Relevance of corpus callosum splenium versus middle cerebellar peduncle hyperintensity for FXTAS diagnosis in clinical practice.

Fragile X-associated tremor ataxia syndrome (FXTAS) is caused by FMR1 premutation. The features include ataxia, action tremor and middle cerebellar peduncle (MCP) hyperintensity, the latter being the only major radiological criterion in the diagnosis of definite FXTAS until very recently. The importance of corpus callosum splenium (CCS) hyperintensity was recently reported and this sign is now considered as an additional major radiological diagnostic criterion in the diagnosis of FXTAS. However, little is known about its relevance for the diagnosis of FXTAS in clinical practice. We report a pr

Rare central-variant of posterior reversible encephalopathy syndrome: Pons and middle cerebellar peduncles involvement.

Central variant of posterior reversible encephalopathy syndrome is an atypical clinical-radiological condition. We report an unusual case of a 37-year old woman with history of chronic renal insufficiency and hypertension. The patient presented with headache, dysarthria, dizziness and balance disturbance. Imaging findings include vasogenic edema of the pons and the middle cerebellar peduncles without supratentorial involvement. After treatment with antihypertensives and dialysis, her blood pressure and physical exam returned to normal and neurological symptoms resolved. Follow-up imaging showe

Wallerian degeneration of the middle cerebellar peduncles secondary to pontine infarction, case report, and review of literature.

Wallerian degeneration (WD) is a process of axonal degradation that occurs downstream from a primary injury site and is often seen after strokes affecting the brainstem. This case report documents WD of the bilateral middle cerebellar peduncles (MCPs) in an 81-year-old woman following chronic left paramedian pontine infarction (PPI). An ischemic stroke affecting the pons can trigger downstream axonal degeneration, known as Wallerian degeneration, which involves the structural breakdown of axons distal to the injury site. In this patient, magnetic resonance imaging (MRI) revealed symmetrically

Neuroimaging and clinical features of bilateral Wallerian degeneration of middle cerebellar peduncles subsequent to pontine infarction.

OBJECTIVE: Wallerian degeneration (WD) of the middle cerebellar peduncles (MCPs) following pontine infarction is a rare secondary degenerative neurological condition. Due to its infrequency, there is limited research on its characteristics. METHODS: This study aims to present three cases of WD of MCPs following pontine infarction and to analyze the prognosis, clinical manifestations, and neuroimaging features by amalgamating our cases with previously reported ones. RESULTS: The cohort consisted of 25 cases, comprising 18 men and 7 women aged 29 to 77 years (mean age: 66.2 years). The majorit

Criteria-unfulfilled multiple system atrophy at an initial stage exhibits laterality of middle cerebellar peduncles.

To elucidate clinically useful imaging characteristics of multiple system atrophy with predominant cerebellar ataxia (MSA-C) at the initial stage showing pure cerebellar ataxia but unfilling consensus criteria (MSA-pc), clinical and neuroradiological analyses on cerebral MRI and single-photon emission computed tomography (SPECT) for measuring regional cerebral blood flow (rCBF) were performed. Seven MSA-pc patients meeting the above condition at an initial evaluation were identified, and all the MSA-pc patients later developed autonomic dysfunction and finally fulfilled the criteria for probab

Cortical, subcortical and brain stem connections of the cerebellum via the superior and middle cerebellar peduncle in the rat.

The role of cerebellum in coordination of somatic motor activity has been studied in detailed in various species. However, experimental and clinical studies have shown the involvement of the cerebellum with various visceral and cognitive functions via its vast connections with the central nervous system. The present study aims to define the cortical and subcortical and brain stem connections of the cerebellum via the superior (SCP) and middle (MCP) cerebellar peduncle using biotinylated dextran amine (BDA) and Fluoro-Gold (FG) tracer in Wistar albino rats. 14 male albino rats received 20-50-nl

Surgical approaches to pathological lesions of the middle cerebellar peduncle and the lateral part of the pons - clinical observation.

