[Kidney involvement in "mixed connective tissue disease" (Sharp syndrome)].

The case in question of the Sharp-syndrome is a mixed collagenosis with symptoms of sclerodermia, Erythematodes visceralis, dermatomyositis and rheumatoid arthritis. Apart from the clinical symptomatology the immunological profile is decisive for the ascertainment of the diagnosis. With the help of 3 descriptions of cases is referred to a frequent participation of the kidneys, particularly histologically and immune-histologically. In contrast to the hitherto existing publications also in missing pathological findings of the urine a renal biopsy should be aspired to, since important therapeutic

Mixed connective tissue disease with associated glomerulonephritis and hypocomplementaemia.

Mixed connective tissue disease is an increasingly recognized entity in which renal disease is thought to be unusual and associated hypocomplementaemia even more uncommon. A patient is described who had both these features and who also illustrates many of the characteristic features of this syndrome. The response to steroids of the systemic sclerosis component is well shown and an additional feature of interest is the family history of other connective tissue diseases.

Gastrointestinal systemic sclerosis in serologic mixed connective tissue disease.

Mixed connective tissue disease is a clinical entity defined by overlapping features of progressive systemic sclerosis, systemic lupus erythematosus, polymyositis, rheumatoid arthritis, and distinct serologic findings. Esophageal dilatation and dysmotility have been the only gastrointestinal manifestations reported. Three patients with serologic findings of mixed connective tissue disease and extensive gastrointestinal involvement compatible with the changes found in progressive systemic sclerosis are presented. Gastrointestinal manifestations of progressive systemic sclerosis are reviewed and

Mixed connective tissue disease in childhood. Relationship Sjögren's syndrome.

Mixed connective tissue disease (MCTD) seems to be a distinct entity that has some manifestations of systemic lupus erythematosus, scleroderma, polymyositis, and Sjögren's syndrome and is serologically characterized by the presence of an antibody to ribonucleoprotein. We report the cases of three children with MCTD with high titers of antibody to ribonucleoprotein. Two fulfilled criteria of lupus erythematosus, two had polymyosis; all three had suggestive features of scleroderma, fulfilled criteria for the diagnosis of juvenile rheumatoid arthritis, and had Sjögren's syndrome. Additional super

Mixed connective tissue disease: the spectrum of radiographic manifestations.

Mixed connective tissue disease (MCTD) is a serologically distinct entity defined by a ribonuclease-sensitive extractable nuclear antigen. This unusual overlap syndrome has clinical features of scleroderma, systemic lupus erythematosus, polymyositis, and rheumatoid arthritis. In order to define the radiographic changes in MCTD, radiographs of the hands of 17 patients were studied, utilizing a fine-detail technique. Diffuse and periarticular osteopenia were found in 8 and 10 patients, respectively; soft-tissue swelling in 11; erosive changes in 9; joint-space narrowing in 7; tuft resorption and

Mixed connective tissue disease: what is behind the curtain?

Although there is still an emotional debate over the existence of mixed connective tissue disease, the evidence from animal models suggests that anti-U1RNP antibodies, similar to other autoantibodies in other connective tissue diseases (such as antisynthetase, anticentromere, and antitopoisomerase), play a pathophysiological role in this disease. Despite an antiendothelial effect of anti-U1RNP antibodies, which is reminiscent of anticentromere antibodies, patients with high-titer autoantibodies to U1RNP in the absence of anti-Sm antibodies do not usually have or develop typical systemic sclero

[Mixed connective tissue disease and HELLP syndrome in pregnancy].

HISTORY AND ADMISSION FINDINGS: A 35-year-old woman (gravida 4, para 3) was known to have mixed connective tissue disease (MCTD-Sharp's syndrome) for five years, treated with immunosuppressives. She was in the 20 + 4 week of an unwanted pregnancy. DIAGNOSIS: On admission she had marked eclampsia with severe hypertension (180/130 mmHg), laboratory tests revealing hemolysis, elevated liver enzymes and thrombocytopenia, indicating an HELPP syndrome (severe eclampsia with hemolysis, elevated liver function and low platelets). TREATMENT AND COURSE: The symptoms worsened over a few days, the patie

[Second degree atrioventricular block in mixed connective tissue disease].

