Regional differences in cortical benzodiazepine receptors of Alzheimer, vascular, and mixed dementia patients.

OBJECTIVE: We examined regional benzodiazepine receptors (rBZR) using single photon emission CT (SPECT) in patients with Alzheimer disease (AD), vascular dementia (VaD), and mixed AD/VaD dementia (MD) and compared the changes in the availability of rBZR with those of regional cerebral blood flow (rCBF). METHODS: A total of 7 patients with AD, 6 with MD, and 9 with VaD underwent SPECT studies with N-isopropyl-p-[(123)I] iodoamphetamine and (123)I-iomazenil to measure rCBF and rBZR. The ratios of rCBF and rBZR uptake in brain subregions to the average global activity were compared among these d

Vascular subcortical hyperintensities predict conversion to vascular and mixed dementia in MCI patients.

BACKGROUND AND PURPOSE: Patients with mild cognitive impairment (MCI) have an increased risk of dementia. The identification of predictors of conversion to dementia is therefore important. The aim of our study was to test the hypothesis that subcortical hyperintensities (SH) are associated with an increased rate of conversion to dementia in MCI patients. METHODS: This was an observational study on consecutive MCI patients attending a memory clinic. We assessed SH on a baseline MRI scan, using a semiquantitative rating scale. A multivariable Cox regression model was used to test the associatio

Development of a Dementia Case Management Information System App: Mixed Methods Study.

BACKGROUND: Case managers for persons with dementia not only coordinate patient care but also provide family caregivers with educational material and available support services. Taiwan uses a government-based information system for monitoring the provision of health care services. Unfortunately, scheduling patient care and providing information to family caregivers continues to be paper-based, which results in a duplication of patient assessments, complicates scheduling of follow-ups, and hinders communication with caregivers, which limits the ability of case managers to provide cohesive, qual

[Neurochemical markers of neurodegeneration in the early diagnosis of Alzheimer's disease, vascular and mixed dementia].

A study of 27 patients with dismnestic variant of moderate cognitive impairment (MCI) and 20 patients with neurodynamic-dysregulatory variant was carried out. The more intensive atrophic process supported by neuroimaging data and changed levels of neurodegeneration biomarkers - phosphorylated tau-protein 181 and beta-amyloid 42 - were found in patients with dismnestic MCI variant. The follow-up study showed that the large number of patients with progressive cognitive disorders associated with conversion to dementia was observed in this group. In conclusion, the determination of the neurochemic

Insight into dementia care management using social-behavioral theory and mixed methods.

BACKGROUND: For health organizations (private and public) to advance their care-management programs, to use resources effectively and efficiently, and to improve patient outcomes, it is germane to isolate and quantify care-management activities and to identify overarching domains. OBJECTIVES: The aims of this study were to identify and report on an application of mixed methods of qualitative statistical techniques, based on a theoretical framework, and to construct variables for factor analysis and exploratory factor analytic steps for identifying domains of dementia care management. Correlations between Gray Matter and White Matter Degeneration in Pure Alzheimer's Disease, Pure Subcortical Vascular Dementia, and Mixed Dementia.

Alzheimer's disease dementia (ADD) and subcortical vascular dementia (SVaD) both show cortical thinning and white matter (WM) microstructural changes. We evaluated different patterns of correlation between gray matter (GM) and WM microstructural changes in pure ADD, pure SVaD, and mixed dementia. We enrolled 40 Pittsburgh compound B (PiB) positive ADD patients without WM hyperintensities (pure ADD), 32 PiB negative SVaD patients (pure SVaD), 23 PiB positive SVaD patients (mixed dementia), and 56 normal controls. WM microstructural integrity was quantified using fractional anisotropy (FA), axia

Systematic Review of Cholinesterase Inhibitors on Cognition and Behavioral Symptoms in Patients of Chinese Descent with Alzheimer's Disease, Vascular Dementia, or Mixed Dementia.

Cholinesterase inhibitors (ChEIs) are the primary pharmacologic treatment for dementia. Their efficacy in patients of Chinese descent is not well described. We reviewed how ChEIs could affect cognition and behavioral and psychological symptoms (BPSD) in Chinese patients with Alzheimer's disease (AD), vascular dementia (VaD), or mixed (AD with vascular component) dementia. MEDLINE, PsycINFO, EMBASE and CINAHL were systematically searched for controlled trials of ChEIs, including donepezil, galantamine, and rivastigmine, for Chinese patients with AD, VaD, or mixed dementia. Outcomes for cognitio

Design and implementation of the participatory German network for translational dementia care research (TaNDem): A mixed-method study on the perspectives of healthcare providers and dementia researchers in dementia care research.