BACKGROUND AND PURPOSE: Pathological lesions of the middle cerebellar peduncle include tumours, arteriovenous malformations, cavernous angiomas and spontaneous haematomas. Because of the very low incidence of these lesions, surgical approaches to the middle cerebellar peduncle are not commonly known. The authors decided to present their own experience based on five cases operated on at the authors' institution. MATERIAL AND METHODS: Five patients were operated on, four with tumours and one with spontaneous haematoma of the middle cerebellar peduncle. In three patients the cerebellomedullary f

Increased T2 signal in the middle cerebellar peduncles on MRI is not specific for fragile X premutation syndrome.

The fragile X premutation tremor/ataxia syndrome (FXTAS) is a recently described adult-onset neurodegenerative disorder, in which ataxia, tremor, cognitive decline, parkinsonism, neuropathy and autonomic dysfunction occur in various combinations. It is reported to display a characteristic MRI appearance, with increased T2 signal in the middle cerebellar peduncles and around the dentate nuclei. Diagnostic criteria have been proposed on the basis of these clinical and radiological features, presupposing that a CGG expansion has been demonstrated. We present three cases in which MRI and clinical

Bilateral Wallerian degeneration of the middle cerebellar peduncles due to paramedian pontine infarction: MRI findings.

Wallerian degeneration is a frequent finding in lesions of the pyramidal tract, but has been observed after damage of the other fibre systems as well. Few reports exist about Wallerian degeneration of cerebellar fibres after distant lesions to the axons. Here, we report on a patient who developed degeneration of both middle cerebellar peduncles after a paramedian pontine infarction.

Probable sporadic Creutzfeldt-Jakob disease with valine homozygosity at codon 129 and bilateral middle cerebellar peduncle lesions.

We describe a 67-year-old Japanese man with probable sporadic Creutzfeldt-Jakob disease (CJD) who had valine homozygosity at codon 129, a rarity in the Japanese. T2-weighted magnetic resonance imaging (MRI) detected high-intensity lesions in the bilateral middle cerebellar peduncles and basal ganglia as well as cerebellar and cortical atrophy. He developed cerebellar ataxia and subsequent mental deterioration, myoclonus, and periodic synchronous discharge as shown in an electroencephalogram. Cerebrospinal fluid examination showed a high level of neuron-specific enolase and a positive immunoass

Magnetic resonance imaging evaluation of Wallerian degeneration of bilateral middle cerebellar peduncles after pontine infarction.

We aimed to present a case of symmetrical Wallerian degeneration (WD) in the middle cerebellar peduncles (MCPs) after a unilateral paramedian pontine infarction, which was examined by multimodality magnetic resonance imaging (MRI). In addition, we summarize the small number of reported cases. In our clinic, we observed a case of symmetrical WD of bilateral MCPs that occurred 6 months after the onset of a pontine infarction. We searched the Wanfang (Chinese) and PubMed databases and found 23 reported cases of this condition with characteristic similar to our patient. From the 24 cases, the dete

[A case showing Wallerian degeneration of the bilateral middle cerebellar peduncles on MRI followed by the right pontine infarction].

We report a 63-year-old man who presented with the left facial palsy, the left hemiparesis, the left limb ataxia, and the bilateral truncal ataxia. On admission, magnetic resonance imaging (MRI) showed an abnormal high intensity lesion at the right paramedian region of the upper to middle pons on T2-weighted images (T2WI). He was diagnosed as having a pontine lacunar infarction. The contralateral cerebellar lesions were caused by involvement of the pontocerebellar fibers. On the 29th day from the onset, MRI showed the new abnormal high intensity lesions at the bilateral middle cerebellar pedun

Decreased signal intensity of the middle cerebellar peduncle on diffusion-weighted MRI in patients with large supratentorial lesions.

