twenty-four rats were randomized into mild hypothermia (M) or normothermia (C) groups. Ventricular fibrillation was electrically induced and left untreated for 8 minutes, followed by 8 minutes of cardiopulmonary resuscitation. The core temperature in group M reduced to 33 ± 0.5°C at 13 minutes after restoration of spontaneous circulation and was maintained for 8 hours. In gr

CHANGES IN SUBLINGUAL MICROCIRCULATION IS CLOSELY RELATED WITH THAT OF BULBAR CONJUNCTIVAL MICROCIRCULATION IN A RAT MODEL OF CARDIAC ARREST.

Following successful resuscitation, a significantly impaired microcirculation has been identified. The severity of the impairment of microcirculation is closely related to that of vital organ dysfunction. Sublingual microcirculation is a traditional site for the measurement of tissue perfusion. In the present study, we investigated the bulbar conjunctival microcirculatory alterations following CPR and its relationship with the changes of sublingual microcirculation in a rat model of cardiac arrest.Male Sprague-Dawley rats (450-550 g) were utilized. Ventricular fibrillation was induced and untr

Haemodialysis-Induced Stress Influences Ocular Microcirculation.

Background: Haemodialysis (HD) superimposes additional circulatory stress on the microvasculature, leading to endothelial dysfunction, which plays a key role in the development of haemodialysis-associated multiorgan dysfunction. This study was undertaken to evaluate the effect of a single HD session on retinal and choroidal microcirculation, using optical coherence tomography angiography (OCTA) in relation to changes in the blood levels of selected biochemical modulators of endothelial function. Methods: The vessel density (VD) of 35 patients was evaluated before and after a single HD session,

Tetrahydrobiopterin improves microcirculation in experimental sepsis.

BACKGROUND: Tetrahydrobiopterin (BH4), an endogenous nucleic acid derivative, acts as an important cofactor for several enzymes found within the vascular endothelium, which is deranged in sepsis. OBJECTIVE: We hypothesized that BH4 would improve capillary density and decrease inflammation within the intestinal microcirculation of septic rats. METHODS: We conducted a randomized, controlled trial using two previously validated models of sepsis in rats: 1) A fecal peritonitis model using a stent perforating the ascending colon, and 2) An endotoxemia model using lipopolysaccharide (LPS) toxin fr

[Hypertensive vascular disease and cerebral microcirculation].

Cerebral microcirculation has a series of complex relationships with arterial hypertension determined, on one hand, by the size and the location of the vessels involved, and on the other hand, by the chronic or acute nature of the hypertension. The small arterial vessels of the cerebral parenchyma react to the effects of chronic hypertension with irreversible structural changes, whose pathologic and radiological correlation is chronic ischemia of the white substance, shown by paleness of the white substance, together with small lacunar infarctions, with a clinical association of dementia, moto

[Disordered peripapillary microcirculation in glaucoma patients].

To study the haemodynamics of the peripapillary choroid, perfusion pressure videoangiography (PVA) was performed in six healthy subjects and in seven patients with primary open-angle glaucoma (poag). In healthy subjects the peripapillary part of the choroid starts filling at distinctly lower ocular perfusion pressures than other parts of the choroid. The mean difference in perfusion pressure between peripapillary and perimacular filling was 13.6 mmHg in the six healthy subjects. A different filling pattern of the choroid was found in the seven glaucoma patients. The mean difference in perfusio

[Measuring microcirculation after replantation and revascularization].

Eighty-four patients were evaluated following replantation or revascularization. Extremity function in daily and professional life was altogether satisfactory. Microcirculation investigations in eight patients included thermography, laser-doppler-flowmetry and photoplethysmography. The rewarming period and postocclusive hyperemia after standardized cooling of the upper extremity were determined. In areas of hypesthesia, the rewarming period was clearly delayed. In addition, reactive hyperemia after occlusion of circulation was significantly reduced.

Ocular microcirculation. Scanning electron microscopic study.

A simple vascular casting technique utilizing a low-viscosity plastic was used to study the three-dimensional ocular microcirculation of the cat eye. Vascular arrangements in different anatomic areas of the eye, i.e., iris, ciliary body, retina, optic nerve, and choroid, were clearly elucidated by scanning electron microscopy. This modified technique is described in detail, and scanning electron micrographs are presented to illustrate the reliable results.