BACKGROUND: Currently, there is a lack of interaction between research and healthcare practice. As a result, research findings reach healthcare practice only late, and topics relevant to practice are often not known in research. Involving people living with dementia (PlwD), their relatives and healthcare providers in dementia care research can accelerate this process. For inclusion, firm and reliable structures are needed, which are to be established with the help of the Translational Network for Dementia Care Research in Germany. However, there is only limited knowledge about the priorities,

Primary Tauopathy With Logopenic/Semantic Mixed Progressive Aphasia and Frontotemporal Dementia-like Behavior.

Here we present the cases of two individuals with language and behavioral symptoms indicative of the early clinical manifestations of both Alzheimer disease (AD) and frontotemporal lobar degeneration (FTLD). Phonological language symptoms similar to those evident in logopenic variant primary progressive aphasia suggested AD pathology, while semantic impairment and behavioral changes (ie, abnormal eating behavior and disinhibition) suggested a diagnosis of FTLD. Multimodal neuroimaging studies revealed underlying neuropathology indicative of primary tauopathy with presumable 3/4-repeat isoform,

End-of-life experiences in individuals with dementia with Lewy bodies and their caregivers: A mixed-methods analysis.

BACKGROUND: Dementia with Lewy bodies (DLB) is one of the most common degenerative dementias, but research on end-of-life experiences for people with DLB and their caregivers is limited. METHOD: Dyads of individuals with moderate-advanced DLB and their primary informal caregivers were recruited from specialty clinics, advocacy organizations, and research registries and followed prospectively every 6 months. The current study examines results of caregiver study visits 3 months after the death of the person with DLB. These visits included the Last Month of Life survey, study-specific questions,

Morphometric imaging biomarker identifies Alzheimer's disease even among mixed dementia patients.

A definitive diagnosis of Alzheimer's disease (AD), even in the presence of co-morbid neuropathology (occurring in > 50% of AD cases), is a significant unmet medical need that has obstructed the discovery of effective AD therapeutics. An AD-biomarker, the Morphometric Imaging (MI) assay on cultured skin fibroblasts, was used in a double-blind, allcomers (ages 55-90) trial of 3 patient cohorts: AD dementia patients, N = 25, all autopsy confirmed, non-AD dementia patients, N = 21-all autopsy or genetically confirmed; and non-demented control (AHC) patients N = 27. Aging and cerebrovascular lesions in pure and in mixed neurodegenerative and vascular dementia brains: a neuropathological study.

INTRODUCTION: The prevalence of dementia is increasing in our aging population. Because of the complexity of disease pathology, dementia classifications remain controversial. The present post-mortem study investigates whether there are age differences between dementia brains with a single pure neurodegenerative or cerebrovascular disease and those with mixed pathological features. Also, the impact of these vascular lesions is compared. MATERIAL AND METHODS: A total of 132 dementia brains with a pure neurodegenerative or cerebrovascular disease and 84 with mixed features were examined. Main ag

Use of low-dose gabapentin for aggressive behavior in vascular and Mixed Vascular/Alzheimer Dementia.

Recent reports highlighting serious adverse effects of antipsychotic medication in behavioral and psychological symptoms of dementia (BPSD) has led to calls for research on alternative agents. The authors describe the use of low-dose gabapentin to treat seven patients with a diagnosis of ICD-10 vascular or Mixed Vascular/Alzheimer Dementia with serious aggressive behavior. All seven patients had impressive and clinically significant responses to treatment. Treatment was tolerated in each case without adverse reactions. Our findings suggest that gabapentin should be considered for treating aggr

CSF biomarkers, impairment of cerebral hemodynamics and degree of cognitive decline in Alzheimer's and mixed dementia.

The in vivo diagnosis of Alzheimer's disease (AD) may be facilitated by cerebro-spinal fluid (CSF) biomarkers in combination with imaging and clinical assessments. By determining the concentration of beta amyloid fragments, total tau (t-tau) and phospho-tau (p-tau), it is possible to detect the conversion of mild cognitive impairment (MCI) to AD or distinguish AD vs. pseudo-dementia. However, these markers are poorly sensitive to the progressive disease stages. And far from clear is their role in "mixed" forms of dementia, as far as hemodynamic deficits complicate the clinical history. We have

Oxidative stress and oxidative DNA damage is characteristic for mixed Alzheimer disease/vascular dementia.

Oxidative DNA damage may contribute to neuronal cell loss and may be involved in pathogenesis of some neurodegenerative diseases. We assessed the broad spectrum of oxidative DNA damage biomarkers and antioxidants in mixed Alzheimer disease/vascular dementia (MD) and in control patients. The amount of the products of oxidative DNA damage repair (8-oxo-2'-deoxyguanosine and 8-oxoguanine) excreted into urine and cerebrospinal fluid (CSF) was measured by gas chromatography/mass spectrometry with HPLC pre-purification. The level of 8-oxo-2'-deoxyguanosine in leukocytes' DNA, antioxidant vitamins an