PURPOSE: The purpose of this work was to evaluate signal intensity change of the middle cerebellar peduncles on diffusion-weighted imaging in patients with large supratentorial lesions. METHODS: Signal intensity of the middle cerebellar peduncles was measured using a region of interest in 40 normal control subjects and in 28 patients with moyamoya disease on axial diffusion-weighted imaging with the motion-probing gradient perpendicular to the image. An additional six patients without moyamoya but with large supratentorial sequelae of hemorrhage or infarction were also included. RESULTS: The

Magnetic resonance T1w/T2w ratio in the middle cerebellar peduncle might be a sensitive biomarker for multiple system atrophy.

OBJECTIVE: We aimed to investigate the use of a myelin-sensitive MRI contrast, the standardized T1-weighted/T2-weighted (sT1w/T2w) ratio, for detecting early changes in the middle cerebellar peduncle (MCP) in cerebellar subtype multiple system atrophy (MSA-C) patients. METHODS: We included 28 MSA-C patients, including a subset of 17 MSA-C patients within 2 years of disease onset (early MSA-C), and 28 matched healthy controls. T1w and T2w scans were acquired using a 3-T MR system. The sT1w/T2w ratio in MCP was analyzed using SPM12 by utilizing a region-of-interest approach in normalized space.

Hypertrophic olivary degeneration concomitant with bilateral middle cerebellar peduncles Wallerian degeneration following unilateral pontine infarction.

BACKGROUND: Wallerian degeneration (WD) can occur in different projecting systems, such as corticospinal tract, dentate-rubro-olivary pathway, and corticopontocerebellar tract. However, the co-occurrence of hypertrophic olivary degeneration (HOD) and middle cerebellar peduncles (MCPs) degeneration secondary to unilateral pontine infarction in a single patient is extremely rare. CASE PRESENTATION: A 71-year-old man presented with acute onset of dizzness, slurred speech, and right-sided weakness. On the next day, his previous neurologic deficits deteriorated. Brain magnetic resonance imaging (M

Bright middle cerebellar peduncle sign in multiple system atrophy with predominant cerebellar ataxia is more apparent in double-inversion recovery imaging than in conventional imaging.

Multiple system atrophy (MSA) is a neurodegenerative disorder that presents as parkinsonism, cerebellar ataxia, and autonomic dysfunction. Magnetic resonance imaging (MRI) findings of MSA are reported to be the atrophy of the putamen/pons/cerebellum, hot cross bun sign, and bright middle cerebellar peduncle (MCP) sign. Here, we assessed the sensitivity of detecting the bright MCP sign in patients with MSA cerebellar variant (MSA-C) using a double inversion recovery (DIR) procedure, comparing it to the sensitivity of detection by T2-weighted image (T2WI) and fluid-attenuated inversion recovery

Diffuse Cortical, Basal Ganglia, and Bilateral Middle Cerebellar Peduncle Diffusion Restriction in Severe Alcohol-Related Hypoglycemic Encephalopathy: A Case Report.

Alcohol-induced hypoglycemia is an uncommon but potentially life-threatening metabolic complication, particularly in the setting of prolonged fasting and chronic alcohol use. We report the case of a 63-year-old man with a history of alcoholism who presented in a deep coma after binge drinking and reduced oral intake. On admission, severe hypoglycemia (30 mg/dL) was documented, prompting urgent metabolic correction and neuroimaging evaluation. Brain magnetic resonance imaging (MRI) subsequently demonstrated diffuse cortical diffusion restriction with additional involvement of the neostriatum, p

Progression of pathological changes in the middle cerebellar peduncle by diffusion tensor imaging correlates with lesser motor gains after pontine infarction.

BACKGROUND: Wallerian degeneration in pyramidal tract following supratentorial stroke has been detected by some studies using diffusion tensor imaging (DTI), but the Wallerian degeneration in middle cerebellar peduncle after pontine infarction and its potential clinical significance remain to be confirmed. METHODS: Seventeen patients with a recent focal pontine infarct underwent 3 DTIs at week 1 (W1), week 4 (W4), and week 12 (W12) after onset. Seventeen age-matched and gender-matched controls underwent DTI one time. Mean diffusivity and fractional anisotropy (FA) were measured in the basis p
