Significance of wave I loss of brainstem auditory evoked potentials during microvascular decompression surgery for hemifacial spasm.

OBJECTIVE: We sought to define the significance of wave I loss of brainstem auditory evoked potentials (BAEPs) during microvascular decompression (MVD) surgery for hemifacial spasm. METHODS: Out of 670 patients, 36 showed wave V loss during MVD surgery. These patients were classified into the following two groups based on wave I loss: Total wave loss, including wave I loss, and wave V loss with persistent wave I. We analyzed the differences in postoperative complications between the groups. We also investigated when wave I loss occurred during MVD surgery. RESULTS: Of the 36 patients, 24 (66

Long-term outcomes of microvascular decompression and Gamma Knife surgery for trigeminal neuralgia: a retrospective comparison study.

BACKGROUND: There is still no clear guideline for surgical treatment for patients with medically refractory trigeminal neuralgia (TN). When it comes to which surgical treatment to choose, microvascular decompression (MVD) or Gamma Knife surgery (GKS), we should know the long-term outcome of each treatment. METHODS: We analyzed 179 patients undergoing MVD and 52 patients undergoing GKS followed for 1 year or longer. We evaluated the patient's neurological status including pain relief, complications and recurrence. Results were assessed with Barrow Neurological Institute (BNI) pain intensity an

Microvascular decompression surgery for vertebral artery compression of the medulla oblongata: 3 cases with respiratory failure and/or dysphagia.

BACKGROUND: It is well known that brainstem dysfunction may be caused by vascular compression of the medulla oblongata (MO). However, only a limited number of reports have found microvascular decompression (MVD) surgery to be an effective treatment for symptomatic patients with MO dysfunction, such as essential hypertension, pyramidal tract signs, dysphagia, and respiratory failure. CASE DESCRIPTION: This report describes 3 patients with vertebral artery compression of MO who presented with respiratory failure and/or dysphagia. MVD surgery using the transcondylar fossa approach was effective

Microvascular decompression surgery is effective for the laterocollis subtype of spasmodic torticollis: a long-term follow-up result.

BACKGROUND: Spasmodic torticollis (ST) is characterized by sustained, involuntary, and painful spasms of specific muscle (s), which results into abnormal posture of the neck and head. Although various treatments for ST have been introduced, none of them shows absolute effectiveness. Earlier research from our department showed that microvascular decompression (MVD) surgery is effective in the short-term for ST patients with confirmed accessory nerve compression. However, the long-term outcome of MVD remains unknown. METHOD: Twelve ST patients with confirmed accessory nerve compression received

Delayed complete remission of hemifacial spasms following microvascular decompression and the implications for optimal time of revision surgery.

BACKGROUND: Microvascular decompression (MVD) is an effective method for directly treating hemifacial spasms (HFS). The timing for the consideration of failed MVD and reoperation has been paradoxical. OBJECTIVE: This study aimed to investigate the delayed complete remission of HFS in terms of prevalence rate, duration between surgery and delayed complete remission, and predictive factors. METHODS: A hundred patients with HFS who underwent MVD from 2012-2021 were enrolled in the study. All HFS occurred as a result of compression of the facial nerve by adjacent blood vessels. Clinical informat

Safety and efficacy of endoscope-assisted versus microscopic microvascular decompression surgery for hemifacial spasm: a prospective cohort study.

Microscopic microvascular decompression (M-MVD) is a classical treatment for relieving long-term hemifacial spasms (HFS). An endoscopy technique has recently been introduced to improve M-MVD; however, this application remains debatable. This study compared the safety and effectiveness of endoscope-assisted microvascular decompression (EA-MVD) and M-MVD for HFS. From February 2021 to September 2022, we enrolled 49 patients with HFS assigned to the EA-MVD (n = 26) and M-MVD (n = 23) groups. The patients were assessed with Park YS grades, operative time, hospital days, and complications. Evaluati

Comparison of first-time microvascular decompression with percutaneous surgery for trigeminal neuralgia: long-term outcomes and prognostic factors.

OBJECTIVE: Common surgical treatments for trigeminal neuralgia (TN) include microvascular decompression (MVD) and percutaneous procedures (glycerol rhizolysis; thermocoagulation; and balloon compression). Although the efficacy of each procedure has been documented, direct comparisons of their relative efficacies for TN are lacking. We aimed to directly compare long-term outcomes after first-time MVD with percutaneous surgery in primary (idiopathic and classical) TN and identify predictors of outcome. METHODS: We conducted a retrospective analysis of prospectively collected data on 185 patient

Diabetes and morbid obesity are associated with higher reoperation rates following microvascular decompression surgery: An ACS-NSQIP analysis.

BACKGROUND: Microvascular decompression (MVD) is the preferred treatment for refractory trigeminal neuralgia, hemifacial spasm, and glossopharyngeal neuralgia. Despite its high rate of success, MVD carries risk of complications. In this study, we examine outcomes following MVD and identify risk factors associated with adverse outcomes. METHODS: A review of the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database was performed with CPT code 61458 queried between 2007 and 2014. Demographics, preoperative comorbidities, and 30-day outcomes were analyzed

Efficacy and safety of microvascular decompression versus gamma knife surgery in patients with trigeminal neuralgia: an updated meta-analysis.

INTRODUCTION: Trigeminal neuralgia(TN) is a debilitating condition often requiring surgical intervention when medical therapy fails. Microvascular decompression (MVD) and Gamma Knife surgery (GKS) are the main treatment options, but their relative efficacy and safety remain debated. This meta-analysis aimed to compare outcomes between MVD and GKS in patients with TN. METHODS: We systematically searched PubMed, Embase, and Cochrane without temporal restrictions, in July 2025, for randomized controlled trials and observational studies that had as an intervention MVD versus GKS in patients with

Microvascular Decompression with Selective Use of MVD-Plus for Classical Trigeminal Neuralgia: Outcomes, Predictors, and Role in Redo Surgery.

BACKGROUND: Classical trigeminal neuralgia (TN) is a severe facial pain disorder most commonly caused by neurovascular conflict (NVC) at the trigeminal root entry zone. Microvascular decompression (MVD) provides durable pain relief by addressing the underlying vascular compression. However, Indian data evaluating predictors of failure, early relapse, redo surgery, and the role of adjunct nerve combing (MVD-plus) remain limited. OBJECTIVE: To evaluate functional outcomes after MVD for classical TN and identify predictors of early failure, pain relapse, redo surgery, and the role of MVD-plus.
