The impact of stigma on health care-seeking behavior in military personnel with mental health challenges.

Prolonged armed conflict has raised significant concern about the mental health of military personnel and veterans. Conditions such as post-traumatic stress disorder and depression affect approximately 14-16% of U.S. service members deployed to Afghanistan and Iraq. Other prevalent mental health issues include suicide, traumatic brain injury, substance use disorder, and aggression. Despite the availability of mental health services, stigma remains a major barrier to care, with nearly 60% of affected personnel avoiding treatment. This review examines how stigma influences help-seeking behavior

Fibromyalgia Diagnosis and Treatment Receipt in the U.S. Military Health System.

INTRODUCTION: Meta-analytic findings and clinical practice guidance recommend pharmacological (e.g., pregabalin, duloxetine, and milnacipran) and non-pharmacological (e.g., exercise and sleep hygiene) interventions to reduce symptoms and improve quality of life in people living with fibromyalgia. However, some of these therapies may lack robust evidence as to their efficacy, have side effects that may outweigh benefits, or carry risks. Although the annual prevalence of fibromyalgia in active duty service members was estimated to be 0.015% in 2018, the likelihood of receiving a fibromyalgia dia

The Burden of Arboviral Infections in the Military Health System 2012-2019.

Arboviral infections, including dengue (DNV), chikungunya (CHIKV), and Zika (ZIKV), impact both travelers and native populations of endemic regions. We sought to assess the disease burden of arboviral infections in the Military Health System, the validity of arboviral diagnostic codes, and the role of pretravel counseling on insect avoidance precautions. We searched for diagnostic codes consistent with arboviral infection and grouped them into DNV, CHIKV, ZIKV, Japanese encephalitis virus (JEV), and Other. Demographic data were evaluated. A subset of charts in each category were reviewed for d

The Gynecologic Health Consequences of Chlamydia trachomatis Infection in Military Servicewomen.

Chlamydia trachomatis is the most common sexually transmitted bacterial infection in the United States. Within the U.S. military, the age- and race-adjusted chlamydia infection rates among female service members are consistently higher than civilian rates, with a 20% annual acquisition rate among young active-duty women. The sequelae of chlamydia disproportionately impact women in terms of severity and cost. Untreated chlamydia progresses to pelvic inflammatory disease in 40% of cases, and is a leading cause of fallopian tube damage and pelvic adhesive disease resulting in ectopic pregnancy, t

Developing an Acoustic Sensing Yarn for Health Surveillance in a Military Setting.

Overexposure to high levels of noise can cause permanent hearing disorders, which have a significant adverse effect on the quality of life of those affected. Injury due to noise can affect people in a variety of careers including construction workers, factory workers, and members of the armed forces. By monitoring the noise exposure of workers, overexposure can be avoided and suitable protective equipment can be provided. This work focused on the creation of a noise dosimeter suitable for use by members of the armed forces, where a discrete dosimeter was integrated into a textile helmet cover.

Tuberculosis as a force health protection threat to the United States military.

Tuberculosis (TB) is a communicable disease that poses a threat to force health protection to the U.S. military. The rate of TB disease in the military is low; however, there are unique challenges for its control in this setting. As a low-risk population, TB testing in the U.S. military can be scaled back from the universal testing approach used previously. Reactivation of latent TB infection (LTBI) present at accession into service is the most important factor leading to TB disease; therefore, its diagnosis and treatment among recruits should be given a high priority. Deployment and overseas

An Analysis of the U.S. Department of Defense's Military Health Readiness Assessments.

Congress mandates that the Department of Defense (DoD) assess and monitor the health readiness of the armed forces. Accordingly, DoD implements a suite of health assessments to monitor service members' health readiness. One annual and four additional deployment-related health assessments screen for issues with physical and behavioral health at specified intervals throughout the deployment cycle to facilitate early intervention and any medical care required to maintain force readiness. The content of many of the items in these assessments overlap, and the required time frames for assessment com

Role of Flight Duration and Altitude in Ocular Health of Military Pilots.

BACKGROUND At great heights, reduced oxygen levels and barometric pressure can cause retinal damage, leading to high-altitude retinopathy. Military pilots additionally experience gravitational forces from rapid changes in speed and direction, which can impair vision due to short-term retinal ischemia. This study evaluated the association between flight duration and altitude with retinal and choroidal changes on optical coherence tomography (OCT) in 44 Polish military pilots. MATERIAL AND METHODS This study enrolled 88 adult men - 44 pilots from military bases in Poland and 44 controls recruite

Provision of social support and mental health in U.S. military veterans.

While social support has been linked to better health, most research has focused on the receipt of social support. In this study, we evaluated associations between provided support and mental health in a nationally representative cohort of 4069 US veterans. The majority (60-72%) of veterans reported providing support on a consistent basis. Veterans who scored higher on certain aspects of personality (i.e., agreeableness, conscientiousness, and extraversion) and received greater support were more likely to provide support. Further, each standard deviation increase in provided support was indepe