INTRODUCTION: Mixed connective tissue disease (MCTD) is characterized by overlapping features of progressive systemic sclerosis, dermatomyositis and systemic lupus erythematosus, and by high rate of antibodies to an extractable nuclear antigen ribonucleoprotein. Cardiac manifestations in MTCD are rare. EXEGESIS: A 58 years old man was admitted for mild fever, a impairment of the general status, muscular pain and a Raynaud's phenomenon. Biologic abnormalities were an inflammatory syndrome, elevated serum CPK and high rate anti-RNP antibodies. Diagnosis of mixed connective tissue disease was ma

Thrombotic thrombocytopenic purpura associated with mixed connective tissue disease.

We report a male case of thrombotic microangiopathy with mixed connective tissue disease (MCTD). Thrombocytopenia and hemolytic anemia developed despite steroid treatment for acute interstitial pneumonitis. The patient became confused and eventually developed coma. Diffusion-weighted MRI revealed high intensity at the brainstem, frontal lobes, basal nuclei, and insula, findings compatible with multiple brain edema due to microthrombosis. Despite the treatment of successive plasma exchange and steroid pulse therapy, he eventually died by multiple organ failure. This rare case suggested that dif

Juvenile-onset mixed connective tissue disease: longitudinal follow-up.

To establish the symptoms and clinical course of juvenile-onset mixed connective tissue disease, we studied 14 patients, classified according the criteria of Kasukawa et al. The patient records were studied retrospectively and all patients were examined in a 1-day follow-up program. Systemic lupus erythematosus and polymyositis/dermatomyositis-like symptoms disappeared in time, whereas scleroderma-like symptoms (such as in the Raynaud phenomenon) and joint abnormalities persisted. Extensive limitation of joint function was found in four patients. At the time of follow-up, no active renal disea

Cardiovascular manifestations of mixed connective tissue disease in adults.

To assess the nature and distribution of cardiovascular abnormalities associated with mixed connective tissue disease, we studied 38 patients with overlapping clinical manifestations of systemic lupus erythematosus, progressive systemic sclerosis and polymyositis, and circulating antibodies to nuclear ribonucleoprotein. The protocol included taking a medical history and a physical echocardiogram, and pulmonary function tests. Cardiac catheterization was performed on 17 patients. Postmortem examination was performed on four of the five patients who died during follow-up. Acute pericarditis and/

Fatal primary pulmonary hypertension in mixed connective tissue disease.

The clinical history and autopsy findings are reported of a 23 year old woman who had symptoms of mixed connective tissue disease (MCTD) for 4 years, and who died suddenly from pulmonary hypertension due to proliferative endarteritis of the lung vessels. These arterial lesions resembled those in progressive systemic sclerosis. Children and adolescents with MCTD appear to have an increased risk for getting this complication. The fatal outcome of pulmonary hypertension in MCTD as shown in this and other cases seems to warrant treatment with immunosuppressive drugs.

Cold-reactive lymphocytotoxic antibodies in mixed connective tissue disease.

Eighty-one sera from 18 patients with mixed connective tissue disease (MCTD) and high titers of antibody to ribonucleoprotein (RNP) were investigated for the presence of lymphocytotoxic antibodies. These were found in 59% sera from 14 patients but in only one of 40 normal subjects. Although lymphocytotoxic activity tended to be higher when the disease was active or there was lymphopenia, the correlation was not statistically significant. The lymphocytotoxic antibodies were found to be cold-reactive, and located in the IgM and/or IgG-containing elution fractions from DEAE cellulose columns. IgM

Pathology of skeletal muscle in mixed connective tissue disease.

To characterize the pathology of muscle involvement in mixed connective tissue disease (MCTD), skeletal muscle biopsies from 13 patients with MCTD were examined by routine light microscopy, histochemistry, and direct immunofluorescence. The histologic and histochemical changes observed corresponded closely to changes seen in idiopathic polymyositis and the myopathy associated with systemic lupus erythematosus. Eight of 13 cases examined by direct immunofluorescence demonstrated immunoglobulin deposition either within normal appearing vessels, within normal fibers, around or on the sarcoplasmic

Atypical Combination of Mixed Connective Tissue Disease and Multicentric Castleman Disease.

Castleman disease (CD) is a rare lymphoproliferative disorder with unicentric (UCD) and multicentric (MCD) forms, first detailed by Benjamin Castleman in 1956. It has three subtypes: hyaline vascular, plasma cell, and mixed. CD incidence is higher in HIV patients and is often associated with human herpes virus-8 (HHV-8). We report a 68-year-old woman with diabetes and mixed connective tissue disease (MCTD), which was diagnosed six months prior to presentation, who presented with lymphadenopathy, splenomegaly, and B symptoms. Imaging showed diffuse adenopathy. Biopsy confirmed the plasma cell s
